Tenders Are Invited For Providing Medical Services In The Field Of Occupational Medicine For Employees Of The Wielkopolska Regional Branch And Arimr District Offices.

The Subject Of The Order Are Preventive Examinations (Initial, Periodic And Control) Of Employees Of The Wielkopolska Regional Branch And Arimr District Offices. Examinations Should Be Understood As Both Their Performance, Receipt Of Their Results, And The Issuance Of A Certificate By An Occupational Medicine Doctor. The Contractor Is Responsible For Determining The Scope Of Tests Necessary To Issue A Certificate Of Fitness For Work For Each Group Of Employees Described By The Ordering Party. The Ordering Party Requires That The Examination By An Occupational Medicine Doctor Be The Last Examination Taking Place After Receiving All The Test Results. The Occupational Medicine Doctor Is Obliged To Issue A Medical Certificate To The Employee And The Employer For The Purposes Specified In The Labor Code. The Description Of Positions And Working Conditions For Individual Groups Of The Ordering Partys Employees Subject To Examination Is Provided In Annex No. 3 To The Swz. The Subject Of The Order Is Also The Obligation Of The Doctor Providing Preventive Health Care For Employees To Participate (Once Per Quarter) As A Member Of The Health And Safety Committee Appointed By The Ordering Party. In This Respect, The Doctor Is Obliged To Perform The Following Tasks: • Periodically Assess The State Of Occupational Health And Safety, • Provide Opinions On Measures Taken By The Ordering Party To Prevent Accidents At Work And Occupational Diseases, • Formulate Proposals Regarding The Improvement Of Working Conditions, • Cooperate With The Ordering Party In The Implementation Of Its Obligations In The Field Of Occupational Health And Safety. In Addition, The Subject Of The Order Is For A Doctor Providing Preventive Health Care To Review Working Conditions For Employees, Depending On The Needs And Recommendations Of The Doctor. The Inspections Will Be Carried Out Together With The Ordering Partys Employee Responsible For Health And Safety, And The Cost Of Transport To The Indicated Locations Is Borne By The Ordering Party. The Subject Of The Order Is Also Protective Vaccinations - Against Tetanus And Against Ticks - For 50 Employees Of The Ordering Party, As Well As Sanitary And Epidemiological Tests: With Feces Testing For Carrier Status For 20 Employees And Without Stool Testing For 40 Employees Of The Ordering Party. After The Examination, The Occupational Medicine Doctor Is Obliged To Issue To The Employer A Medical Certificate From The Examination Carried Out For Sanitary-Epidemiological Purposes. 10. If An Employee Of The Ordering Party Does Not Have A Sanitary And Epidemiological Booklet, The Contractor Is Obliged To Provide Such A Booklet To The Employee Along With The Entry Made After The Tests, Identical To The Issued Decision.
