The Subject Of The Order Is The Provision Of Medical Services To Persons Employed And Employees Of The Ministry Of Agriculture And Rural Development In Warsaw In The Scope Of: 1) Performing Preventive Medical Examinations (Initial, Periodic, Control) In The Field Of Occupational Medicine, Including Radiological And Laboratory Tests And Diagnostics, Specialist Medical Consultations And Vaccinations Required By Sanitary Regulations, Referred To In The Contractors Offer Constituting Annex No. 2 To The Contract, And Other Additional Tests Not Listed In The Contractors Offer, Hereinafter Collectively Referred To As Research, 2) Ensuring The Participation Of An Occupational Medicine Doctor In The Occupational Health And Safety Committee Operating At The Ordering Party, To The Extent Resulting From The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2023, Item 1465, As Amended .) And Internal Regulations In Force At The Ordering Party, Hereinafter Also Referred To As Services. 3) Collecting And Storing Information On The Health Condition Of Persons Covered By Preventive Health Care.
