1. The Subject Of The Order Is A Comprehensive Cleaning Service In The Facilities Of The Park Avia Sports And Recreation Complex And The Avia Zielony Lof Park Area Located At Ul. Fabryczna In Świdnik, 21-040 Świdnik And At Al. Lotników Polskich And Ul. Firefighters Street. 2. Internal Area Covered By Cleaning - 1933M2 - 2633M2 Depending On The Period (1933M2 Applies To The Period From 01/01/2025 To 31/05/2025 And From 08/09/2025 To 31/12/2025; 2633M2 Applies To The Period From 01/06/2025 To 07/09 .2025 .).3. The Ordering Party Provides For The Possibility Of An On-Site Visit. 4. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 9 To Swz.5. The Ordering Party Specifies That Activities Directly Related To The Implementation Of The Subject Of The Order, I.E. Cleaning Services, Are To Be Performed By Persons Employed Under An Employment Contract For The Entire Duration Of The Contract. The Obligation To Employ Under An Employment Contract Does Not Apply To A Person Acting As A Coordinator Assigned By The Contractor To The Facility That Is The Subject Of The Service Provided. In Order To Verify The Employment By The Contractor Or Subcontractor Under An Employment Contract Of Persons Performing The Above-Mentioned Activities, The Ordering Party Provides For The Possibility Of Requesting During The Execution Of The Order In Particular: 1) Declarations Of The Contractor Or Subcontractor On The Employment Of An Employee Under An Employment Contract, 2) A Certified True Copy Of The Employment Contract Of The Employed Employee, 3) Other Documents - Containing Information, Including Personal Data, Necessary To Verify Employment Under An Employment Contract, In Particular: Name And Surname Of The Employed Employee, Date Of Conclusion Of The Employment Contract, Type Of Employment Contract, Scope Of The Employees Responsibilities. The Ordering Party, No Later Than Three Days Before The Commencement Of The Service Provision, Requests That The Contractor Submit A List Of Persons Employed Under The Employment Contract Work To Implement The Subject Of The Order. The List Is To Include The Following Information: Names And Surnames, Dates Of Conclusion Of Contracts, Type Of Employment Contracts And Working Hours Of Persons Employed Under An Employment Contract. The Ordering Party May Request An Inspection By The National Labor Inspectorate. The Ordering Party Also Reserves The Right To Request Anonymized, Certified Copies Of Employment Contracts Of Employed Employees And Anonymized Certificates Of The Social Insurance Institution Or The Agricultural Social Insurance Fund, Confirming The Payment Of Social And Health Insurance Contributions By The Contractor Or Subcontractor For Employment Under Employment Contracts. .
