4.1. The Subject Of The Order Is: • Dismantling, Transport And Disposal Of Products Containing Asbestos From 9 Properties Owned By Natural Persons From The Głowno Commune In The Amount Of Approximately 63.296 Mg; • Collection, Transport And Disposal Of Products Containing Asbestos From 79 Properties Owned By Natural Persons From The Commune Mainly In The Amount Of Approx. 411,696 Mg; As Part Of The Execution Of The Contract, The Contractor Before The Start Work Is Obliged To Agree The Date And Time Of Collection Of Products Containing Asbestos With The Owner Of The Property From Whom The Service Is To Be Performed. The Contractor Is To Ensure The Transport Of Waste To A Hazardous Waste Landfill Located In The Łódź Voivodeship (Pursuant To The Act Of April 27, 2001, Law Environmental Protection On Limiting The Adverse Impact On The Environment Due To The Distance From The Landfill, Pursuant To The Act Of December 14, 2012 On Waste, Art. 20 Sections 1 And 2). After Completing The Service, The Contractor Will Present To The Ordering Party: A) A Draft Work Acceptance Report Confirming The Performance Of The Service, B) Reports Confirming The Receipt Of Asbestos Boards Confirming The Amount Of Asbestos Waste To Be Disposed Of In Mg, Signed By The Contractor And The Property Owner, At Which The Service Was Performed, C) A Statement On The Proper Performance Of The Work In Compliance With Appropriate Technical And Sanitary Regulations (In Accordance With The Regulation Of The Minister Of Economy, Labor And Social Policy In On Methods And Conditions For The Safe Use And Disposal Of Products Containing Asbestos (Journal Of Laws Laws Of 2004, No. 71, Item 649, As Amended. Amended), D) Originals Of Transfer Notes For Waste Containing Asbestos For Disposal At An Appropriate Asbestos Waste Landfill (Pursuant To The Waste Act, The Producer Of Hazardous Waste Is The Service Contractor, Unless The Contract Provides Otherwise). It Is The Manufacturers Obligation To Keep Waste Records Using The Following Documents: Waste Record Cards, Waste Transfer Cards).4.2. On-Site Visit: The Ordering Party Informs That The Submission Of The Offer Will Be Preceded By An On-Site Visit Or Checking The Documents Relating To The Order At The Ordering Partys Disposal And Which Will Be Made Available To Entities Willing To Participate In The Procedure. In Order To Arrange An On-Site Visit Or Review The Documentation Available On Site At The Ordering Partys, Please Contact The Persons Designated To Communicate With The Contractors. The Cost Of The On-Site Visit Is Borne By The Contractor. Submitting An Offer Without Conducting An On-Site Visit Or Without Checking The Documents Necessary To Complete The Order Available On-Site At The Ordering Party Will Result In Its Rejection In Accordance With Art. 226 Section 1 Point 18 Pzp.
