Tenders are invited for Supply of Laboratory Consumables Applicants should Pay a Non-Refundable Fee of Rs. 1,000.00 in Cash, for each Supply / Service for Which Registration is Sought, to the Accountant of the Colombo East Base Hospital and the Receipt Form General 172 Obtained should be Attached to the Application. Money Orders will not be Accepted. Suppliers should Agree to Supply Goods and Services on at Least One Months Credit Basis and the Transport of Goods to the Stores should be Provided by the Supplying Institution Self-Prepared Applications as Per the Specimen Given in No. 05 below may be Submitted after Perfecting the Application to the Address Given below. The Caption, "Registration for Supplies and Services for Year 2025" and the Ser. No. Under (a) if Intending to Supply Goods and Ser. No. Under (B) if Intending to Supply Services (Example: A01, b32) should be Mentioned on the Left-Hand Top Corner of the Envelope Containing the Application.
