The Subject Of The Order Is: Adaptation Of A Gymnasium For The Needs Of A Medical Caregivers Workshop Along With The Renovation Of Adjacent Rooms And Retrofitting Of Subject Laboratories In The Medical And Social Center For Vocational And Continuing Education In Jasło As Part Of The Regional Program European Funds For Podkarpacie 2021-2027, Fepk Priority .05 Friendly Social Space, Fepk Activity.05.01 Education2. The Scope Of Works Covered By The Order Includes: Renovation Of Rooms: Hall In Front Of The Secretariat, Room No. 8, Service Room, Dental Hygienists Laboratory With A Corridor And Bathroom, Renovation Of The Main Hall Of The School And Adaptation Of The Gymnasium Into A Medical Assistants Laboratory At The Medical And Social Center For Vocational And Continuing Education In Jasło.3. The Detailed Scope Of Works Is Specified In The Attached Technical Documentation Together With The Technical Specification For The Execution And Acceptance Of Works (Stwior) And The Bill Of Quantities (Indicative Only), Constituting Annex No. 4 To The Siwz.4. Equivalence: A) The Ordering Party, In The Case Of Describing The Subject Of The Contract Using Trademarks, Standards, Patents Or Origin, Source Or Specific Process Referred To In Art. 101 Section 4-5 Of The Act, Indicates That It Allows Solutions Equivalent To Those Described In Terms Of The Adopted Technical Solutions, Standards, Materials Used, Products, Standards, Provided That Those Proposed In The Offer Are Significantly Equivalent To Those Specified In The Design Documentation. B) If The Description Of The Subject Of The Order Includes Any Trademarks, Patents, Standards Or Other Reserved Or Exclusive Rights, Or The Origin Of The Subject Of The Order Or Its Part Is Specified - It Should Be Assumed That The Ordering Party, Due To The Specificity Of The Subject Of The Order, Has Provided Such A Description With An Indication Of The Type And Allows The Submission Of Equivalent Offers In Terms Of Functionality And With Technical And Operational Parameters Not Worse Than Those Given In The Description Of The Subject Of The Order. If An Offer Is Submitted Containing The Use Of Solutions Equivalent To Those Described By The Ordering Party, The Obligation To Prove Their Equivalence With The Requirements Specified By The Ordering Party Rests With The Contractor. In Such A Case, The Contractor, Together With The Offer (Not Subject To Supplementation), Will Submit A List Of Equivalent Materials Together With Other Necessary Documents Confirming The Fulfillment Of The Above-Mentioned Requirements, E.G.
