Subject Of The Order1. The Subject Of The Order Is The Provision Of Specialized Residential Care Services For People With Mental Disorders (Hereinafter Also Referred To As: Suo) From The Czernica Commune, Divided Into 5 Parts, Described In Detail In Annex 1 To This Swz. 2. The Scope Of The Subject Of The Order Includes The Provision Of Specialist Care Services In The Place Of Residence For People With Mental Disorders Resulting From Art. 18 Section 1 Point 3 Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2023, Item 901, As Amended) In Connection With From § 2 Of The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialist Care Services (Journal Of Laws No. 189, Item 1598, As Amended). 3. The Scope Of Services Provided Includes The Provision Of At Least One Of The Services Indicated Below, In Accordance With The Scope Specified For Each Part Of The Order In Annex 1 To This Tor: Part 1 Name Of The Service: Physiotherapy Monthly Number Of Hours Of Services (Not More Than): 12 Hours Clock Annual Number Of Hours Of Services: 48 Clock Hours Client: A Person Requiring Assistance In The Form Of Specialized Services Resulting From The Type Of His Or Her Illness Or Disability Place Of Service Provision: Place Of Residence Of A Person With A Disability - In The Czernica Commune Type Of Illness: Mental Disorders - In Accordance With Art. 3 Point 1 Of The Act Of August 19, 1994 On Mental Health Protection. Scope Of Services: 1) Service No. 2: Care - As Support For The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arrangements For Monitoring Appointments Medical Examinations, Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At The Pharmacy, D) Monitoring The Intake Of Medicines And Monitoring Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Auxiliaries And Medical Materials, Orthopedic Items, As Well As In Maintaining Hygiene, Access To Health Services, F) Assistance In Getting To Health Care Facilities, G) Assistance In Getting To Rehabilitation Facilities.2) Service No. 3 Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act On Of August 27, 2004 On Health Care Services Financed From Public Funds: A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical, Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services.
