The Order Includes: 1) Submitting An Application To The Municipal Office In Zabrze (Construction Department) For A Decision On Determining The Location Of A Public Purpose Investment For The Planned Investment Called Construction Of A Low-Parameter Heating Network For Central Heating And Hot Water On The Premises Of The Specialist Hospital In Zabrze Sp. Z O. O. (If Necessary).2) Pre-Design And Design Works, Including Formal And Legal Services On Behalf Of The Investor, And Obtaining A Decision On A Building Permit Or Notification And Use Of The Facility In The Above-Mentioned Investment (If Necessary).3 ) The Necessary Design Documentation Should Include The Following Industries: A) Installation.4) Construction Of A Turnkey Linear Facility Of The Heating Network Along With Formal And Legal Support On Behalf Of The Investor.5) Undertaking All Necessary Activities On Behalf Of The Investor Not Included In Points 1-4 , And Possibly Necessary To Carry Out The Full Implementation Of The Turnkey Investment Called Construction Of A Low-Parameter Heating Network For Central Heating And Hot Water On The Premises Of The Specialist Hospital In Zabrze Sp. Z O. O.6) The Ordering Party Informs That It Excludes The Following Scope Of Works From The Functional And Utility Program (Page No. 7): The Reconstruction Works Should Include The Construction Of A New Road Surface And A Sidewalk Made Of Concrete Cubes (The Scope Of Construction Of The New Road And Sidewalk Is Shown On The Overview Map Constituting Annex No. 3). The Road Should Be Designed And Constructed For Traffic Category Kr1. This Means That As Part Of The Reconstruction Works, The Contractor Will Restore To Its Original Condition Only The Existing New Paved Surfaces That Will Be Necessary For Dismantling During The Installation Works Of The Heating And Hot Water Networks.
