The Subject Of The Order Is The Purchase And Delivery Of 1,018 Office Chairs For The Needs Of The Social Insurance Institution Branch In Lublin And Subordinate Local Units, In Accordance With The Detailed Description Of The Subject Of The Order And The Terms And Conditions Of The Order Included In The Contract Template, Constituting Annexes No. 1 And 5 To The Swz. The Subject Of The Order Will Be Implemented At The Following Locations: 1) Zus Branch In Lublin, Ul. Tomasza Zana 36, 38C, 20-601 Lublin 2) Zus Inspectorate In Puławy, Ul. Lubelska 20B, 24-100 Puławy, 3) Zus Inspectorate In Kraśnik, Ul. Fr. J. Popiełuszki 3A, 23-210 Kraśnik 4) Zus Inspectorate In Lubartów, Ul. Lipowa 6, 21-100 Lubartów 5) Zus Inspectorate In Radzyń Podlaski, Al. Armii Krajowej 22/32, 21-300 Radzyń Podlaski 6) Zus Inspectorate In Biała Podlaska, Ul. Sadowa 23, 21-500 Biała Podlaska 7) Zus Inspectorate In Łuków, Ul. Cardinal S. Wyszyńskiego 56, 21-400 Łuków 2. The Scope Of The Order Includes: 1) Delivery, Unloading And Bringing In Office Chairs At The Contractors Own Expense And Risk, In Individual Locations Of The Ordering Party. 2) Placement Indoors, In Places Indicated By The Ordering Party. 3) Arranging The Collection Of Packaging As Well As The Collection And Disposal Of Old Armchairs On Your Own And At Your Own Expense. 3. The Place Of Delivery And The Number Of Office Chairs Delivered, As Well As The Collection And Disposal Of Old Chairs, Are Determined By The Opz Constituting Annex No. 1 To Swz 4. The Contractor Is Obliged To Provide A Guarantee And An Equivalent Warranty For The Subject Of The Order. The Ordering Party Specifies Them For The Period Of: 36 Months For The Seats And 48 Months For The Actuator (Lifting Mechanism), Counting From The Date Of Signing By The Parties Of The Acceptance Report For The Quantitative And Qualitative Execution Of The Subject Of The Order Without Any Reservations, And In The Event Of Any Defects, From The Date Of Signing The Final Acceptance Report Containing Confirmation Of Their Removal. Defects.
