1. The Subject Of The Order Are Construction Works Related To Restoration And Conservation Works At The Monument - A Multi-Family Residential Building At Ul. Kościuszki 6 In Myszków.2. The Subject Of The Order Includes The Following Construction Works To Be Performed: - Replacement Of The Roof Truss And Roof Covering, - Strengthening Of The Ceiling Above The Second Floor, - Construction Of New Partition Walls Made Of Plasterboards In The Attic, - Thermal Insulation Of The Roof, - Thermal And Acoustic Insulation Of The Ceiling Above The Second Floor. First Floor, - Gypsum Fiber Board Floors With Ceramic Tiles And Floor Panels, - Thorough Renovation Of The Staircase With Replacement Of Parts Of Wooden Treads, Wooden Handrails And Cast Iron Balustrade Balusters, - Renovation Of Basement Rooms, - Replacement Of Parts Of Windows And Doors, - Replacement Of Parts Electrical Installations With New Fixtures, - Replacement Of Parts Of The Water And Sewage Installation, - Replacement Of The Entire Lightning Protection Installation.3. The Subject Of The Order Is Described In Detail In Annex No. 1 To The Swz - Detailed Description Of The Subject Of The Contract And In Annex No. 2 To The Swz - Documentation For The Execution Of The Order.4. Adaptation Of The Facility To The Needs Of Disabled People: According To The Construction Design, The Residential Building Is Not Adapted To The Needs Of Disabled People. Access To The Upper Floors Is Only Via Stairs. The Building Is Not Equipped With An Elevator Or Other Devices Enabling Disabled People To Travel To Higher Floors. The Subject Of The Order Therefore Includes Construction Works (Restoration And Conservation Works, Renovation And Reconstruction Works With A 1:1 Layout), Which, Due To Their Scope, Do Not Require Adapting The Building To The Requirements Of The Regulation Of The Minister Of Infrastructure On The Technical Conditions To Be Met By Buildings And Their Location. In This Case, The Silesian Provincial Conservator Of Monuments, By Resolution No. C-Nr-5142.52.2024.Dsz Of April 17, 2024, Agreed On The Investment Planned To Be Implemented Within A Multi-Family Residential Building - A Historic Tenement House Located In Myszków At Ul. Kościuszki 6, On Plot 1540/2, Specified In The Design Documentation, An Integral Part Of Which Is The Expert Opinion On The Condition Of The Existing Building, Agreed On The Scope Of Work Referred To Above. Then, By Decision Of The Starosta Of Myszków No. 194/24 An.6740.129.2024 Of April 24, 2024, The Land Development Design And The Architectural And Construction Design Were Approved And A Building Permit Was Granted. 5. Type Of Order: Construction Work.6. Cpv Codes:1) Main Cpv Code: 45261000-4: Roof Coverings And Structures And Similar Works2) Additional Cpv Code: 45111300-1: Demolition Works, 45453100-8: Renovation Works, 45410000-4: Plastering, 45223500-1: Reinforced Concrete Structures, 45421000-4: Joinery Works, 45430000-0: Floor And Wall Coating, 45262100-2: Scaffolding Works, 45310000-3: Electrical Installation Works, 45330000-9: Water And Sewage Installation Works And Sanitary.7. In Each Case When The Description Of The Subject Of The Contract Refers To The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Section 1 Point 2 And Section 3 Of The Public Procurement Law, The Ordering Party Allows Solutions Equivalent To Those Described, And Such Reference Is Accompanied By The Words Or Equivalent. Other Information Regarding Equivalent Solutions Is Described In Chapter 5 Of The Swz.8. The Ordering Party Does Not Reserve The Obligation For The Contractor To Personally Perform Key Tasks, In The Manner Specified In Art. 121 Of The Public Procurement Law. 9. The Contractor May Entrust The Execution Of Part Of The Order To A Subcontractor. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts (Scopes) Of The Order The Performance Of Which He Intends To Entrust To Subcontractors, And That The Contractor Provide The Names Of Any Subcontractors, If They Are Already Known. For This Purpose, The Contractor Should Provide Appropriate Information In The Offer Form, Constituting Annex No. 3 To The Swz. If The Contractor Does Not Intend To Perform The Order With The Participation Of Subcontractors, Please Enter Not Applicable Or Other Similar Wording In The Appropriate Place In The Offer Form Table.10. Entrusting The Performance Of Part Of The Order To Subcontractors Does Not Release The Contractor From Responsibility For Proper Performance Of The Order.11. In These Proceedings, The Ordering Party Does Not Allow The Submission Of Partial Offers.12. Reasons For Not Dividing The Awarded Contract Into Lots. Due To The Scope Of Works Located On A Small Area (Within One Building), It Is Inappropriate To Divide The Order Into Parts In Which The Entire Task Would Be Divided Into Parts In An Artificial And Irrational Manner For Economic Reasons, Contrary To The Technology And Rational Organization Of Construction Works On Individual Areas. Sections Of Works. In The Ordering Partys Opinion, Objective And Justified Reasons Support Awarding The Contract In Question To One Contractor, And The Lack Of Division Into Parts Will Not Impede Fair Competition Resulting In A Reduction In The Participation Of Small And Medium-Sized Enterprises In The Proceedings. Moreover, In Accordance With The Provisions Of The Preliminary Promise Received Under The Government Program For The Protection Of Monuments, The Ordering Party Must Announce A Purchase Procedure To Select The Contractor(S) For The Investment - Within A Period No Longer Than 12 Months From The Date Of Making The Preliminary Promise Available By Bgk. In The Event Of The Execution Of The Contract In Question Divided Into Parts, There Is A Risk Of Selecting The Contractor Only For Some Of These Parts, And The Co-Financing Conditions Require The Implementation Of The Entire Scope Of The Subject Of The Contract. In This Situation, Failure To Conclude Contracts In The Next Initiated Procedure(S) For The Still Unresolved Parts Of The Contract In Question May Lead To The Loss Of The Entire Funding Amount (Expiration Of The Initial Promise), Because The Ordering Party May No Longer Have Enough Time To Use The Period Of Time Indicated In The Initial Promise. To Make Further Attempts To Award The Contract In The Entire Scope Covered By The Funding Application.
