The Average Forecast Number Of Individual Types Of Brick Graves During The Planned Order Implementation Period (3 Years) Is: 1) Brick Grave For A Single Urn - 1 Piece, 2) Single Brick Grave - 18 Pieces, 3) Double Brick Grave - 51 Pcs.4) Triple Brick Grave – 1 Pc.4. Services Preceding The Construction Of Brick Graves As Part Of Funeral Services: 1) Checking, Together With The Cemetery Administrator, Whether In The Indicated Place There Are No Remains Of A Deceased Person Buried Within A Period Of Less Than 20 Years From The Commencement Of Work, Except For The Circumstances For Which Consent Was Granted Sanitary Inspector, 2) When Starting To Perform The Subject Of The Order, The Contractor Should Be Prepared For The Possibility Of Excavations That Are Subject To Special Protection, Such As: Human Remains, Mementos Of The Deceased Or Items Constituting Historical Monuments. The Administrator Of Cemeteries Should Be Immediately Notified About Any Such Situation, 3) In The Case Of Constructing A Brick Grave In A Place Where There Are Human Remains Buried Earlier Than 20 Years And The Contractor Is Informed About This Situation Before Starting The Work, All Excavated Remains Should Be Cleaned And Placed In Container, Bag Or In A Manner Indicated By The Family Of The Deceased Or The Cemetery Administrator. After This Procedure, The Remains Are Placed Below The First Level Of The Brick Grave Or In A Place Indicated By The Cemetery Administrator, 4) After Completing The Above Activities, The Contractor May Start Constructing A Structure Of A Specific Structure And Purpose. Attention! As Part Of The Execution Of The Order, Brick Graves Must Be Completed Within The Deadline Specified By The Ordering Party, Regardless Of The Prevailing Weather Conditions.5. The Subject Of The Order Was Described In Detail In Accordance With Art. 103 Of The Act Using Technical Drawings, Bills Of Quantities And Stwiorb, Which Constitute Annexes To The Swz. Pursuant To Art. 101 Section 4 Of The Public Procurement Law, The Ordering Party Allows Solutions Equivalent To Those Described In The Order Documents, Stwiorb Using Standards, European Technical Assessments, Approvals, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Section 1 Point 2 And Section 3 Of The Public Procurement Law. 6. The Ordering Party Requires The Contractor To Provide A Warranty For The Completed Subject Of The Order (Brick Graves) For A Period Of Not Less Than 12 Months, Counting From The Date Of Final Acceptance Of Each Brick Grave Or A Protocol Stating That The Defects Identified During The Acceptance Of The Brick Grave Have Been Removed. The Warranty Period For The Completed Subject Of The Order Is One Of The Criteria For Evaluating Offers And Will Be Assessed In Accordance With The Principles Specified In Swz.7. The Ordering Party Requires The Contractor To Provide A Warranty For The Completed Subject Of This Order (Brick Graves) For A Period Of 36 Months, Counting From The Date Of Acceptance Of Each Brick Grave Or A Protocol Stating That The Defects Identified During The Acceptance Of The Brick Grave Have Been Removed.8. The Ordering Party Requires That The Fleet Used By The Contractor To Complete The Order Contains At Least 10% Of Vehicles Powered By Electricity Or Natural Gas.9. The Ordering Party Does Not Allow The Submission Of Partial Offers. The Reason For Not Dividing The Order Is That The Frequency Of Works Is So Low That Dividing The Order Into Parts Is Completely Unjustified. The Processing Capacity Of One Unit Is Completely Sufficient To Carry Out The Subject Of The Order. Splitting The Contract Would Require Two Or More Contractors To Be Called On To Perform The Same Job, Which Would Inevitably Result In Contractor Chaos At The Job Site.10. The Ordering Party Does Not Allow The Submission Of Variant Offers Or Offers In The Form Of Electronic Catalogues.11. The Ordering Party Does Not Envisage Awarding Contracts Referred To In Art. 214 Section 1 Point 7 Of The Pzp.12 Act. The Contractor Is The Owner And Producer Of Waste Generated In Connection With The Execution Of The Order. Therefore, The Contractor Is Subject To Obligations Arising From The Act Of December 14, 2012 On Waste. Therefore, The Disposal Of The Generated Waste Should Be Carried Out In Accordance With The Above. Act And Include Its Cost In The Offer Price.13. The Ordering Party Does Not Require Any Means Of Evidence To Confirm The Compliance Of The Offered Construction Works With The Requirements, Features Or Criteria Specified In The Description Of The Subject Of The Order Or The Description Of The Offer Evaluation Criteria, Or The Requirements Related To The Implementation Of The Order.14. The Conditions For The Implementation Of The Subject Of The Order Are Indicated In The Contract Template. Annex No. 8 To Swz.15. This Procedure Provides For A Lump Sum Settlement. The Bill Of Quantities Attached To The Swz (Appendix No. 10) Is For Information Purposes And Does Not Constitute A Summary Of All Planned Works And All Expected Costs Related To The Execution Of The Subject Of The Order. Works Not Included In The Bill Of Quantities But Appearing In The Description Of The Subject Of The Order And Technical Drawings Or Resulting From Them Are Not Additional Works.
