3.1 The Subject Of The Order Is To Act As An Investors Supervision Inspector Over The Implementation Of The Investment Task Titled: Reconstruction And Expansion Of The District Indoor Swimming Pool In Biłgoraj - Stage Ii. 3.2 Construction Works Under The Task Reconstruction And Expansion Of The District Indoor Swimming Pool In Biłgoraj - Stage Ii Are Carried Out On The Basis Of Documents Available On The Website Of The Ongoing Procedure At: Https:// Ezamowienia.Gov.Pl/Mp-Client/Search/List/Ocds-148610-Aa6cf925-0095-11Ef-B81b-Aebd110f5279 And Https://Spbilgoraj.Bip.Lubelskie.Pl/Index.Php?Id=543&P1=Szczegoly&P2= 91777 3.3 The Scope Of Works To Be Performed Under This Order, Which Will Be Supervised By The Supervision Inspector, Includes In Particular The Following: 1) Cutting Off The Installation Of The Existing Indoor Swimming Pool Building, 2) Demolition And Preparatory Works Of The Existing Indoor Swimming Pool / Demolition Of The Roof Of The Swimming Pool Hall, 3) Demolition The Upper Structure Of The Flat Roof Over Part Of The Facilities, Construction And Installation Demolition Of Elements Unnecessary For The Planned, Rebuilt Facility, 4) Basic And Comprehensive Construction And Installation Works Related To The Reconstruction Of The Swimming Pool Hall And The Remaining Part Of The Existing Facility, 5) Construction Of The Newly Designed Part Of The Extension On The North Side, 6) Comprehensive Construction And Final Finishing And Equipment Of The Swimming Pool Hall And The Remaining Part Of The Facility Along With The Sub-Pool,7) Works Not Performed In Stage I, Which Should Be Additionally Performed In Stage Ii:A) Priming And Facing With Ceramic Tiles Of The Walls Of The Water Slide Staircase In The Amount Of - 130.05 M2, B) Execution (In The Room No. 1.57 – 1.59) Two Partition Walls With A Thickness Of 11.5 Cm Of Ceramic Hollow Bricks In The Amount Of 10.66 M2 With Plastering On Both Sides, Priming And Painting With Acrylic Paints Twice, C) Construction Of A Section Of The Access Road From The Eastern Side (Longitudinal Profile A-B Along The Length - Kilometer From 0+023.86 To 0+ 045.31) Along With The Construction Of A Sidewalk Along This Length In Accordance With The Technical Design Of Roads And Accesses, 8) After The Implementation Of The Works Of Stage Ii - Reconstruction Of The Completed Part Of Stage I, Consisting Of: - Dismantling The Fire Protection Wall. Separating Stage I From Stage Ii, - Liquidation Of The Cloakroom And Sanitary Facilities And Construction Of The Final Saunarium In Its Place, - Dismantling Of The Temporary Floor For The Braking Bathtub, - Dismantling The Temporary External Steel Entrance Stairs Together With The Foundations, - Construction Of The Internal Part Of The Water Slides With The Braking Bathtub, Including : Steel Spiral Staircase With Starting Elements Made On A Pole Fixed In The Foundation, Forging 4 Holes In The External Wall To Allow The Slides To Pass Through The Wall, Along With Finishing The Places Where The Slide Passes Through The Building Partitions, Electric Power Supply, Start-Stop System And Integrating It With The System Supplying Water To The Slide, Launching The Water Installation Supplying Water To The Slide, Launching Multimedia, Instructions For Using The Slide In The Form Of Information Boards, Assembly Of The Last Sections Of Slides (Inlet And Outlet) Provided By The Ordering Party, Configuration Of The Entire Slide System With A Dynamometer And Final Launch Of The Slide,9 ) After Completing These Works, Connecting The Sports Swimming Pool Hall Constructed As Part Of Stage Ii With The Recreational Swimming Pool Hall - To Obtain The Target Facility Of A Rebuilt And Expanded Indoor Swimming Pool Along With Obtaining A Non-Final Occupancy Permit For The Entire Facility (Stages I And Ii), 10) Execution Of Works Restoration Works, Water Exchange In The Basins Of Stage I And Stage Ii.3.4 The Contractor Provides Services In The Field Of Acting As An Investors Supervision Inspector In The Construction, Sanitary, Electrical And Road Industries Over The Implementation Of The Subject Of The Order In Accordance With The Act Of July 7, 1994. – Construction Law (Consolidated Text: Journal Of Laws Of 2024, Item 725), And In Particular: 1) Represents The Investor On The Construction Site By Checking The Correctness And Compliance Of The Order Execution With The Contract And Scope Of Works, Design Documentation And Construction Permit, And Regulations And Principles Of Technical Knowledge, 2) Checks The Quality Of Completed Works And Built-In Construction Products And Devices, And In Particular Prevents The Use Of Defective Construction Products And Those Not Approved For Use In Construction, 3) Checks And Accepts Works In Individual Industries That Are Covered Or Disappearing And Are Subject To Partial Acceptance And Final Results Within 3 Working Days From The Date Of Their Notification By The Contractor Of The Works (Confirming This Fact In The Construction Log), Participates In Tests And Technical Acceptance Of Installations, Technical Devices, And Prepares And Participates In Acceptance Activities Of The Works Or Their Parts (Stages), As Also Takes Part In Putting Them Into Use, 4) Confirms The Works Actually Performed And The Removal Of Defects And Defects, 5) Takes Part In The Work On Handing Over The Construction Site And The Work Front, 6) Checks The Calculations, As-Built, Additional And Replacement Cost Estimates And The Measurements Of The Works Submitted By The Contractor, 7) Maintains Constant Contact With The Authors Of The Design Documentation During The Execution Of The Works, 8) Participates In The Submission Of As-Built Documentation By The Contractor (Including Documents Allowing For The Assessment Of The Correct Performance Of The Subject Of The Contract Required By Current Legal Regulations, Construction Logs, Technical Acceptance Reports , Test, Measurement And Examination Protocols, As-Built Design Documentation With A List Of Changes Introduced In Relation To The Original Documentation And Other Documents Required For Final Acceptance) By Verifying And Checking Its Correctness,9) Informs The Ordering Party About Any Circumstances That May Affect The Timeliness And Correctness Of The Work Performed Works,10) Participates In The Activities Of Partial And Final Acceptance Of The Supervised Order And Preparation Of The Acceptance Report Of The Completed Works,11) Controls The Correctness Of The Execution Of The Order During The Period Of: A) Warranty Granted By The Contractor Of Construction Works Of 60 Months, B) Warranty Of 5 Years, In In Particular, The Supervision Inspector Will Participate In Guarantee And Warranty Reviews And Will Supervise The Removal Of Identified Defects And Faults During These Periods, 12) Ensures Compliance With Occupational Health And Safety Conditions During The Execution Of The Order By The Contractor, 13) Prepares A Template Of A Monthly Report On The Progress Of Works And Submits For Use By The Contractor, After Approval By The Ordering Party, 14) Prepares Protocols Of Necessity In The Event Of Additional, Replacement Or Abandoned Works, 15) Verifies The Report Prepared By The Contractor Of The Works From A Photographic And Descriptive Inventory Of Buildings, Networks And Other Devices Located On The Site Construction Site Or In Its Surroundings And Approves It,16) Stays On The Construction Site In Accordance With The Following Rules: A) Presence Of The Head Of A Multi-Discipline Supervision Team With Construction Qualifications To Manage Construction Works In The Construction And Construction Specialization To The Extent Enabling Supervision Of The Works Covered By The Subject Of The Contract - Min. 1 Time A Week, Confirmed By An Entry In The Register Of The Inspectors Presence At The Ordering Partys Headquarters Or In The Construction Log, B) The Presence Of An Inspector With Construction Qualifications To Manage Construction Works In The Road Engineering Specialty To The Extent Enabling Supervision Of The Works Covered By The Subject Of The Contract - Min. 1 Time A Week, If Industrial Works Are Carried Out There, Confirmed By An Entry In The Register Of The Inspectors Presence At The Ordering Partys Headquarters Or In The Construction Log, C) The Presence Of An Inspector With Construction Qualifications To Manage Construction Works In The Installation Specialty In The Field Of At Least Thermal Installations And Devices, Ventilation, Water Supply And Sewage Systems To The Extent Enabling Supervision Of The Works Covered By The Subject Of The Order - Min. 1 Time A Week, If Industrial Works Are Carried Out There, Confirmed By An Entry In The Register Of The Inspectors Presence At The Ordering Partys Headquarters Or In The Construction Log, D) The Presence Of An Inspector With Construction Qualifications To Manage Construction Works In The Installation Specialty In The Scope Of At Least Electrical Installations And Devices And Power Engineering To The Extent Enabling Supervision Of The Works Covered By The Subject Of The Order - Min. Once A Week, If Industrial Works Are Carried Out There, Confirmed By An Entry In The Register Of The Inspectors Presence At The Employers Headquarters Or In The Construction Log, 17) Approves Or Comments Within 7 Business Days On The Construction Managers Applications Regarding The Materials Submitted For Their Installation ,18) Participates In Construction Councils, Including: A) Organizing Meetings And Informing Invited Persons About Their Date And Place, B) Recording And Preparing Minutes Of Construction Councils And Other Meetings Held, C) Submitting Them To The Contractor And The Ordering Party Within 3 Business Days From The Date On Which The Meeting Of The Construction Council Took Place, The Minutes Of The Council To Be Agreed Upon, D) Organizes And Informs Invited Persons About Additional Meetings Regarding The Implementation Of The Investment, E) Immediately Informs The Ordering Party About Problems Or Circumstances That May Affect The Quality Of The Works Or Delay Deadline For Completing The Task,19) Confirms The List Of Construction Materials Subject To Disposal And/Or Dismantling Prepared By The Contractor Of Construction Works,20) Provides The Ordering Party With Information On The Correctness Of The Works And Their Progress At The Request Of The Ordering Party Within 3 Business Days, Due To The Limited Number Of Characters C.
