The Subject Of The Order Is The Design-Build-Install Modernization Of The Hospital Emergency Department And The Driveway For Ambulances In The Provincial Hospital Center Of The Jelenia Góra Valley According To The Functional And Utility Design. The Subject Of The Order Includes The Execution Of Design Works Together With The Implementation Of Construction Works Based On Them For Investment Task Entitled: Modernization Of The Hospital Emergency Department At Wcskj In Jelenia Góra And Retrofitting With Specialized Medical Equipment. The Scope Of Documentation Includes: 1) Architectural And Construction Design, 2) Technical And Executive Design, 3) Bill Of Quantities, 4) Bid Cost Estimate Simplified With A List Of Materials, Labor And Equipment And An Indication Of The Structure Of Overheads.5) Technical Specification For The Execution And Acceptance Of Works. As Part Of The Implementation Of Construction Works, The Following Should Be Carried Out: 1) Reconstruction And Renovation Of Rooms In Part Of The Existing Hospital Emergency Department (Hereinafter Referred To As Emergency Department);2 ) Reconstruction Of The Existing Passenger Lift Connecting The Emergency Department; 3) Reconstruction And Renovation Of The Communication Overpass (Ambulance Ramp) Along With External Pedestrian Communication Providing Access To The Emergency Department. The Subject Of The Order Was Specified In Detail In Annex No. 1 To The Swz - Functional And Utility Program (Hereinafter: Pfu ). The Subject Of The Order Includes The Modernization, Adaptation And Equipment Of An Existing Building In Order To Improve The Availability And Quality Of Health Services Provided In The Area Of ​​The Hospital Emergency Department By Increasing The Efficiency Of The Emergency Medical System, Including The Effectiveness Of Providing Rescue Services. The Planned Works Include Reconstruction Of Areas Strictly Defined In The Fup.2. The Contractors Obligations Under This Order Also Include The Delivery, Installation And Commissioning Of Equipment Permanently Associated With The Building, Resulting From The Pfu, Such As: Vertical Bed Panel, Upper And Lower Medical Buildings, Ceiling Screen, Plaster Dressing Table, Worktop Stainless Steel, Operating Lamp, Etc. Note: Prices For The Above. The Equipment Should Be Included In The Price Of Construction Works. In Addition, The Contractors Obligations Under This Order Also Include Providing Elements Of Movable Equipment: A Trolley For Transporting Patients, A Transport Trolley, A Treatment Trolley, An Emergency Trolley, A Desk, A Mobile Container, A Medical Couch, A Multifunctional Bed With Electric Drive, Bedside Cabinet With Side Tabletop.3. The Subject Of The Order Includes The Performance Of All Works, Activities And Supplies Related To The Execution Of Construction Works, As Well As The Delivery And Installation Of Elements Of Equipment Permanently Connected With The Building And Elements Of Movable Equipment - Necessary For The Proper Implementation Of The Subject Of The Order.4. Before Commencing Design Work And At Each Stage Of Their Implementation, The Contractor Is Obliged To Agree With The Ordering Party On All Issues Regarding The Adopted Solutions And Technologies, Material Solutions And Equipment And Any Other In Order To Ensure The Execution Of The Subject Of The Order In Accordance With The Fup, Taking Into Account The Information Contained In Point 4 Of The Tor. , Design Documentation, Principles Of Construction Art And The Needs And Guidelines Of The Ordering Party. Moreover, The Contractor Is Obliged To Keep The Ordering Party Informed About The Progress Of Works That Are The Subject Of The Order And About Any Problems That Affect The Performance Of The Contract, In Particular The Implementation Date.5. The Contractors Obligations Also Include Making All Arrangements With The Relevant Authorities And Services At Every Stage Of The Implementation Of The Subject Of The Contract, Obtaining All Administrative Decisions Required By Law, Including In Particular Reporting Works That Do Not Require A Building Permit Or Obtaining A Building Permit To The Extent Resulting From The Prepared Documentation, As Well As And Making All Necessary Reports To Public Administration Bodies, Performing All Types Of Expertise, If Necessary.
