Mobility Is Carried Out In Connection With The Implementation Of The Accredited Project No. 2023-1-Pl01-Ka121-Vet-000131518 Under The Erasmus+ Program. 2. The Subject Of The Order Includes Comprehensive Service Provision, Including Providing Airline Tickets For All Participants From The Airport In Warsaw (And From Lisbon To Warsaw, I.E. Both Ways), Providing Transport From The Airport In Lisbon To Places Of Accommodation, Accommodation, Meals, Organization Of Internships And Implementation Of A Cultural Program, Providing Public Transport Tickets. The Service Will Cover 32 Participants (30 Participants And 2 Caregivers). 3. The Service Will Be Provided On The Following Dates: - Departure Flight Tickets On June 30, 2024 - Return On July 13, 2024 For 32 Participants - Accommodation And Meals From June 30, 2024 - July 13, 2024 - 13 Nights, Meals On 30/06/2024 And On 13/07/2024 Depending On The Transfer To And From The Airport. - Organization Of Internships From July 1, 2024 To July 12, 2024 - 12 Days Of Internship. 4. As Part Of The Implementation Of The Subject Of The Order, The Contractor Is Obliged, Among Others, To: To: A) Find A Portuguese Agency/Partner Whose Tasks Will Include: - Organizing Internships In Portugal (Lisbon And The Surrounding Area) For Students Studying In The Following Fields: Veterinary Technician - 15 Participants, Nutrition And Catering Services Technician - 15 Participants, - Organizing Cultural Program On Days Free From Teaching, Min. Two Trips For 30 Participants And 2 Guardians, Assumptions Regarding The Cultural Program, E.G. A Walk Around Lisbon With A Guide, A Visit To The Oceanarium, The Organization Of A Shorter Trip Is Also Allowed In The Afternoon On The Day When The Internship Takes Place, E.G. On Friday. - Monitoring During Mobility, - The Contractor Should Provide A Polish-Speaking Coordinator Who Will Be Available At The Place Of Accommodation, - Provide A Supervisor During The Internship, - The Internship Should Take Place In Lisbon Or The Surrounding Area, In Workplaces - In The Case Of A Nutrition And Catering Services Technician - Companies Conducting Catering Activities (Restaurants, Hotels, Snack Bars, Cafes, Bars And Office Restaurants), Hotels, Guesthouses, Holiday Homes, Ferries, Passenger Ships And Other Entities In The Field Of Education Appropriate To The Profession, In The Case Of Veterinary Technicians - Companies Dealing With Animal Clinics And Clinics, Zoos, Shelters, Industrial Farms, Animal Breeding And Insemination Plants, Food Processing Plants, Veterinary Labor Inspection And Other Entities In The Field Of Education Relevant To The Profession. - Internships Should Take Place Between 08:00 And 16:00. - Foreign Internships Must Be Carried Out In Accordance With The Standards/Requirements Of The Polish Quality Framework For Internships And Traineeships Available On The Website Https://Www.Parp.Gov.Pl/Storage/Publications/Pdf/1.%20Polskie_Ramy_Jakosci_Praktyk_I_Stazy_Informator.Pdf B) Ensuring Transport Of All Participants From The Lisbon Airport To The Places Of Accommodation And From The Places Of Accommodation To The Airport (Both Sides) C) Providing Participants With Public Transport Cards Entitling Them To Travel By Public Transport Throughout The Duration Of The Mobility. D) Provide Accommodation For All Participants. E) Providing Food For All Participants. 5. The Ordering Party Requires That The Following Requirements Be Met When Providing The Accommodation Service: A) Accommodation In Apartments Or A Hotel With Meals In Places Of Residence Or At Designated Points, Which Means That The Ordering Party Allows The Possibility Of Meals In A Place In The Immediate Vicinity Of Place Of Residence. B) Accommodation In Lisbon And The Surrounding Area In Portugal, Where Internships As Part Of Educational Mobility Will Take Place. C) Accommodation In 1-, 2-, 3-, 4- Or Multi-Person Rooms Depending On The Facilitys Availability, Capable Of Accommodating A Total Of 32 Participants (30 Participants + 2 Guardians). Rooms Should Be Equipped With Single Beds, Each With Direct Access To The Bathroom (Bath/Shower, Toilet, Washbasin, Hairdryer). Guardians Will Be Responsible For Placing Participants In Their Rooms On Site. D) Apartments/Hotel Should Be Equipped With Wireless Internet Available Throughout The Facility, Including In Participants Rooms, Tv, Public Iron, Washing Machines. E) The Facilities Where Participants Will Be Accommodated Should Ensure Peace And Quiet From 22:00 To 07:00. F) The Ordering Party Requires That Participants Have Access To The Kitchen. 6. The Ordering Party Requires That The Following Requirements Be Met With Regard To Meals: A) The Contractor Will Provide Meals For All Participants. B) The Contractor Will Provide Meals For All Participants At Three Times Of The Day: Breakfast, Lunch And Dinner. C) Breakfast Will Be Provided In The Form Of A Buffet, The Menu Of Which Will Include At Least The Following Food Products: - Fresh Bread, Butter, Cheese, Ham, Vegetables, Fruit, Multi-Fruit Jams, Honey, Milk, Breakfast Cereals, Yogurts, Coffee, Tea. D) Lunch Provided In The Form Of A Two-Course Meal, I.E. First Course Soup And Second Course E.G. Meat/Potatoes/Salad/Fish, In Accordance With The Menu Prepared By The Organizer. E) Dinner Will Be Provided In The Form Of A Buffet, Which Will Include At Least: - French Fries, Vegetables, Fried Eggs, Fish, Various Types Of Meat, Potatoes, Cold And Hot Drinks. F) The Ordering Party Reserves That In The Event Of Special Nutritional Needs Of Participants, It Has The Right To Modify The Food Order In Accordance With The Participants Diet Requirements. 7. The Ordering Party Informs That The Contractor Is Obliged To Keep Service Documentation In Paper And Electronic Form In Accordance With The Templates Provided By The Ordering Party On The Day Of Signing The Contract For The Provision Of The Service. 8. Other Requirements For Contractors: A) The Rooms In Which Classes Will Be Conducted Should Be Marked In Accordance With The Guidelines Of The Erasmus+ Program, B) Internships Will Take Place From Monday To Friday, The Cultural Program Will Be Carried Out On Weekends At Hours Tailored To The Needs Of Project Participants - In Accordance With A Previously Agreed Schedule, The Organization Of A Shorter Trip Is Allowed On The Day When The Internship Is Held, E.G. On Friday. C) All Classes Will Be Conducted Stationary. D) As Part Of The Service, The Contractor Is Obliged To Create Documentation On An Ongoing Basis And Then Provide The Complete Documentation To The Ordering Party. Documentation Confirming The Implementation Of The Service Should Include: - Internship Log With An Attendance List, Number Of Hours, Topics And Program Of Classes, Including Markings, Photos, Certificates/Attestations In Accordance With The Erasmus+ Program Guidelines, - Attendance Lists With Signatures Of Course Participants, - Summary Summary Containing: Date, Time And Topic Of The Classes. 9. The Ordering Party Reserves The Right To Inspect The Implementation Of Classes At Any Time, As Well As The Right To Inspect The Documentation Related To The Implementation Of Classes.
