1. The Subject Of The Order Is The Provision Of Personal Assistance Services To People With Disabilities. The Estimated Total Maximum Number Of Personal Assistance Services For People With Disabilities Is 9,247 Clock Hours. Services Will Be Provided From Monday To Sunday, Depending On Reported Needs. The Ordering Party Reserves The Right To Change The Above Demand For Personal Assistance Services. The Estimated Division Of The Number Of Hours Of Personal Assistance Services Per One Person With A Disability During The Duration Of The Contract Is: 1) 540 Hours For Each Of 9 Disabled People With A Certificate Of Severe Disability With Multiple Disabilities ,2) 420 Hours For Each Of 10 Disabled Persons With A Certificate Of A Significant Degree Of Disability,3) 187 Hours For 1 Disabled Person With A Certificate Of A Moderate Degree Of Disability.2. The Assistance Must Be Provided In Accordance With The Personal Assistant For A Person With Disability Program For Local Government Units - 2024 Edition - Of The Ministry Of Family And Social Policy, Financed By The Solidarity Fund (Sf).
