1. A Detailed Description Of The Subject Of The Contract Was Specified In Particular In The Specification Of Terms Of Contract (Swz) And Design Documentation - And This Documentation Was Prepared For A Larger Scope Than That Covered By This Procedure. This Order Concerns The Scope Marked In The Graphic Attachment And Indicated In The Swz, I.E. Kościuszki Street And Does Not Cover The Remaining Scope Included In The Documentation), No Objection From The Staroste Of Będzin To The Notification Of July 12, 2018, The Decision Of The Staroste Of Będzin To Authorize The Implementation Of A Road Investment No. 5.2018 Of August 29, 2018, Conservation Permit No. K/431/2022 Of April 25, 2022, Target Traffic Organization Design With Approvals And Opinions, Technical Specifications For The Execution And Acceptance Of Works, Graphic Annex, Permits, Conditions, Approvals, Resolutions, Opinions, Decisions, Arrangements And Letters Issued To The Above-Mentioned Tasks And Constituting An Annex To The Swz Or Documentation And Includes In Particular: 1) Reconstruction Of The Municipal Road On Tadeusza Kościuszki Street From The Intersection With Ludowa Street To The Bridge By The Castle, Together With The Intersection With Kopernika And Czarnieckiego Streets And Parking Spaces At Kościuszki Street (From The South) Including Construction Of The Road With Sidewalks And Individual Exits And The Necessary Accompanying Infrastructure. Lowered Curbs Should Be Installed At Pedestrian Crossings Throughout The Entire Development Area. The Scope Of The Order Was Specified In The Graphic Attachment (Appendix No. 1).2) Construction Of Road Drainage - Including In Particular The Construction Of A Storm Sewage System With Drains And The Construction Of An Additional Well In Czarnieckiego Street In Place Of The Existing Well K30 Together With Drains And A Connection To The Planned Well D80 (K31) (The Additional Scope Should Be Estimated, It Was Not Included In The Bill Of Quantities), 3) Reconstruction Of The Sanitary Sewage System - Including In Particular The Construction Of New Connections, 4) Construction Of A Water Pipeline With Connections, 5) Securing The Gas Network, 6) Construction Of Street Lighting. Lighting Lamps Must Have A Flag Holder And The Possibility Of Connecting Additional Lighting Elements.7) Reconstruction Of The Power Grid Into An Underground Power Grid - Including In Particular The Construction Of An Earth Power Grid Along With The Construction Of Wlz Receiving Lines To All Facilities, Including Residential Buildings,8) Construction Of A Service Channel Together With Branches To The Boundaries Of All Properties,9) Carrying Out Works Around The Józef Piłsudski Monument At Ul. Kościuszki, I.E. A Hardened Surface Identical To The Sidewalk Along With The Replacement Of Two Lighting Lamps (Additional Bill Of Quantities),10) In Places Where The Existing Network Collides With The Designed Infrastructure, Securing These Networks,11) Adjustment And Cleaning Of All Manholes And Manholes (Existing And Constructed) ,12) Inventory Of The Existing Construction Site Along With Networks And Connections In Consultation With The Managers Of Individual Elements And, If Necessary, Execution Of Control Ditches, 13) Introduction Of The Temporary Organization Of Road Traffic Along With The Development And Approval Of The Project Of Temporary Traffic Organization, 14) Introduction Of The Target Organization Of Road Traffic , In Accordance With The Traffic Organization Design And Opinions And Approvals,15) Carrying Out All Earthworks, Preparatory And Demolition Works Resulting From The Design Documentation And The Technical Specification For The Execution And Acceptance Of Works, Including The Removal And Disposal Of Unnecessary Materials And The Transport Of Suitable Materials To The Designated Place For Reuse. Dismantling And Disposal Should Be Carried Out In Accordance With The Relevant Waste Regulations.16) Demolition Of Fences Colliding With The Investment, Including Disposal And Relocation Of The Colliding Fence At The General And Technical School Complex.17) Felling Of Some Trees And Shrubs. The Ordering Party Allows No Cutting Down Of Trees/Bushes Provided There Is No Conflict With The Investment And The Contractor Presents The Method Of Finishing The Surface Around The Trees/Bushes.18) Connection, Finishing And Connections To The Existing Surfaces At The Connections With The Surfaces At The Final Sections Of The Works Performed. 19) Carrying Out Finishing And Cleaning Works, In Particular Topsoiling And Grassing Green Areas. 20) Carrying Out The Works In Accordance With The Arrangements. If New Arrangements Are Obtained During The Implementation Of The Investment, In Particular From Network Operators, The Contractor Will Adjust The Scope Of Implementation To The Current Arrangements. 2. The Contractor Will Also Be Obliged To: A) Provide Archaeological Research And Supervision During Earthworks In Accordance With The Requirement Contained In The Permit No. K/431/2022 Of The Silesian Provincial Conservator Of Monuments In Katowice Of April 25, 2022 And In The Letter No. K-Ar /5183.204.2022.Kwk Of November 18, 2022, Along With The Implementation Of The Research Program, Obtaining A Permit To Conduct Research And Other Necessary Consents And Permits, B) Ensuring Constant Access For Pedestrian And Vehicular Traffic To The Property (Including In Particular In Places Where Business Activity Is Conducted) And To The Complex Of General And Technical Schools, The Rectory At The Church Of St. Macieja In Siewierz During The Investment Implementation Period, Until The Completion And Final Acceptance Of The Works, C) Ensuring Road Traffic Access To The Property For Other Vehicles, E.G. Those Collecting Waste/Garbage, During The Investment Implementation Period, Until The Completion And Final Acceptance Of The Works. If This Is Not Possible, The Contractor Is Obliged To Transport, E.G. The Collected Waste To A Place Accessible To Collection Vehicles, D) Equipment And Marking Of The Construction Site And Its Appropriate Security And Ensuring Appropriate Health And Safety Protection, E) Carrying Out Security And Reconstruction Works, In Accordance With The Design, F) Carrying Out All Accompanying Works In Accordance With The Design And The Correct Technology Of Performing The Works, G) Carrying Out Any Water And Power Connections For The Construction Purposes And Incurring The Costs Of Media Consumption, H) Incurring Any Costs Of Turning On And Off The Media, I) Carrying Out The Organization And Technology Design Works And Applying It In Practice, Taking Into Account Occupational Health And Safety Conditions,J) Preparing, Agreeing And Implementing A Project For Temporary Traffic Organization And Securing The Works During The Construction Period,K) Submitting Applications For The Occupation Of The Road And Incurring Possible Costs Of Occupation,L) Performing The Subject Of The Contract In Accordance With The Arrangements With Road Managers, Network And Land Infrastructure Managers, M) Ensuring Supervision Of Appropriate Road Or Land Infrastructure Managers, If The Works Require Supervision Or If It Results From Other Arrangements, Also If It Is Paid - The Contractor Will Bear These Costs ,N) Carrying Out The Necessary Examinations, Tests And Measurements As Well As All Examinations, Tests And Measurements Upon Each Order Of The Supervision Inspectors In Order To Determine The Correctness Of The Works Performed,O) In The Event Of Destruction Or Damage To Devices Or Existing Land Infrastructure Networks During The Execution Of The Task - Repair Them And Restore It To Its Original State, P) Report To The Employer Any Works That Are Covered Or Disappearing. In The Event Of Failure To Report Works That Are Covered Or Disappearing For Acceptance, The Contractor, At The Request Of The Employer, Is Obliged To Carry Out Examinations, Tests And Commissioning, As Well As To Make Uncovered Works.Q) Ensure Authorized Supervision And Supervision, As Well As Appropriate Occupational Health And Safety Conditions,R) Removal Unnecessary Materials, Waste And Garbage From The Construction Site On An Ongoing Basis, S) Tidying Up The Construction Site After The Completion Of The Works And Handing It Over To The Employer No Later Than The Date Of Final Acceptance, T) Restoring The Area After The Works At Least To Its Original State (Unless Special Arrangements Stipulate Otherwise Scope Of Reconstruction),U) Taking All Precautions Necessary To Preserve Materials And Parts Obtained From Demolition Indicated By The Ordering Party. The Materials Indicated By The Ordering Party That Are Suitable For Reuse Or Installation Are The Property Of The Ordering Party. The Remaining Materials Will Be Disposed Of By The Contractor. Regardless Of The Purpose For Which The Ordering Party Intends To Use The Said Materials And Parts To Which He Reserves The Ownership Rights, All Costs Incurred For Transport And Storage In The Place Indicated By The Ordering Party Will Be Covered By The Contractor For Transport Over A Distance Of Up To 10 Km.V) Providing Service Surveying For The Purpose Of Marking Out, Ongoing Control And Inventory, And Marking The Completed Elements On Geodetic Resources (Post-Construction Inventory). The Measurement Must Be Unambiguous, Including And Describing All Completed Objects With Plot Numbers Entered. The Results Of The As-Built Measurement Should Be Supplemented With The Map Resource Located At The District Geodetic And Cartographic Documentation Center In Będzin. By The Date Of Final Acceptance, The Contractor Is Obliged To Provide The Ordering Party With An Inventory Map Signed By A Surveyor With Completely Marked Objects, Together With The Surveyors Declaration On Submitting The Map To The District Center (A Cover Letter With A Receipt Stamp Or A Surveyors Declaration). After Receiving The Inventory Approved By The Center, The Contractor Will Forward It To The Ordering Party Within The Time Specified In The Memo.W) Making Arrangements, Obtaining All Opinions Necessary To Perform The Subject Of The Contract And Putting It Into Use, X) Performing Other Works Necessary For The Proper Implementation Of The Subject Of The Order. 4. The Remaining Terms Of The Order Are Specified In Chapter Iii, Point 4 Of Swz.5. The Criteria Used To Assess Equivalence Are Specified In Chapter Iii, Point 6 Of Swz.6. The Obligation Of The Contractor Or Subcontractor To Employ Persons On The Basis Of An Employment Relationship Is Specified In Chapter Iii, Point 9 Of The Swz
