1. The Subject Of The Order Is The Collection, Loading, Transport, Depositing And Disposal Of Waste Containing Asbestos Lying On Properties In A Landfill.2. The Scope Of The Order Includes: 1) Preparation For Transport And Loading; 2) Organizing The Place Where The Service Is Performed From Waste In A Way That Prevents Its Emission Into The Environment; 3) Transport Of Received Products Containing Asbestos; 4) Deposition (Transfer Of Waste To A Landfill With An Appropriate Permit For Waste Disposal) Hazardous) And Disposal Of Waste Containing Asbestos In An Amount Of Up To 386.115 Mg.5) The Amount Indicated In Point 5 Is The Amount Declared By The Residents And The Ordering Party Reserves The Right To Reduce The Material Scope, But Not To Less Than 50% Of The Indicated Amounts.6) In The Case Of A Larger Scope Than The Declared One The Final Value Of The Service Provided Will Result From The Cost Estimate And Will Depend On The Number Of Households Interested In Providing The Service And The Financial Capabilities Of The Ordering Party.7) The Collection Of Waste From The Property Will Be Confirmed By The Property Owner With A Legible Signature On The Waste Collection Confirmation, Which Contains At Least The Following Information: Name And Name Of The Property Owner From Whom Waste Containing Asbestos Is Collected, Address Of The Property From Which Waste Containing Asbestos Is Collected, Date Of Waste Collection, Amount Of Waste Collected, Legible Signature Of The Property Owner Confirming Proper Performance Of The Service.8) The Contractors Remuneration Will Be Determined On The Basis Of The Unit Price And Actually Collected Asbestos Waste. The Basis For Issuing An Invoice Will Be:I. Confirmation Of Waste Collection, Legibly Signed By The Property Owner, Ii. Waste Transfer Card To Landfills
