The Investment Involves The Expansion, Superstructure And Reconstruction Of A Commercial Building With A Change Of Use Into A Service Building (Two Toddler Clubs) With Technical Infrastructure And A Retaining Wall. The Investment Task Is Co-Financed Under The Program For The Development Of Care Institutions For Children Up To 3 Years Of Age Maluch+ 2022-2029 And The National Reconstruction Program. The Construction Works Include: Reconstruction Of An Existing Commercial Building With A Small Expansion Of The Buildings Horizontal Projection And Thermal Insulation Of Some Parts. Roof And Construction Of New Attics. The Scope Of Work Also Includes Insulating The Entire External Walls Of The Building. Inside The Building, It Is Planned To Remove: Fragments Of Structural Walls And Partitions, Windows And Doors, As Well As All Infrastructure Installations And Devices. In Return, The Interior Will Be Rebuilt To Suit The Required Function. The Reconstruction Will Consist In Constructing New Internal And Partition Walls, Adapting Existing Rooms To The Technical And Legal Requirements Of The Toddler Club Premises And All Infrastructural Equipment Inside And Outside. In The Southern Part, A New Retaining Wall Will Be Built In Place Of The Old One, And The Surrounding Fence Will Be Supplemented With New Ones In Some Places. A Detailed Description Of The Subject Of The Order Is Included In The Design Documentation And Stwiorb. Please Pay Attention To The Attachment Boards With Changes. This Is An Attachment Corrected By The Interior Designer And Prepared For The Arrangement Of The Building. Please Take These Comments Into Account In Your Offer Price. No Later Than On The Day Of Signing The Contract, The Contractor Whose Offer Has Been Selected Will Submit A Material And Financial Schedule To The Ordering Party. Employment Requirement Based On An Employment Relationship Requirements Referred To In Art. 95 Pzp.1. Because The Implementation Of This Order Must Be Carried Out By Performing Activities Under The Conditions Specified In Art. 22 § 1 Of The Act Of June 26, 1974, Labor Code - For The Benefit Of The Contractor Or Subcontractor, At The Place And Time Indicated By The Contractor Or Subcontractor - The Ordering Party, In Accordance With Art. 95 Section 1 Of The Public Procurement Law Requires The Contractor Or Subcontractor To Employ Persons Performing Activities Falling Within The So-Called Direct Costs Based On The Employment Relationship. This Requirement Applies To Persons Who Perform Activities Directly Related To The Performance Of Construction Works Covered By The Subject Of The Contract, I.E. The So-Called Manual Workers, In Particular: 1) General Construction (Including Earthworks, Paving Works And Other Works) Persons Performing The Activities Indicated By The Ordering Party Should Be Employed By The Contractor Or Subcontractor On The Basis Of An Employment Relationship, At Least For The Entire Period In Which They Will Perform The Above-Mentioned Activities.2. The Requirement Does Not Apply To Persons Managing The Construction Site, Persons Performing Independent Technical Functions In Construction, Providing Construction Materials Supply Services, Transport Services, Equipment Services And Similar.3. Method Of Documenting The Employment Of Persons Referred To In Art. 95 Of The Public Procurement Law. The Contractor Is Obliged To Submit To The Ordering Party, At His Request Within The Deadline Specified In The Application, Documents Confirming The Employment Of Persons Performing The Activities Referred To Above, In Particular A Written Statement From The Contractor Or Subcontractor Confirming That The Contractors Or Subcontractors Employees Are Employed Under An Employment Contract In Within The Meaning Of The Provisions Of The Act Of June 26, 1974 - Labor Code, Taking Into Account The Minimum Remuneration For Work Determined Pursuant To Art. 2 Section 3-5 Of The Act Of October 10, 2002 On Minimum Wages (Consolidated Text: Journal Of Laws Of 2020, Item 2207). This Declaration Should Include In Particular: Precise Identification Of The Entity Submitting The Declaration, Date Of Submission Of The Declaration, Indication That The Activities Listed In Point 1 Is Performed By Persons Employed Under An Employment Contract, Indicating The Number Of These Persons, The Type Of Employment Contract And The Working Hours, As Well As The Signature Of The Person Authorized To Submit A Declaration On Behalf Of The Contractor Or Subcontractor.4. The Contracting Authoritys Rights To Control The Contractors Compliance With The Requirements Referred To In Art. 95 Of The Public Procurement Law.
