The Subject Of The Order Is The Construction Of A Low-Parameter Heating Network For Central Heating And Domestic Hot Water On The Premises Of The Specialist Hospital In Zabrze Sp. Z O. O. The Order Includes: 1) Submitting To The Municipal Office In Zabrze (Construction Department) An Application For A Decision On Determining The Location Of A Public Purpose Investment For The Planned Investment Called Construction Of A Low-Parameter Heating Network For Central Heating And Hot Water On The Premises Of The Specialist Hospital In Zabrze Sp. Z O. O. (If Necessary).2) Pre-Design And Design Works Along With Formal And Legal Services On Behalf Of The Investor And Obtaining A Decision On A Building Permit Or Notification And Use Of The Facility In The Above-Mentioned Investment (If Necessary).3 ) The Necessary Design Documentation Should Include The Following Industries: A) Installation, 4) Construction Of A Turnkey Linear Facility Of The Heating Network Along With Formal And Legal Support On Behalf Of The Investor. 5) Undertaking All Necessary Activities On Behalf Of The Investor Not Included In Points 1 - 4 , And Possibly Necessary To Carry Out The Full Implementation Of The Turnkey Investment Called Construction Of A Low-Parameter Heating Network For Central Heating And Hot Water On The Premises Of The Specialist Hospital In Zabrze Sp. Z O. O.6) The Ordering Party Informs That It Excludes The Following Scope Of Works From The Functional And Utility Program (Page No. 7): The Reconstruction Works Should Include The Construction Of A New Road Surface And A Sidewalk Made Of Concrete Cubes (The Scope Of Construction Of The New Road And Sidewalk Is Shown On The Overview Map Constituting Annex No. 3). The Road Should Be Designed And Constructed For Traffic Category Kr1. This Means That As Part Of The Reconstruction Works, The Contractor Will Restore To Its Original Condition Only The Existing New Paved Surfaces That Will Be Necessary For Dismantling During The Installation Works Of The Heating And Hot Water Networks. E.G.: • Fragment Of The Road And Sidewalk In The Area Of ​​The Gable Wall Of Building D• Fragment Of The Entrance To The Parking Lot At Building D• Fragment Of The Road In The Area Between Buildings B And C• Fragment Of The Road In The Area Of ​​Building A Employer Does Not Provide For The Reconstruction Of The Surface Indicated In Annex 3 To The Fup In The Form Of New Sidewalks Between Buildings C And D And In The Area Of ​​Building B, As Well As In The Form Of A New Road Between Buildings D And F, But Requires Standard Soil Compaction In The Excavations To The Level Of The Existing Terrain. Proper Names Of The Producers Included In The Documentation - Annex No. 3 To The Swz Are Not Binding On The Contractor. They Should Be Treated Only As Examples To Illustrate The Described Parameters And Technical Requirements. The Ordering Party Allows The Use Of Materials, Devices And Systems With Parameters No Worse Than Those Indicated By The Ordering Party. Equivalent Solutions Must Ensure Interoperability Of Systems And Installations In Accordance With Those Provided For In The Design And Operating At The Ordering Party. All Parameters And Technical Requirements Of Sample Materials, Devices And Systems Provided For In The Design Documentation Are Minimum Parameters, Unless The Provision States Otherwise Or The Data Concerns The Dimensions And Weight Of The Device. Wherever In The Description Of The Subject Of The Order, Technical Documentation, Technical Specification Of Execution And Acceptance Of Works Construction And In Other Documents, The Ordering Party Referred To The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems Referred To In Art. 101 Section 1 Point 2 And Section 3 Of The Public Procurement Law, And Such References Were Not Accompanied By The Expression Or Equivalent, The Ordering Party Allows Solutions Equivalent To Those Described In Each Such Standard, European Technical Assessment, Approval, Technical Specification, Technical Reference System. In Connection With The Above, It Should Be Assumed That Each Standard, European Technical Assessment, Approval, Technical Specification, System Of Technical References Appearing In The Description Of The Subject Of The Contract Is Accompanied By The Words Or Equivalent. The Burden Of Proof That The Offered Materials, Products Or Services Comply With The Standards, Technical Assessments, Technical Specifications And Technical Reference Systems To Which The Subject Of The Order Refers Rests With The Contractor (In Accordance With Article 101(5) Of The Public Procurement Law). The Contractor Must Prove In The Offer, In Particular By Means Of Evidence Referred To In Art. 104-107 Of The Public Procurement Law, That The Proposed Solutions Meet The Requirements Specified In The Description Of The Subject Of The Order To An Equivalent Extent.
