1. The Subject Of The Order Is A Service Consisting In The Performance Of Periodic Annual Inspections (Basic Inspection) Of 19 Bridge Structures And 47 Engineering Structures - Culverts Located On Provincial Roads In The Rdw Area In Hrubieszów.2. The Scope Of The Order Includes: 1) Preparation Of A Schedule For Checking The Technical Condition Of Facilities As Part Of Basic Inspections And Submitting It To The Ordering Party For Approval Within 7 Days After Signing The Contract2) Access To Each Facility Covered By The Order3) Performance Of Field Inspections.4) Preparation Of Periodic Protocols Annual Inspection - Basic Inspection For Each Facility Separately. 5) Preparation Of Lists Of Needs For The Current Maintenance And Renovation Plan For Each Facility Separately, Along With Their Valuation And Unit Prices. 6) Inspections Should Meet The Requirements Of Periodic Inspections Specified In Art. 62 Section 1, Point 1 And Section 1A Of The Act Of 1994 Construction Law (Consolidated Text: Journal Of Laws Of 2024, Item 278) In Order To Check The Technical Condition Of Building Elements Exposed To The Harmful Effects Of Weather Conditions And The Destructive Effects Of Factors Occurring During The Use Of The Facility.7) Inspections Should Be Prepared In Paper Version In The Amount Of 2 Copies For Each Object Separately And In Electronic Version (In Pdf Format) On A Cd On Forms In Accordance With The Templates Specified In The Instructions For Carrying Out Road Inspections Of Engineering Structures And Templates Of Forms 1 A, 1 B - Attachments To Swz, 8) Inspection Reports Should Be Prepared In Accordance With: • Instructions For Conducting Road Inspections Of Engineering Structures Constituting An Annex To Order No. 35 Of The General Director For National Roads And Motorways Of September 28, 2020, • Rules For Applying The Scale Of Point Assessments Of The Technical And Suitability For Use, Aesthetics Of Road Engineering Structures And Their Surroundings, Part I, Bridge Structures, Constituting An Annex To Order No. 1 Of The General Director For National Roads And Motorways Of January 30, 2019.9) Content Of The Protocol:• Title Page Containing The Name Of The Road Management Board , Name Of The Facility, Jni, Road Number And Mileage, General Photograph Of The Facility, • Protocol With A Completed Part Regarding The Proposed Decisions/Applications, • List Of Needs For The Current Maintenance And Renovation Plan.10) The Contractor Should Have Qualified Personnel With Unlimited Construction Qualifications In The Specialty Mostowa Issued On The Basis Of The Regulation Of The Minister Of Investment And Development Dated 29/04/2019 Regarding Professional Preparation To Perform Independent Technical Functions In Construction Or Corresponding Authorizations Issued On The Basis Of Previously Applicable Regulations And Successfully Completed Training In The Field Of Basic Inspections Of Bridge Structures, Therefore There Is No Need To Perform The Order By Persons Employed Under The Labor Law.11) However, In The Case Of Persons Who Obtained Authorizations In Other European Union Countries As Well As Persons From The European Economic Area (I.E. The Kingdom Of Norway, The Principality Of Liechtenstein, The Republic Of Iceland) And Citizens Of The Swiss Confederation, The Authorizations Held Must Meet The Conditions Specified In The Act Of December 22, 2015 On The Principles Of Recognition Of Professional Qualifications Acquired In The Member States Of The European Union (Consolidated Text: Journal Of Laws Of 2023, Item 334, As Amended).12) Persons From Outside The Territory Of The Republic Of Poland Must Present A Decision On The Recognition Professional Qualifications Or A Decision On The Right To Provide Cross-Border Services, Issued By The Competent Councils Of The Chamber Of Engineers, Or Document That They Have Passed The Examination For Construction Licenses Before The Competent Chamber.13) The Subject Of The Contract Has Been Described In The Manner Specified In Art. 101 Section 1 Point 2 And Section 3 Of The Public Procurement Law. The Ordering Party Allows Solutions Equivalent To Those Described, Provided That The Contractor Proves In The Offer, In Particular By Means Of Evidence Referred To In Art. 104-107, That The Proposed Solutions Meet The Requirements Specified In The Description Of The Subject Of The Order To An Equivalent Extent.3. The Ordering Party Does Not Require The Submission Of The Evidence In Question As Part Of These Proceedings, Except For Those Specified In Point. 134. As Part Of The Contractual Remuneration, The Contractor Is Obliged To: Ensure The Share Of Electric Vehicles Or Natural Gas-Powered Vehicles In The Fleet Of Vehicles Used To Perform The Public Task In Question, In The Share Specified In Art. 68 Section 3 Of The Act Of January 11, 2018 On Electromobility And Alternative Fuels (Journal Of Laws Of 2023, Item 875, As Amended), Having Regard To Art. 36A, Specifying The Rules For Rounding Off Percentages When Calculating The Number Of Required Vehicles, If The Performance Of A Public Task Requires The Use Of Motor Vehicles. The Specific Number Of Vehicles Will Be Indicated By The Contractor At The Stage Of Signing The Contract.5. The Ordering Party Reserves The Right To Participate In Field Inspections Of Its Choice.6. The Contractor Is Obliged To Ensure Safe Road Traffic Conditions (Vehicular And Pedestrian) In The Area Of ​​Services Performed And To Mark The Place Of Performance On The Basis Of The Temporary Traffic Organization Project Submitted By The Ordering Party And Approved By The Traffic Manager On Provincial Roads.7. The Contractor Is Obliged To Conclude A Civil Liability Insurance Contract At His Own Expense For The Duration Of The Order8. The Ordering Party Does Not Allow The Possibility Of Submitting Partial Offers.9. The Subject Of The Order Is The Provision Of Services Of Relatively Low Value And Its Scope Allows Contractors From The Group Of Small Enterprises To Submit An Offer. Dividing The Contract Into Parts Could Result In Less Interest In The Procedure On The Part Of The Contractors Due To The Too Low And Unattractive Values ​​Of Individual Parts Of The Contract, And May Also Unnecessarily Increase The Costs Of Providing The Service Due To The Need To Involve An Increased Number Of Means Of Transport Or The Number Of People Carrying Out The Order (For Each Task Separately In The Case Of Division Into Parts). Moreover, Due To The Location Of The Facilities (Same Roads), One Contractor, In The Ordering Partys Opinion, Will Be Able To Organize The Provision Of The Service Better And More Effectively.
