Tenders Are Invited For Delivery Of Medical Gases

1. The Subject Of The Order Is The Successive Delivery Of Medical Gases For The District Hospital In Pruszków. 2. The Subject Of The Order Includes The Supply Of Medical Gases In Cylinders, Medical Oxygen In A Tank (Liquid Medical Oxygen In A Tank), Liquid Nitrogen, Oxygen And Carbon Dioxide In Cylinders Along With The Lease Of The Cylinder.3. Liquid Oxygen Will Be Supplied: Directly To A Cryogenic Tank With A Capacity Of 5,000 Kg Along With An Atmospheric Evaporator Located On The Tank Jacket Compressed Oxygen In Cylinders With A Capacity Of 2 L - 0.43 M3 (200 Bar), 10 L - 1.5 M3 Aluminum (150 Bar), 40 L 6.4 M3 (150 Bar).4. The Contractor Will Deliver And Install Within 24 Hours From The Date Of Notification By The Ordering Party And Will Lease A Cryogenic Tank For Liquid Medical Oxygen With A Capacity Of 5,000 Kg. The Tank Will Be Delivered With Full Equipment (Including Its Own Evaporator On The Tank Jacket), The Natural Evaporation Rate Of The Tank Is No More Than 0.5% Per 24 Hours. 5. The Contractor Will Lease Technically Functional Oxygen Cylinders To The Ordering Party For The Duration Of The Contract In The Quantity And Range Specified In The Price Form. 6. The Contractor Ensures Service Of The Delivered And Installed Cryogenic Tank For The Duration Of The Contract; Liquid Oxygen Will Be Delivered By Tanker With A Total Weight Not Exceeding 15 Tons.7. The Contractor, At His Own Expense, Will Install A Wireless System For Monitoring The Operation Of The Oxygen Tank And The Liquid Level In The Tank.8. The Subject Of The Order Also Includes Carbon Dioxide For Laparoscopy/Endoscopy In 10-Liter Cylinders. The Contractor Will Lease Technically Efficient Cylinders For Laparoscopy/Endoscopy To The Ordering Party For The Duration Of The Contract, In The Quantity And Range Specified In The Price Form.9. The Contractor Will Ensure The Delivery Of Liquid Nitrogen In A Small Tanker Approved For The Transport Of Hazardous Gases. Supplies Of Liquid Nitrogen, Including Quantities, Will Be Ordered Successively Depending On Needs.10. The Order Processing Time Cannot Be Longer Than 3 Business Days From The Date Of Placing The Order By E-Mail, Confirmed In Writing By E-Mail.11. The Contractor Must Deliver The Subject Of The Order To The Ordering Party At His Own Risk And Expense.12. A Description Of The Properties, Safety Rules, Expiration Dates And The Marketing Authorization Number Must Be Included On The Packaging, If Such An Indication Is Provided In Accordance With The Applicable Regulations.13. The Contractor Will Provide Products That Meet The Requirements And Are Manufactured In Accordance With Applicable Law, Including In Particular: Act Of September 6, 2021 - Pharmaceutical Law. 14. The Contractor Is Obliged To Absolutely Guarantee The Fulfillment Of The Quality Conditions Specified In The Production Permit Or Other Conditions Established By The Ministry Of Health On The Basis Of Which They Were Admitted To Trading And Compliance With The Expiry Dates For The Delivered Goods.15. The Offered Medical Gases Should Have Confirmation Of Entry In The Register At The Office For Registration Of Medicinal Products, Medical Devices And Biocidal Products.16. The Offered Subject Of The Order Should Have A Declaration Of Conformity With The Essential Requirements For A Medical Device Marked With The Ce Mark.17. If Quality Defects Or Quantity Deficiencies Are Found, The Ordering Party Will Immediately Notify The Contractor Of This Fact, Who Will Be Obliged To Replace The Goods With Defect-Free Ones Or To Remedy The Quantity Deficiencies Within 2 Business Days (I.E. From Monday To Friday, Excluding Saturdays And Public Holidays). Work), Counting From The Moment The Contractor Is Notified About The Defect Or Lack.18. The Contractor Must Deliver The Subject Of The Order To The Ordering Party At His Own Risk And Expense. Transport Of Medical Gases Is Included In The Gas Price.19. The Description Of The Properties, Safety Rules, Expiration Dates And The Marketing Authorization Number Must Be Included On The Packaging, If Such An Indication Is In Accordance With The Applicable Regulations.20. The Offered Product Must Have A Production And Expiration Date On Each Packaging. The Contractors Cylinders Must Meet All Requirements And Standards Regarding Pressurized Cylinders (Color Code Marking On Each Cylinder). The Contractor Is Obliged To Absolutely Guarantee That The Safety Conditions Of The Delivered Cylinders Are Met. Cylinders Should Be Technically Efficient And Legal.21. Each Time The Contractor Delivers, The Contractor Will Attach An Invoice Containing The Name Of The Goods, Quantity, Total Price, Unit Prices And Payment Deadline, As Well As A Quality Control Certificate For The Supplied Gas.22. The Subject Of The Order Was Also Described In Annex No. 2 To Swz.23. The Conditions For The Implementation Of The Subject Of The Order Were Also Included In The Draft Provisions Of The Agreement, Constituting Annex No. 5 To The Swz.
