The Subject Of The Order Is The Successive Delivery Of Professional Disinfectants For Use In Medical Offices Of Sanatorium Departments And In Offices Of Treatment Bases In The Facilities Of Uzdrowisko Kołobrzeg S.A. In Kołobrzeg In Accordance With Annex No. 1.3. Disinfectants Delivered Are Transported By The Contractor.4. Deliveries According To Monthly Needs,5. Deliveries At The Suppliers Expense To The Indicated Place,6. Payment By Transfer 30 Days From Invoice Issuance7. All Preparations Offered Must Be Intended For Use In The Medical Area, I.E. In Doctors Offices, Nursing Treatment Rooms, Isolation Rooms, And Treatment Base Offices.8. The Offered Disinfectants Should: A) Meet The Requirements Specified By Law, B) Be Of High Quality And Fulfill The Function For Which They Were Intended, C) Be Registered Medical Devices With A Broad Spectrum Of Activity, C) Have A Shelf Life Of Not Less Than 12 Months, Counting From Delivery Date, D) Do Not Have Mutagenic Or Carcinogenic Properties, E) Should Not Damage The Surfaces On Which They Will Be Applied. F) Marketing Authorization9. The Supplied Disinfectants Should Be Intended Only For Use In The Medical Area; In Doctors Offices, Nursing Treatment Rooms, Isolation Rooms, In Offices Of Treatment Bases.10. The Products Should Have Valid Safety Data Sheets, Marketing Authorizations, Relevant Permits, Tests, Etc. 11. Any Changes In The Documentation Of The Supplied Disinfectant Regarding: Withdrawal, Restriction Of Use/Application, Significant Change In Clinical Trial Results, Etc. Should Be Immediately Provided To The Recipient. 12. Sprinklers, Measuring Cups/Pumps/Dosing Devices, Taps For Pouring Preparations, Etc. Must Be Provided Free Of Charge To The Recipient.13. The Funds Are To Be Delivered At The Suppliers Expense In Original Packaging, Without Damage, Packed In A Way That Protects Them Against Damage. 14. We Reserve The Right To Deliver Agents (Substitutes) Other Than Those Indicated In The Offer15. In The Event Of Unavailability Of The Agent, Only After Agreement And With The Consent Of The Recipient, It Is Possible To Deliver A Disinfectant With Identical Properties And An Identical Spectrum Of Action And At A Price As The Agent Indicated In The Offer.16. Deliveries Are To Be Made Successively In Batches In Accordance With Partial Orders Specifying The Quantity And Type Of Disinfectants.17. The Supplier Is Obliged To Deliver The Preparations To Individual Facilities At The Indicated Address18. Completion Of The Order Within No Longer Than 36 Hours From The Moment Of Placing The Order, Excluding Non-Working Days, I.E. Saturdays, Sundays And Holidays.19. The Supplier Will Accept Orders For The Delivery Of Specific Products From Monday To Friday From 8.00 A.M. To 1.00 P.M., By Phone , E-Mail Or Fax.20.The Supplier Is Obliged To Collect, At His Own Expense, Packaging For The Delivered Disinfectants.21.The Supplier Is Obliged To Conduct Training For The Recipients Employees Who Use Occupational Health And Safety Measures Using The Delivered Disinfectants.22. On The Day Of The First Delivery, The Contractor Is Obliged To Provide Sprinklers, Measuring Cups/Pumps/Dosing Devices, And Taps For Pouring The Preparations Free Of Charge.23. The Funds Must Be Delivered At The Suppliers Expense In Original Packaging, Without Damage. No Substitute Labels Are Allowed. The Following Must Be Visible On The Packaging: Product Name, Expiry Date, Batch Number, Quantity/Weight Of Packaging, Manufacturer, Occupational Safety Measures, Inscriptions On Collective And Unit Packaging Must Have Labels In Polish. 24. The Supplier Undertakes To Collect The Packaging Of The Delivered Disinfectants.25. The Ordering Party Requires The Order To Be Processed Within No Longer Than 36 Hours From The Moment Of Placing The Order, Excluding Non-Working Days (Saturday, Sunday, Holidays).26. The Supplier Is Obliged To Deliver The Preparations To Individual Facilities Of The Spa At The Indicated Address.27. The Delivered Funds Must Be Packaged In A Way That Protects Them Against Damage28. The Ordering Party Reserves The Right To Reduce The Number Of Product Items Purchased Under This Agreement, Listed In The Offer, Depending On The Current Needs Of The Ordering Party.29. Reducing The Quantity Of The Purchased Subject Of The Contract (E.G. Due To Lower Consumption) Will Not Result In Any Legal And Financial Consequences For The Ordering Party.30. . The Supplier Is Obliged To Deliver The Ordered Disinfectant Indicated In The Annex To The Offer.31. If The Agent Specified In The Agreement Is Unavailable, The Supplier, After Consultation And With The Consent Of The Ordering Party, Is Obliged To Provide The Ordering Party With A Disinfectant With Identical Properties And An Identical Spectrum Of Action And At The Price Of The Agent Specified In The Agreement.32. The Supplier Is Obliged To Provide The Ordering Party With Disinfectants In Accordance With The Agreement. 33. In The Event Of Failure To Comply With The Terms Of The Contract, The Ordering Party Will Purchase Funds From Another Supplier And Charge The Costs Indicated In The Contract.34. The Ordering Party Reserves The Right To Supply Other Means Than Those Indicated In The Annex To The Contract.35. The Supplier Will Conduct Occupational Health And Safety Training Using The Supplied Disinfectants For Employees Of Uzdrowisko Kołobrzeg S.A. Using Funds.36. Deliveries Will Be Made Successively In Batches In Accordance With Partial Orders Specifying The Type And Quantity Of Disinfectants By Authorized Persons.
