2. The Subject Of The Order Includes The Execution Of The Three Indicated In Section. 3 Investments. All Three Investments Will Be Implemented At The Same Time, On One Facility, Located In Krotoszyn At Ul. Bolewskiego 4-8. For The Three Investments Constituting The Subject Of The Order, One Design Documentation, Stwior, Detailed Designs Were Prepared And One Wwkz Permit As Well As One Construction Permit Were Issued. Implementation Of The Investments Indicated In Section 3 Will Be Financed From Three Separate Sources (Promise). Settlements Will Be Made Separately For Each Of The Three Indicated Investments, In Accordance With The Principles Set Out In This Swz And The Draft Agreement - Annex No. 4 To The Swz.3. The Subject Of The Order Is: 1) Increasing Access To Long-Term Care In Areas Of Former State Agricultural Farms By Adapting The Hospital Building To Zol And Zop. The Investment Is Covered By Co-Financing From The Polish Government Fund: Strategic Investment Program. Description Of The Investment: The Task Is A Stage Of The Investment Consisting In The Reconstruction And Change Of Use Of The Former Hospital Building At Ul. Bolewskiego 4-8 For A Care, Treatment And Palliative Care Facility. The Building Is Located In The Krotoszyn District, Where There Are Many Areas Of Former State Agricultural Farms. The Investment Is Aimed At Increasing Access To Long-Term Stationary Care Services, Including Palliative Care, Among Others. For Former State Farm Employees. As A Result Of Its Implementation, The Quality Of Services Provided And Health Safety Will Improve. Former State Agricultural Farm Employees Are Elderly People, Often Chronically Ill, Requiring Support In Everyday Functioning, Which Poses Challenges To The Health Care System, Including: With The Provision Of Nursing And Care Services Within Long-Term Care For This Group. Statistics Show That The Availability Of Long-Term Care Beds Is Insufficient. The Waiting Time Is From 144 To 293 Days. Adaptation Of The Building At Ul. Bolewski For Long-Term Care Purposes Will Result In An Increase In The Available Places To 63 (48 Zol + 15 Zop). The Planned Activities Include Partial Renovation Of The Facade And Part Of The Roof, External Electrical And Sanitary Installation Works, Development Of The Area Around The Building And Reconstruction Of The Fence. The Renovation Of The Facade Will Include, Among Others: Cleaning Of Wall Surfaces, Desalting Of Walls, Renovation Plastering, Painting Of Plasters, Filling Of Defects With Bricks, Impregnation Of Walls, Reprofiling Of Walls, Repair Of Cracks In Walls, Local Patination And Coloring Of Putties, Hydrophobization Of Brick Surfaces, Partial Replacement Of Gutters, Execution Of New Sheet Metal Works. It Is Necessary To Install New Roof Windows On The Roof. The Planned External Sanitary Installations Include The Construction Of A Water Supply Connection, A Sanitary Sewage Connection, A Rainwater Drainage System, Connections To The Medical Gas Installation From The Medical Oxygen Expansion Building And The Compressor And Vacuum Building To The Adapted Building, Heating Connections For Central Heating, Domestic Hot Water, And Circulation. External Electrical Installation Works Will Include A New Power Connection To The Building And External Lighting. As Part Of The Development Of The Area, Internal Roads, Sidewalks, Parking Lot, Lawns Around The Building Will Be Constructed, And The Fence On The Street Side Will Undergo Renovation Works, Along With Renovation And Strengthening Of The Foundations.2) Modernization Of The Building Of The District Hospital In Krotoszyn At Ul. Bolewski For The Care And Treatment Plant - Stage I. The Investment Is Covered By Co-Financing From The Government Fund Polish Order: Strategic Investment Program. Description Of The Investment: The Implementation Of The Project Aims To Increase The Availability Of Stationary Medical Care Services In The Greater Poland Voivodeship, Including Palliative Care And Long-Term Care . Moreover, As A Result Of The Investment, The Quality Of Services Provided, Living Conditions, Health Safety And Sanitary Conditions Will Improve. According To The Current Map Of Health Needs For The Period From January 1, 2022 To December 31, 2026, A Continuous, Progressive Process Of Population Aging Is Observed Both In The World And In Poland. Polish Society Is One Of The Fastest Aging Among Eu Countries. An Aging Society Means An Increase In The Number Of People Who Are Chronically Ill And Require Support In Everyday Functioning, Which Poses New Challenges For The Polish Health Care System, Including: With The Provision Of Nursing And Care Services Within Long-Term Care. The Task Is A Stage Of The Investment Consisting In The Reconstruction And Change Of Use Of The Former Hospital Building At Ul. Bolewskiego 4-8 In Krotoszyn For A Care, Treatment And Palliative Care Facility. Until March 2022, Other Hospital Departments Operated In The Facility, Which Were Moved To The Newly Built Part Of The Hospital At Ul. Mickiewicza. The Task Was Included In The Local Revitalization Program Of The City Of Krotoszyn. The Planned Works Will Include Renovation Of The Main Entrance Door And The Balustrade Of Internal Stairs, Installation Of Radiators, Partial Replacement Of Windows And Window Sills In Order To Improve Energy Efficiency And Adapt To Fire Protection Requirements. Installation Of Glazed Display Cabinets In Order To Create Additional Space For Rehabilitation Treatments And Conducting Motor Classes With Patients. Installation Of Internal And External Doors To Comply With Applicable Standards And Regulations. Construction Of Walls And Partitions To Reduce The Number Of Beds In Patient Rooms, Which Will Increase The Level Of Intimacy And Ergonomics Of The Rooms. The Implementation Of This Investment Will Have A Key Impact On Improving The Comfort Of Patients And Their Families.3) Modernization Of The Building Of The District Hospital In Krotoszyn At Ul. Bolewski For The Care And Treatment Facility Along With The Necessary Technical Infrastructure - Stage Ii. The Scope Of Works Includes: - Construction Of Moisture-Proof And Thermal Insulation Of Foundations, Floors And Ground Floor Walls As Well As New Floors; - Reconstruction Of The Room Layout; - Execution Of New Plasters And Gypsum Plasters; - Wall Insulation; - Construction Of Newly Designed Roof Windows; - Replacement Of Floor Coverings With Seamless Pvc; - Construction Of New Waterproof, Washable Wall Coverings In Wet/Humid Rooms, Such As Showers; - Painting The Walls With Easily Washable Paints; - Construction Of A New Elevator Shaft And Installation Of A Crane For Transporting People On Stretchers And Hospital Beds, Installation Of A Lift For Nps; - Installation Of Suspended Ceilings; - Installation Of Bumpers, Covers, Protective Tapes, Markings And Other Elements Facilitating The Movement Of People With Limited Mobility And Perception And Increasing Patient Safety, Including Those Resulting From The Accessibility + Standards; - Execution Of Central Heating Installation. And Technological Heat, Domestic Hot Water, Cold Water, Sanitary And Rainwater Sewage, Electrical, Fire Protection, Emergency Calls, Access Control, Rtv-Sat, Cctv Ip, Computer, Medical Gases With Power Sources, Ventilation And Air Conditioning With Control Units; - Construction Of Reinforced Concrete Stairs. The Current Infrastructure Is Insufficient And Inappropriate To Meet The Needs Of People Requiring Long-Term Care. The Task Was Included In The Local Revitalization Program Of The City Of Krotoszyn.4. A Detailed Description Of The Subject Of The Order Is Included In The Design Documentation, Conservation Work Program, Detailed Designs, Stwior, Wwkz Permit, Wpwis, Wkwpsp Decision, Construction Permit, Constituting Appendices No. 2A To 2D Of This Swz, Respectively. The Ordering Party Informs That The Attached Bills Of Quantities/Offer Cost Estimates/Blind Estimates Are Of Illustrative/Informative Nature And Cannot Constitute The Sole Basis For Calculating The Offer Price. The Values Presented In The Bill Of Quantities Are For Auxiliary Purposes In Calculating The Offer Price, And The Price Offered By The Contractor Is A Lump Sum And Therefore Should Include All Expenditure That Is Necessary To Complete The Subject Of The Order Described In The Swz And Design Documentation.
