The Subject Of The Order Is The Provision Of Specialized Speech Therapy Counseling For Children, Adolescents, The Elderly And People With Aphasia For Residents Of The Szyd owo Commune As Part Of The Project Entitled Development Of The Greater Poland Network Of Social Service Centers Co-Financed By The European Union From The European Funds For Greater Poland 2021 2027. Measure Fewp.06.13 Social And Health Services.2. The Above Service Will Consist In Particular In 1) Conducting Consultations On Speech Development, Communication, Food Selectivity, 2) Providing Tips On Speech And Language Development For Caregivers, 3) Instruction On Supporting The Child In Development, 4) Maintaining Documentation. 3. The Services Will Be Provided At The Place Indicated By The Contractor4. The Planned Number Of Services During The Period Of Their Provision Is 1,215 Hours, I.E. An Average Of 45 Hours Per Month. (27 Months)5. Services Will Be Provided 5 Days A Week, From 08:00 (Earliest Service Start Time) - 18:00 (Latest Service End Time).6. The Duration Of The Service Is 60 Minutes. 7. The Contractor Will Provide Services On The Basis Of Individual Service Orders Submitted By The Ordering Party, Specifying The Start And End Date And The Number Of Hours.8. The Contractor Will Provide A Place To Provide The Service On Its Own, Adapted To The Provision Of The Service And Meeting The Requirements Contained In The Quality Standards Of Social Services In The Szyd owo Commune. The Contractor Will Include His Address In His Offer. 9. The Contractor Is Obliged To Keep Work Cards According To The Template Provided By The Ordering Party, Which Will Constitute An Attachment To The Invoice/Bill.10. The Service Should Be Completed Within 7 Days From The Date Of Submission Of A Written Order By The Ordering Party. 11. The Contractor Will Be Obliged To Constantly Cooperate And Consult With The Ordering Party At Every Stage Of The Execution Of The Order In Question.12. The Ordering Party Reserves The Right To Control The Implementation Of The Subject Of The Order At Every Stage Of Work.13. Settlements Between The Ordering Party And The Contractor Will Be Made In Polish Zlotys.14. A Detailed Description Of The Subject Of The Contract And The Conditions For The Performance Of The Contract Are Set Out In Annex No. 2 To The Swz (Drafted Provisions Of The Public Procurement Contract).15. The Ordering Party Indicates That The Contract Was Not Divided Into Parts Due To The Inability To Coordinate The Activities Of Various Contractors Implementing Individual Parts Of The Contract, Which Could Seriously Jeopardize The Proper Performance Of The Contract. Failure To Divide The Contract Into Lots, Due To Its Small Scope And Size, Will Not Hamper Competition.
