The Approved Provider List (Apl) 1 - Care At Home (Domiciliary Care) Service Will Be Delivered In The Main In The Customers Own Home (Including Community Based Settings Such As Independent Living Communities), But Also In The Community Where It Is Appropriate To Enable The Customer To Achieve Their Agreed Outcomes. The Service Is Available To All Adults Aged 18 Years And Over Who Live Within The City Of York Council Boundaries And Have Been Referred To The Service By Adult Social Care (Asc).
