1. General Informationthe Subject Of The Order Is The Provision Of Veterinary Care Services For Homeless Animals And Free-Living Cats From The Bielany District Of The Capital City. Warsaw. A Detailed Description Of The Subject Of The Contract Is Specified In: A) Description Of The Subject Of The Contract Constituting Annex No. 7 To The Swz, B) Price Form Constituting Annex No. 6 To The Swz, C) Designed Provisions Of The Contract - Constituting Annex No. 2 To The Swz. The Ordering Party Reserves That The Given Quantities In Individual Items Of The Price Form Are Approximate And May Change Depending On Actual Needs During The Term Of The Contract, However, The Final Scope Of The Service May Not Exceed The Amount Resulting From The Offer.2. Location: The Animal Treatment Facility (Hereinafter Referred To As Zlz) Accepting Animals Under Veterinary Care Must Be Located In The Bielany District Of The Capital City Of Warsaw. Warsaw Or In Another Local Government Unit, With Good Transport Connection*, However, Two Conditions Must Be Met: - The Medical Facility Will Be In The Area Served By Transport Organized By The Public Transport Authority, - By * Good Transport Connection, The Ordering Party Means Access By One Means Of Transport Urban From The Last Stop Located In The Bielany District To The Stop Located At A Distance Not Exceeding 500 M From The Clinic (Distance In A Straight Line), In A Time Not Exceeding 60 Minutes (Maximum Travel Time Determined On The Basis Of The Timetable Of The Public Transport Authority).3. Division Of The Contract Into Lots.1) The Ordering Party Awards The Contract In Parts, Each Of Which Is The Subject Of A Separate Procedure: No2) It Is Possible To Submit Partial Offers: Not Justification: The Contract Was Not Divided Into Parts Due To The Fact That It Is A Homogeneous Contract Provided By Veterinarians. Dividing The Contract Into Smaller Orders Would Be Pointless And Could Even Cause Unfavorable Consequences For The Ordering Party In The Form Of, For Example, An Increase In The Offered Prices Or The Impossibility Of Deciding On The Procedure Due To The Fact That Submitting Offers For Such Small Parts Of The Order Would Be Unprofitable For The Contractors. Due To The Above, Partial Offers Were Also Not Allowed. This Does Not Threaten To Restrict Competition And Does Not Constitute An Obstacle To Submitting Offers For Small And Medium-Sized Enterprises.4. Accessibility For People With Disabilitiesnot Applicable5. Social Clausesthe Ordering Party Provides Requirements Related To The Implementation Of The Order In Terms Of Employment By The Contractor Or Subcontractor Under An Employment Relationship, In The Circumstances Referred To In Art. 95 Of The Public Procurement Law, Which Are Included In 1 Section 6 Draft Provisions Of The Contract - Constituting Annex No. 2 To The Swz.
