1. The Subject Of The Order Is The Renovation Of Bituminous Surfaces With Emulsion And Grit Of Roads And Streets In The City And Commune Of Wieliczka In 2024.2. The Scope Of Works Includes In Particular: 2.1) Renovation Of The Bituminous Surface With A Thickness Of 4 Cm With Basalt Grits And Quick-Disintegrating Asphalt Emulsion Along With Supplementation With Stone Aggregate Or Ground Recycline Of Deeper Potholes To A Depth Of 4 Cm - 580.00 M2; 2.2) Surface Consolidation On The Bituminous Surface, Sealing Cracks Or Leveling Surface Cracks With A Thickness Of 1.0 -3.0 Cm With Asphalt Emulsion, Modified Quick-Disintegrating And Basalt Grits - 2670.00 M2;3. The Scope Of Works Includes Partial Renovation Of Road Sections, Each Time Agreed With The Investor. The Deadline For Commencing The Work Is Within 24 Hours Of Receiving Information From The Ordering Party, And In Special Cases (Threaking A Threat To Road Traffic Safety) Up To 6 Hours From Being Notified By Phone Or Fax. Note!: Before Starting The Works, A Project Must Be Prepared For A Temporary Change In The Traffic Organization On Time. Carrying Out Works In The Road And Approve It At The District Office In Wieliczka Ul. Słowackiego 29, 32-020 Wieliczka.4. Common Public Procurement Dictionary Cpv:45233220-7 Road Surface Works5. The Execution Of The Order Is Subject To Polish Law, In Particular The Act Of July 7, 1994. Construction Law (Consolidated Text: Journal Of Laws Of 2023, Item 682, As Amended), Act Of April 23, 1964 Civil Code (I.E. Journal Of Laws Of 2023, Item 1610, As Amended) And The Act Of September 11, 2019 Public Procurement Law (I.E. Journal Of Laws Of 2023, Item 1605, As Amended).6. If The Origin (Brand, Trademark, Manufacturer, Supplier) Of Materials Or Standards, Approvals, Specifications And Systems Referred To In Art. 99 Section 5 Of The Public Procurement Law, The Ordering Party Allows The Offer Of Materials Or Equivalent Solutions Provided That They Guarantee The Proper Implementation Of The Works And Ensure That Technical Parameters Are Not Worse Than Those Assumed In The Above-Mentioned Documents, In Particular In The Bill Of Quantities And Stwior. A Contractor Who Refers To Equivalent Solutions Is Obliged To Demonstrate That The Solution He Offers Meets The Requirements Specified By The Ordering Party. In Such A Case, The Contractor Attaches To The Offer A List Of Equivalent Solutions Together With Its Description Or Standards.7. The Provision Of Art. 101 Section 5 And 6 Of The Act Apply. 9. The Ordering Party Requires The Contractor To Provide At Least A 6-Month Warranty And Guarantee For The Works Performed, Counting From The Date Of Final Acceptance. The Extension Of The Guarantee And Warranty Is One Of The Criteria For Evaluating Offers In Accordance With Point 13 Of Swz.10. Additional Information Regarding The Subject Of The Order: A) The Contractor Will Make The Subject Of The Order Using Its Own Materials And Equipment. The Materials And Devices Used Should Comply With Declarations Of Compliance With Polish Standards, Certificates And Technical Approvals. The Contractor Is Obliged To Have Valid Documents For The Materials Used Confirming Permission To Use/Install Them (E.G. Approvals, Certificates, Technical Approvals, Declarations Of Conformity). The Transport Of Materials To The Construction Site And The Delivery And Operation Of Machines And Devices Are The Responsibility Of The Contractor. The Contractor Is Obliged To Perform Construction Works In Accordance With Construction Practice, Applicable Regulations And Standards And In Compliance With Occupational Health And Safety And Fire Protection Regulations. And Maximum Reduction Of The Nuisance Of Works. The Contractor Also Guarantees The Execution Of The Subject Of The Order Under The Supervision Of Persons With The Required Professional Preparation To Perform Independent Functions In The Construction Industry. B) During The Performance Of The Works, The Contractor Is Responsible For The Safety Of His Employees And Other Persons Located Within The Transferred Construction Site Due To The Works Carried Out. C) Before Proceeding With The Final Acceptance Of The Works, The Contractor Presents The Ordering Party With Reports On The Tests And Examinations Performed That Were Necessary For The Proper Execution Of The Order, Certificates Or Approvals For The Materials Used, And Documentation Regarding The Construction Works Performed (As-Built Inventory).11. In Order To Secure The Implementation Of The Subject Of The Order, The Ordering Party Obliges The Contractor Whose Offer Is Selected As The Most Advantageous In These Proceedings To Have Paid Civil Liability Insurance For The Activities Related To The Subject Of The Order Throughout The Entire Period Of The Order, Up To A Minimum Amount Of Pln 50,000.00.12. The Ordering Party Pursuant To Art. 95 Section 1 Of The Public Procurement Law Requires The Contractor Or Subcontractor To Employ, On The Basis Of An Employment Relationship, Persons Performing Activities Related To The Execution Of The Order, In Accordance With Point 20.2. Swz.13. The Ordering Party Informs That It Is Possible To Conduct An On-Site Visit, But It Is Possible To Submit An Offer Without It. The Ordering Party Does Not Anticipate Rejecting The Offer Of A Contractor Who Did Not Carry Out An On-Site Visit As Part Of The Proceedings In Question. Due To The Fact That The Future Construction Site Is Generally Accessible And Not Fenced, It Is Possible For Interested Contractors To Carry Out An Optional Site Visit On Their Own. All Necessary Documents Are Posted On The Website Of The Ongoing Procedure And Therefore The Ordering Party Does Not Provide For The Possibility Or Requirement For The Contractor To Check The Documents Necessary To Complete The Order Available On-Site At The Ordering Party.
