Work Detail

The Subject Of The Order Is The Provision Of Veterinary Services For Service Dogs And Horses Owned By The Provincial Police Headquarters In d And The Units Of The d Garrison. A Detailed Description Of The Subject Of The Order, Depending On The Part, Is Included In Annex 5.1-5.2 To The Swz. The Services Will Be Provided Successively During The Contract Period. The Contractor Undertakes To Implement The Subject Of The Order In Accordance With The Provisions Of The Contract, The Template Of Which Is Included In Annex No. 9 To The Swz. Pursuant To Art. 95 Section 1 Of The Act On Public Procurement, The Ordering Party Requires That The Contractor/Subcontractor, During The Period Of Contract Implementation, Employs Persons Performing Activities Related To The Treatment Of Animals Under An Employment Contract Within The Meaning Of The Provisions Of The Act Of June 26, 1974 - Labor Code (Journal Of Laws Of 2023, Item 1465). The Above Requirement Does Not Apply To Natural Persons Running A Business To The Extent That They Will Personally Provide Services To The Ordering Party. Provisions Regarding The Method Of Documenting Employment And Monitoring The Contractors Or Subcontractors Compliance With The Requirements Regarding Employment Under An Employment Contract And Provisions Regarding Sanctions For Failure To Meet The Requirements Specified In Art. 95 Section 1 Is Specified In Paragraph 5 Of Each Contract Template (Annex No. 9 To The Swz). Part 1. Provision Of Veterinary Services For Service Dogs. 1. The Subject Of The Order Is The Provision Of Veterinary Services For Service Dogs Registered With The Provincial Police Headquarters In d And Units Of The d Garrison2. The Contractor Undertakes To Properly Maintain Up-To-Date Medical Records Of Treated Animals And Complete The Documents Constituting Annexes To The Contract. The Ordering Party May At Any Time Inspect The Medical Records Of Treated Dogs And Request A Copy Of The Test Results And Dogs Treatment Records. The Term Medical Documentation Should Be Understood As: A Set Of Data On The Health Condition Of A Service Dog, Medical Services Provided To It, Medications Administered, Procedures Performed And Medical Visits. Proper Documentation Means: A) An Entry In The Documentation Is Made Immediately After The Service Is Provided; B) Each Entry Is Marked With The Data Of The Person Who Made It; C) An Entry Made In The Documentation Cannot Be Deleted From It, But It Can Be Changed In The Event Of An Error, However, A Detailed Annotation Must Be Included Specifying The Reason For The Error And The Name Of The Person Who Made It Changes To The Entry;D) The Pages Of The Medical Documentation Must Be Numbered;E) The Documentation Should Be Completed Legibly;F) Entries Should Be Made Chronologically;G) Each Page Of The Service Dogs Documentation Should Be Marked With At Least Its Name And Identification Number;H) Document , Which Has Been Included In The Documentation, Cannot Be Removed From It.3. The Ordering Unit Is Individually Obliged To Prepare An Order Before The Planned Visit With A Detailed Description Of The Reason For The Visit And Send It To The Gmt Department Of The d Police Headquarters In Order To Secure Financial Resources. After Obtaining Consent, An Employee Of The Gmt Department Of The Police Headquarters In d Will Send An Order By E-Mail To The Unit In Order To Arrange A Meeting With A Veterinarian. The Dogs Handler Is Obliged To Send The Order Accepted By The Gmt Department Of The Police Headquarters In d To The Contractor Or Show It To The Veterinarian During The Visit. In Exceptional Situations, A Veterinarian May Accept A Service Dog Without An Order Only In The Case Of An Emergency Visit (Threaking A Threat To The Dogs Life And Health), Each Subsequent Visit Requires An Order Accepted By The Gmt Department Of The Police Headquarters In d .4. In The Case Of Sudden Injuries Or Accidents, An Order Is Not Required, But After The Service Has Been Performed, The Dog Handler Is Obliged To Prepare A Detailed Official Note Signed By The Superior And Attach It To The Invoice Issued By The Veterinary Clinic. 5. Each Order Must Contain The Number / Month / Year / Kwp, Kmp Or Kpp / And The Date Of Issue.6. The Contractor Undertakes To Issue Invoices After Each Service Performed, Within A Period Not Exceeding 10 Days From The Date Of Its Completion And Sending The Invoice To The Ordering Partys Registered Office. Each Invoice Must Be Accompanied By An Order Submitted To The Clinic By The Unit And A Specification Of The Service Performed, Detailing All Activities Undertaken By The Veterinarian And The Medications Administered. If It Is Necessary To Administer Mineral And Vitamin Or Care Preparations, E.G. After Treatments/Surgeries, The Contractor Will Issue A Separate Invoice, Provided That He Has Obtained Written Consent From The Head Of The Gmt Department In d Before Using Them.7. The Scope Of Services Should Include, Among Others: 1. Periodic Veterinary Inspection Performed At The Contractors Headquarters And, If Necessary, On The Premises Of The Ordering Party, During Which The Following Activities Will Be Performed: - Weighing - Temperature Measurement - Auscultation - Inspection Of Eyes, Ears, Skin And Teeth - Palpating - Protection Against Fleas And Ticks And An Additional Dose For The Next 6 Months - Until The Next Periodic Inspection - Deworming - According To The Veterinarians Instructions, However, Not Less Than Twice A Year But Not More Than Four Times A Year. If, As A Result Of The Inspection, The Veterinarian Determines The Need For Care Procedures, The Veterinarian May Perform The Following Activities: - Cleaning The Ears - Trimming The Claws - Cleaning The Perianal Glands - Removing Tartar2. Protective Vaccinations Performed Once A Year At The Contractors Premises Or During A Periodic Inspection (If The Date Of The Next Vaccination Allows It) - Vaccination Against Rabies - Vaccination Against Infectious Diseases (Multi-Component Combined Vaccine Protecting Against, Among Others, The Following Diseases: Distemper, Adenoviral Infection And Parasitic Viral, Leptospirosis, Rubarths Disease, Parainfluenza)3. Diagnosing, Treating, Performing Tests And Procedures As Needed At The Contractors Premises Includes The Following Activities: - Diagnosis And Treatment Of Diseases Of The Respiratory, Circulatory, Digestive, Urinary And Nervous Systems, Dietary Treatment Of Internal Diseases, Treatment And Monitoring Of Chronically Ill Animals, - Diagnosis, Prevention And Treatment Of Infectious Animal Diseases, E.G. Parvovirus, Distemper, Etc., - Diagnosis Of Diseases Qualifying For Surgical Treatment, Minor Surgical Procedures, - Diagnosis Of Limb And Spine Injuries (X-Ray Examination), Treatment Of Bone And Joint Fractures, Application Of Stiffening And Plaster Dressings, Diagnosis And Treatment Of Joint Diseases. - Ophthalmological Examination, Conservative Treatment Of Eyeball Diseases, Suturing Of Eyelid Wounds, Etc. - Diagnosis Of Diseases Of The Reproductive System Of Females And Males, Diagnosis And Treatment Of Testicular And Prostate Diseases, - Diagnosis Of Ectoparasitic Diseases, Fungal And Bacterial Dermatoses And Diseases Allergic And Hormonal Skin Tests, Allergy Tests, Laboratory Examination Of Skin Scrapings And Ear Swabs, Performing Cultures And Anibiograms, Treatment Of Hair Coat Disorders, Dietary Treatment (Therapeutic Diets). - Performing Euthanasia, Visual Inspection And Collection Of Animal Carcasses Along With Issuing Appropriate Certificates And Protocols. The Ordering Partys Planned Services Are Those Referred To In Points 1-2, While Unplanned Services Are: Random Events, Diseases, Etc. 8. The Contractor Undertakes To: - Provide Veterinary Services At The Contractors Headquarters And, If Necessary, In The d Voivodeship. - Performing Periodic Examinations (Inspections) Of Dogs - Possible Participation In The Work Of The Team Responsible For Organizing The Use And Maintenance Of Dogs In Accordance With Order No. 91 Of The Commander In Chief Of Police Of October 4, 2022 - Ensuring Appropriate Housing Conditions And Technical And Medical Equipment Necessary For Proper Implementation Services - In The Case Of Services Provided At The Contractors Premises. - Keeping Up-To-Date Medical Records Of Treated Animals, Completing Documents Constituting Annexes To The Contract, Making Entries In Treatment Progress Cards And In Dogs Health Books. Including The Type Of Treatment Used, Including Procedures Performed And Medical Measures Used. - Present The Medical Documentation Of The Service Dog To The Ordering Party At Any Time. - Placing The Following Information In The Clinic In A Visible Place For Everyone: Service Dogs That Are In The Register Of The Provincial Police Headquarters In d Are Admissioned Out Of Their Turn9. The Ordering Party Reserves The Right To Change The Number Of Dogs (Reduce/Increase) Specified In Annex 2.1 To The Swz (Product And Price Form) In The Event Of Purchasing New Animals Or Withdrawing Them From Service, This Change Will Not Constitute A Change To The Terms Of The Contract.10. Determined In The Above-Mentioned Annex No. 2.1, The Quantities Of Services Are Estimated Values, Constituting The Basis For Preparing The Offer. The Actual Quantity And Type Of Services Will Depend On The Needs Of The Ordering Party And Its Budget.11. The Ordering Party Allows The Service To Be Performed In Another Veterinary Office In The Event Of Sudden Illnesses Or Random Events That Threaten The Dogs Health And Life12. The Contractor Undertakes To Provide Services In Accordance With The Legal Bases Applicable At The Provincial Police Headquarters In d , I.E.: 1. Regulation Of The Minister Of Internal Affairs And Administration Of December 22, 2021 On Service Dogs And Service Horses In The Police.2. Order No. 91 Of The Police Commander In Chief Of October 4, 2022 On Methods And Forms Of Performing Tasks Using Service Dogs In The Police.
