1. The Subject Of The Order Is The Modernization Of The Building At Ul. Modrzewiowa 5A In Świdnik, Owned By Lokum Tbs Sp. Z O. O. By Replacing Window Joinery And Blinds Between Windows Along With Insulation.2. The Task Concerns The Replacement Of Wooden Window Joinery With Pvc In One-Sided Golden Oak Color, Along With Glazing Beads, Air Vents, A Warm Window Package With A Frame And Handles, With A Coefficient U≤0.9 W/M2k. Insulation Of The Building In The Places Of Blinds Between The Windows Should Be Made With Eps 033 Thick Facade Polystyrene. 14 Cm, Including The Construction Of A Facade Imitating Wood. The Works Covered By The Specification Include All Activities Enabling And Aimed At: 1) Making A Window Or Balcony Door From Pvc Profiles Outside The Installation Site, 2) Delivery Of The Product To The Installation Site, 3) Replacing The Joinery Construction, 4) Dismantling The Blinds Between The Windows And Insulating Them With Wool, 5) Insulating The Spaces Between The Windows With Eps 033 Gr. Facade Polystyrene. 14 Cm,6) Finishing The Facade With A Board Imitating Wood In The Color Of Golden Oak,7) Processing Of Door Frames And Painting,8) Removal Of Demolition Material And Rubble.3. The Subject Of The Order Will Be Performed In A Residential Facility. 4. A Detailed Description Of The Subject Of The Order Is Included In The Annex To Swz.5. The Ordering Party Specifies That Persons Performing Certain Activities Aimed At Implementing The Order Are To Be Performed By Persons Employed Under An Employment Contract Throughout The Duration Of The Contract. Requirements Related To The Implementation Of The Order In Terms Of Employment By The Contractor Or Subcontractor On The Basis Of An Employment Relationship Of Persons Performing The Activities Specified By The Ordering Party In The Scope Of The Implementation Of The Order, If The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code, Include Persons Who Will Participate In The Execution Of The Order, I.E. With The Profile Of A Manual Worker Performing The Assembly Of The Subject Of The Order. In Order To Verify The Employment By The Contractor Or Subcontractor Under An Employment Contract Of Persons Performing The Above-Mentioned Activities, The Ordering Party Provides, During The Execution Of The Order, The Possibility Of Requesting In Particular: 1) A Declaration From The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Referred To In The Request. Ordering Party. This Declaration Should Include In Particular: Precise Identification Of The Entity Submitting The Declaration, Date Of Submission Of The Declaration, Indication That The Activities Covered By The Request Are Performed By Persons Employed Under An Employment Contract, Together With An Indication Of The Number Of These Persons, Their Names And Surnames, The Type Of Employment Contract And The Salary. Full-Time Position And The Signature Of The Person Authorized To Submit A Declaration On Behalf Of The Contractor Or Subcontractor, 2) A Copy Of The Employment Contract/Contracts Of Persons Performing The Activities Referred To Above During The Execution Of The Order, Certified As True Copies By The Contractor Or Subcontractor, Respectively. Declaration Of The Contractor Or Subcontractor (Together With A Document Regulating The Scope Of Obligations, If Prepared). A Copy Of The Contract(S) Should Be Anonymized In A Way That Ensures The Protection Of Employees Personal Data, In Accordance With The Provisions Of Regulation (Eu) 2016/679 Of The European Parliament And Of The Council Of 27 April 2016 On The Protection Of Natural Persons With Regard To The Processing Of Personal Data And On The Freedom To Flow Of Such Data And Repealing Directive 95/46/Ec (General Data Protection Regulation) (Oj Eu L 119 Of 04/05/2016, P. 1) (I.E. In Particular Without Addresses, Pesel Number Of Employees). The Employees Name And Surname Are Not Anonymized. Information Such As: Date Of Conclusion Of The Contract, Type Of Employment Contract And Working Hours Should Be Identifiable, 3) Other Documents Containing Information, Including Personal Data, Necessary To Verify Employment Under The Employment Contract, In Particular: Name And Surname Of The Employee Employee, Date Of Conclusion Of The Employment Contract, Type Of Employment Contract, Scope Of The Employees Responsibilities. The Ordering Party, Both Before Concluding The Contract And After Awarding The Contract, May Request That The Contractor Submit A List Of Persons Employed Under An Employment Contract Who Are To Perform The Subject Of The Contract. The List Is To Contain The Following Information: Names And Surnames, Dates Of Conclusion Of Contracts, Type Of Employment Contracts And Full-Time Hours Of Persons Employed Under An Employment Contract.
