Tenders Are Invited For Sanitary Transport Service For Ipin

1. The Subject Of The Order Includes The Provision Of Health Services In The Field Of Sanitary Transport Services: 1) Transport By A Specialized Ambulance With The Participation Of A Doctor, A Rescuer And A Driver. 2) Transport By A Basic Ambulance With The Participation Of A Rescuer And A Driver. 3) Transport By A Transport Ambulance With The Participation Of A Medic And A Driver. 4 ) General Ambulance Transport With The Participation Of A Doctor, Paramedic, Driver And 2 Security Workers (Patients Of The Forensic Psychiatry Clinic) 5) General Ambulance Transport With The Participation Of A Paramedic/Paramedic And Driver And 2 Security Workers (Patients Of The Forensic Psychiatry Clinic).2 . These Services Include: 1) Transport Of Patients Between Hospitals For Consultations And Diagnostic Tests, Transfer To Another Healthcare Entity, 2) Between The Hospital And The Patients Place Of Stay (From The Hospital As Well As To The Hospital At The Request Of The Ordering Party), 3) Transport Of Biological Material To Tests Or Other Medical Materials/Devices, Medical Records.3. The Services Provided Require: 1) Ensuring That The Sanitary Staff Of The Emergency Relief Team Is Staffed And Equipped In Accordance With The Standards And Applicable Regulations, Meeting The Technical And Quality Standards Specified In The Polish Standards Transposing European Harmonized Standards And In The Act On State Emergency Medical Services, 2) Ensuring Continuity In Securing Teams, 3) Maintaining Medical Confidentiality, 4) Max. Required Time Of Ambulance Arrival From The Moment Of Placing The Order By Telephone: A) Specialist Ambulance - Immediately, No Later Than 25 Minutes From The Cito Notification, B) Other Cases - Up To 45 Minutes.4. The Ordering Party Envisages Ordering, In Necessary Cases, Transport By All Types Of Transport At A Distance Of More Than 25 Km From The Ordering Partys Headquarters And Outside Warsaw. 5. The Ordering Party Requires That Urgent Transport Within A Distance Of 150 Km From The Ordering Partys Registered Office Be Carried Out On The Day Of Issuing The Transport Order.6. The Ordering Party Also Envisages, On The On-Call Days Of The Child And Adolescent Psychiatry Clinic (10-12 Shifts A Month), In Emergency Cases, The Transport Of Patients To Other Hospitals Located In Poland That Have A Children And Adolescent Ward In Their Structure. The Obligation To Search For And Indicate The Medical Facility To Which The Patient Should Be Transported Rests With The Ordering Party. 7. The Price Of One Trip Should Include The Cost Of Travel From The Base To The Institutes Headquarters And Should Be A Lump Sum Price For Transport Up To 25 Km From The Ordering Partys Headquarters In Warsaw. 8. Transport Over A Distance Of More Than 25 Km From The Ordering Partys Headquarters Will Be Charged At The Rate Per 1 Km. 9. The Contractor Is Not Entitled To Validate Applications (I.E. Service Orders) Ordered By The Ordering Partys Employees And Is Obliged To Fulfill Them At The First Request Of The Ordering Partys Employees. 10. Sanitary Transport Services Will Be Provided 24 Hours A Day, Every Day Of The Week.11. The Contractors Employees Providing Sanitary Transport Services Will Be Equipped With Personal Protective Equipment Appropriate To The Transport Performed And Uniform Protective Clothing (Uniforms), Distinguishing Them So That Outsiders Can Recognize Them As Sanitary Transport Workers.12. In The Offer Form, The Ordering Party Specifies The Expected Number Of Trips During The Implementation Of The Contract And Stipulates That This Average Is Based On Expectations Related To The Ordering Partys Current Activity And Is Used Only To Calculate The Offer Price.13. Trips Over A Distance Of 25 Km From The Ordering Partys Headquarters Will Be Settled According To The Prices Per 1 Km Provided By The Contractor In The Offer - And Are Not Included In The Calculation Of The Offer Price. 14. The Contractor Should Estimate The Parking/Waiting Time In The Tender Form, Provided That A Parking Time Of Up To 30 Minutes Is Included In The Travel Price.
