Qualified bidders/contractors owned by women are particularly encouraged to apply. - Price responsiveness: The supplier shall submit a filled and stamped quotation in both excel and PDF format. - Please note the successful bidder will be required to provide maintenance in case of any defects and/or technical failure in the future. - Supplier ethical standard form: Fill, stamp and submit the attached supplier ethical standard form. - Supplier who has a valid contract with NIS will not be considered in this bidding process. - Previous similar experience of supply and delivery: Shall submit ONLY 3 previous similar supply and delivery of medical equipment related contracts implemented with and/or for government institutions, International non-governmental organizations (INGOs) or UN agencies in the last 3 years. Note: Forget contracts will lead automatic disqualification. o Valid registration from relevant government authorities/bodies such us Minister of Health and Ministry of Commerce and Industry. - Tax compliance certificate: A valid tax compliance certificate (TIN) from Ministry of Finance. - Proof/evidence of supplier ownership: all suppliers shall submit a signed public notary document stating the ownership of the supplier including names, positions/titles, contacts details and shares owned.
