1. The Subject Of The Order Are Works Consisting In The Performance Of Partial Renovations Of Bituminous Road Surfaces In The City And Commune Of Busko-Zdrój. The Scope Of Work Includes: Task No. 1 - Performance Of Partial Renovations Of Bituminous Road Surfaces In The City Of Busko-Zdrój With A Mineral And Asphalt Mixture: - Milling To A Depth Of Up To 4 Cm, Removal Of Transverse And Longitudinal Humps, Removal Of Rubble From Milling To The Contractors Storage Yard, - Laying And Compacting A Mineral-Bituminous Mixture Of Thickness 4 Cm - Sprinkling The Bottom Of The Defect And Smearing The Walls With Asphalt Emulsion, Closing The Contact Between The Newly Made Surface And The Existing Surface By Lubricating The Edges Of The Renovated Hole With A Grout And Filling It With Crushed Sand, - Sealing The Cracks In The Surface By Widening The Gap With A Milling Machine, Cleaning The Space Between The Edges Of The Cracks And The Edges Themselves With Brushes Mechanical, Drying The Edges, Priming And Filling The Crack With Hot Grout And Backfilling With Fine, Dust-Free, Hot Crushed Sand, - Sealing Mesh Cracks In Asphalt Surfaces By Milling The Layer To A Depth Of Approx. 3 Mm, Cleaning The Surface And Then Filling The Defect With An Emulsion Using A Renovator And Backfilling With Fine, Dust-Free, Hot Crushed Sand, - Regulation Of Sewage Wells On A Ring With Milling Of The Surface Around The Well, Reconstruction Of The Surface Of The Defect Around The Well Manholes With Mineral And Bituminous Mass Along With Compaction, With A Width Around The Well Of 0.50 M. Task No. 2 - Execution Partial Renovation Of Bituminous Road Surfaces In The City And Commune Of Busko-Zdrój Using The Method Of Surface Consolidation With Asphalt Emulsion And Grit. Types Of Works To Be Performed Under Task No. 1: - Milling Of Bituminous Surface 4 Cm Thick With Removal Of Material Up To 5 Km - 350 M2, - Allowance For Increasing The Milling Depth By 1 Cm Over 4 Cm - 350 M2, - Installation Of Asphalt Mass (Cavities Up To 4.0 Cm Deep) - 500 M2, - Allowance For Adding Mass (Cavities More Than 4.0 Cm Deep ) - Each Next 1.0 Cm Thick - 560 M2, - Repair Of Cracks In Single Asphalt Surfaces Using A Milling Machine - 35 M2, - Repair Of Mesh Cracks In Asphalt Surfaces - 350 M2, - Adjustment Of Sewage Wells - 10 Pcs., - Elimination Of Breakthroughs (Involving On Removing The Existing Bituminous Surface And The Old Subgrade To A Depth Of 30 Cm And Making A New Subbase Of Crushed Aggregate Fraction 0-63 Mm (Gr. 30 Cm After Compaction) And A New Mineral And Asphalt Surface (2 Layers, 4 Cm Thick Each) - 35 M2. Types Of Works To Be Performed As Part Of Task No. 2: - Defects Up To 2.0 Cm Deep - 1000 M2, - Defects With A Depth From 2.0 Cm To 4.0 Cm - 1750 M2, - Defects With A Depth Exceeding 4.0 Cm - Each Additional 1.0 Cm In Thickness - 1000 M2, The Given Scopes Of Individual Works Are Estimated Ranges, Determined Based On Partial Road Renovations Carried Out In Previous Years. The Offer Value Calculated On This Basis Will Be Used To Compare Offers In Terms Of Price Criteria. Settlement Of The Completed Works Will Be Based On As-Built Measurements According To The Unit Prices Included In The Offer, Constituting Annex No. 1 To The Swz. Attention! Before Starting The Works, The Representative Of The Ordering Party, Together With The Representative Of The Contractor, Will Tour The Roads In The City And Commune Of Busko-Zdrój In Order To Determine The Number And Scope Of Works To Be Performed On Individual Streets And Non-Urban Roads. The Contractor Will Provide The Vehicle For Road Detour Purposes. The Ordering Party Does Not Provide For Payment For The Road Detour. The Annual Amount Of Works Is An Estimated Amount Determined On The Basis Of The Volume Of The Completed Order In 2022 - 2023. This Amount May Change (Reduce Or Increase The Scope). The Remuneration Due To The Contractor Will Be Determined Based On The Actual Amount Of Works Performed Taking Into Account The Unit Prices Given In The Offer. Securing And Marking The Work Sites Is The Responsibility Of The Contractor. The Contractors Material Is New, For Which It Has A Quality Certificate. Materials: 1. The Contractor Undertakes To Perform The Subject Of The Contract Using Its Own Materials And Raw Materials.2. The Materials Used To Perform The Works Should Meet The Quality Requirements For Products Approved For Trading And Use In Construction Specified In Art. 10 Of The Construction Law, The Requirements Of The Specification Of Essential Terms Of The Contract.3. Materials That Are Not Of The Specified Quality Will Be Rejected. Any Job Using Rejected Materials Will Not Be Accepted.4. At Each Request Of The Ordering Party (Supervision Inspector), The Contractor Is Obliged To Present In Relation To The Indicated Materials: Copies Of Purchase Invoices Along With A Safety Mark Certificate, Declaration Of Compliance Or Certificate Of Compliance With The Polish Standard Or Technical Approval. The Contractor Will Not Be Able To Demand Additional Remuneration Due To Ignorance Construction Site, Lack Of Access, Water Intake And Connections For The Construction Site, Etc. These Deficiencies Will Not Constitute A Sufficient Reason For The Contractor Not To Start The Works On The Scheduled Date. The Contractor Will Be Responsible For Any Damage To Roads, Drainage Ditches, Drainage System, Waterworks And Gas Pipelines, Power Poles And Lines, Cables, Geodetic Control Network Points And Installations Of Any Kind Caused By Him Or His Subcontractors During The Execution Of The Works. The Contractor Will Immediately Repair Any Damage At His Own Expense. The Contractor Undertakes To Perform The Subject Of The Contract In Accordance With The Practice Of Construction And Technical Specifications And Applicable Standards And Regulations And With Due Care. All Notifications And Correspondence Between The Ordering Party And The Contractor Should Be Delivered In Writing To The Registered Office Of The Ordering Party And The Contractor At The Address Indicated In This Agreement. The Service Will Be Performed On The Basis Of Written, Telephone Or E-Mail Messages Instructions Of The City And Commune Office Specifying The Type, Scope, Place And Date Of Execution Of The Works. The Contractor Is Obliged To: A) Secure The Area Covered By The Works And Ensure Traffic Safety, B) Secure The Existing Trees And Greenery, And In The Event Of Damage - Restore It To Its Original Condition, C) Installing Warning Signs In Accordance With The Provisions Of The Highway Code, D) Changing The Traffic Organization For The Duration Of The Works, E) Tidying Up The Area After The Works Are Completed. The Price Of The Order Should Include: A) Costs Of A Road Tour Carried Out Before The Commencement Of The Works In Order To Determine The Quantity And Scope Of The Works ,B) Partial Renovation Of The Surface Should Include The Costs Of Pavement Compaction,C) Costs Of Purchase And Transport Of Materials,D) Costs Of Renting And Operating Equipment And Utilities Necessary For The Operation Of The Construction Site,E) Costs Of Marking And Securing The Area Where The Works Are Carried Out,F) Costs Related To Tidying Up The Area After The Works Are Completed.2.
