1. The Subject Of The Order Are Services Of Providing 24-Hour Transport By Specialized Means Of Transport Meeting The Pn-En 1789 Standard For Type C Ambulances With A Doctor For The Independent Public Health Care Center Of The Ministry Of Internal Affairs And Administration In Krakow (Hereinafter Referred To As Sp Zoz Mswia In Krakow).2. The Estimated Quantity Of The Order Over A Period Of 36 Months Is Specified In Annex 2 To Swz.3. Conditions For Providing Health Services: I. The Provision Of Health Services Within The Scope Of The Subject Of The Order Includes In Particular: A) 24-Hour Provision Of Transport Meeting The Pn-En 1789 Standard For Type C Ambulances With A Doctor, On All Days Of The Week - Also On Saturdays, Sundays And Holidays, As Well As Public Holidays - - On Including Ensuring The Possibility Of Transporting An Intubated Patient (Ventilator, Oxygen, Infusion Pump);B) As Part Of The Received Rate, Covering All Costs Related To The Operation Of The Ambulance, Including Fuel Costs And Crew Costs;C) Ensuring The Continuity Of The Service Throughout The Duration Of The Contract;D ) Availability Of At Least 1 Ambulance Meeting The Pn-En 1789 Standard For Type C Ambulances, With A Doctor, With A Qualified Crew And Standard Equipment. The Crew Of An Ambulance Meeting The Pn-En 1789 Standard For Type C Ambulances With A Doctor Should Consist Of At Least 3 People, At Least One Of Whom Is A Doctor And The Remaining 2 Are Medical Professionals (Nurse Or Paramedic). If None Of These Three Medical Persons Is Authorized To Drive Emergency Vehicles, The Fourth Person May Be A Driver Without Medical Education. F) In The Event Of A Failure Of An Ambulance Meeting The Pn-En 1789 Standard For Type C Ambulances With A Doctor, The Contractor Is Obliged To Provide Within 15 Minutes, A Replacement Means Of Medical Transport For The Duration Of The Repair With Parameters Meeting The Requirements Of The Procedure. G) The Contractor Is Obliged To Ensure Constant Communication Between The Ordering Party And The Persons Performing The Transport Order. For This Purpose, The Contractor Should Provide The Ordering Party With Mobile Phone Numbers Enabling Contact With The Transport Operator. The Patient Must Be Transported Within A Maximum Of 60 Minutes From The Time Of Notification. The Declared Travel Times Do Not Apply During The Period Of Declaration Of The State Of Epidemic, If The Delay Was Caused By The Lack Of Means Of Transport In The Contractors Base Related To The Epidemic Situation And The Extended Time Of Carrying Out Transport Orders.J) The Transport Of The Patient Will Take Place From The Department From Which The Order For The Ambulance Is Issued And Back To The Ward; K) The Ordering Party Reserves The Right To Control The Authorizations Of The Ambulance Team, Its Equipment And The Working Time For The Ordering Party; L) Undertaking To Submit To The Control Of The Małopolska Provincial Branch Of The National Health Fund On The Principles Set Out In The Act Of August 27, 2004 On Health Care Services Financed From Public Funds, To The Extent Resulting From The Contract; M) Maintaining And Archiving Medical Records In Accordance With The Regulation Ministry Of The Interior And Administration Of April 29, 2020 On The Types, Scope And Patterns As Well As The Method Of Processing Medical Documentation In Medical Entities Established By The Minister Responsible For Internal Affairs (Journal Of Laws Of 2020, Item 788, As Amended) And Statistical Reporting Regarding The Subject Orders;N) Compliance With The Provisions On The Protection Of Personal Data, Occupational Health And Safety Rules, The Act On Health Care Services Financed From Public Funds, The Act On Medical Activities, The Act On Patient Rights And The Patient Ombudsman;O) Ensuring The Protection Of Data Covered By Medical Documentation; P) Submitting The Contract To The Providers Portal, In Accordance With The Requirements Of The Mow Nfz No Later Than 14 Days From The Date Of Conclusion Of This Contract. Q) Proper And Timely Performance Of Services. Ii. Other Conditions: The Scope Of Health Services Provided By A Medical Entity In Accordance With The Entry In The Register Of Entities Performing Medical Activities Must Correspond To The Subject Of The Order.
