1. The Subject Of The Order Is The Provision Of Services Of Collection, Transport, Storage And Release Of Bodies Of Patients Who Died At The Municipal Hospital In Ruda l ska Sp. Z O. O. And Conducting Autopsies And Examinations. A Detailed Description Of The Subject Of The Order In The Scope Of The Provision Of Services Is Specified In Annex No. 1, 2 To The Swz/Agreement And 11 To The Swz.2. Pursuant To Art. 95 Section 1 Of The Act, The Ordering Party Requires That Persons Performing Activities Directly Related To The Performance Of The Subject Of The Contract Be Employed Under An Employment Contract.3. The Requirements Regarding The Method Of Documenting The Employment Of A Person Under An Employment Contract, The Ordering Partys Control Rights And The Sanctions For Failure To Meet The Requirements Referred To In Point 2 Above Are Specified In Detail In The Draft Provisions Of The Contract, Which Will Be Introduced Into The Content Of The Procurement Contract (Appendix No. 11 To The Swz ).
