1. The Subject Of The Order Is To Act As An Inspector Of Investor Supervision Over Construction Works Carried Out As Part Of The Investment Task Entitled: Our Monument - Manor In Uniejów 2. The Order Includes Supervision Of The Execution Of Construction Works Regarding The Reconstruction Of The Building Of A Historic Manor House Serving As An Urban And Municipal Cultural Center And A Music School, Along With Land Development And The Necessary Technical Infrastructure As Part Of The Project Our Monument - Manor House In Uniejów. The Subject Of The Works Covered By Supervision Is Co-Financed By The Government Fund Polish Governance Of The Strategic Investment Program Edycja6pgr/2023/2023/1338/Polskilad; The Subject Of The Works Supervised Is Co-Financed By A Grant In The Our Monument Competition Organized By The Most The Most Foundation Based At Ul. Chmielna 73, 00-801 Warszawa.3. The Manor Building Is Located On The Plot No. 1522/1 In The Town Of Uniejów, In The B Spa Protection Zone Of The Uniejów Spa. The Building Is Protected On The Basis Of Entry In The Register Of Monuments Of The Łódź Voivodeship Under No. 360/102 Of August 10, 1984. The Scope Of Works Covered By The Supervision Includes: - Replacement Of The Roof Structure And Roofing; - Replacement Of Ceilings And Conservation Work On Ceiling Beams; - Masonry Works (Including Demolition, Wall Additions, Construction Of New Partition Walls); - Steel And Concrete Lintels; - Replacement Of Windows And Doors; - Flooring And Cladding Works; - Wall Cladding (Including Conservation Work On Built-In Wardrobes); - Ceiling Cladding And Suspended Ceilings; - Façade Works (Including Plastering Work On The Plinth, Sheet Metal Work And Drainage, Restoration Of Stucco, Repair Of Cornices); - Internal Electrical Installations (Disassembly Of Existing Installations, Construction Of The Buildings Grounding And Lightning Protection Installation, Assembly New Electrical Switchboards, Construction Of Electrical And Telecommunications Cable Routes, Installation Of Accessories, Installation Of Lighting Fixtures, Construction Of Outlets And Cables, Construction Of Call Installations, Ssp Installations, Sswin Installations And Construction Works); - Sanitary Installations (Including Central Heating Installations, Ventilation, Sanitary Sewage, Water Supply Installations) - Land Development Works (Works On External Stairs And Ramps - External Sanitary Installations (Including Rainwater Drainage), External Electrical Installations - Including Dismantling Works And Construction Of New Power Cable Lines And Cable Line Supplying The Lighting Pole); - Equipment (In Accordance With The Published Bill Of Quantities). In Accordance With The Permit For Carrying Out Construction Works On A Monument Entered In The Register Of Monuments, No. Wuoz-Zn.5142.286.2023.Hg Of April 4, 2023, During Earthworks, It Is Necessary To Carry Out Archaeological Research. The Contractor Of Construction Works Covered By Supervision Is Obliged To Apply To The Łódź Provincial Conservator Of Monuments For A Permit To Conduct The Above Research. The Contractor Of The Works Subject To Supervision Is Obliged To Perform These And Any Other Obligations Arising From The Decision Of The Łódź Provincial Conservator Of Monuments.4. The Obligations Of The Contractor Of The Works Covered By Supervision Will Also Include Removing And Transporting, Together With Bringing In The Current Equipment Of The Facility, To The Place Of Temporary (For The Period Of Works) Operation Of Mgok In Uniejów. The Place Of Temporary Activity Will Be Located In Uniejów. 5. The Scope Of The Supervised Works Also Includes The Execution And Placement Of A Temporary Information Board During The Implementation Of The Works On The Award Of A Grant For The Project By The Most The Most Foundation, And Then The Construction And Placement Of A Final Information Board On The Award Of A Grant For The Project By The Most The Most Foundation. If It Is Necessary For A Person Acting As An Inspector To Participate In The Implementation Of The Subject Of The Order Supervision Of The Telecommunications Industry, Authorized To Perform Independent Technical Functions In Construction In The Scope Of Managing Construction Works, In The Installation Specialty In The Field Of Telecommunications Networks, Installations And Devices, Or Having Other Equivalent Authorizations Issued On The Basis Of Regulations In Force On The Date Of Issue Of The Authorization Authorizing To Manage Construction Works In To The Extent Necessary To Implement The Subject Of The Order (Supervision Of Works), The Contractor Is Obliged To Ensure Her (Telecommunications Industry Supervision Inspector) Participation In The Implementation Of The Order.8. The Contractor Will Be Obliged To Provide The Investors Supervision Service In Accordance With Applicable Standards And Technical Knowledge, Specified, Among Others, The Act Of July 7, 1994 Construction Law (Consolidated Text: Journal Of Laws Of 2023, Item 682, As Amended) While Maintaining Due Diligence, Proper Work Organization And Occupational Health And Safety Rules.9. Exercising Investor Supervision Over The Implementation Of Construction Works In Particular Includes: A. Representing The Ordering Party On The Construction Site By Checking The Compliance Of The Works With The Technical Documentation, Applicable Regulations And Standards And The Principles Of Technical Knowledge; B. Verifying Design Documentation, Checking The Quality Of Completed Works And Built-In Construction Products, And In Particular Preventing The Use Of Defective Construction Products And Those Not Approved For Use In Construction. Agreeing With The Ordering Party On Any Deviations And Changes In Work Technology, Materials And Equipment Reported By The Work Contractor; D. Checking And Acceptance Of Construction Works That Are Covered Or Disappearing, Participating In Tests And Technical Acceptance Of Installations, Technical Devices, As Well As Preparation And Participation In Partial Acceptance Activities, Acceptance Of Finished Construction Objects And Putting Them Into Use. E. Confirmation Of Actually Performed Works And Removal Of Defects, As Well As, At The Investors Request, Controlling Construction Settlements.10. Moreover, The Investor Supervision Inspector Is Obliged To: A. Controlling The Correctness Of Keeping The Construction Log And Making Entries In It Confirming All Circumstances Relevant To The Proper Construction Process And The Valuation Of Works. B. Resolving Technical Doubts Arising In The Course Of Works, And, If Necessary, Obtaining The Opinion Of The Design Author, Except For Matters Affecting The Change In Construction Costs. C. Submitting Reports On The Progress Of The Investment At The Request Of The Ordering Party And Any Difficulties Encountered In The Construction, Immediately Informing About Deviations From The Contract Concluded By The Ordering Party With The Contractor. D. Participating In All Tests, Measurements And Checks. E. Participation In Meetings Organized By The Ordering Party On Matters Relating To The Implementation Of The Construction In Question. F. Participation In The Partial And Final Investment Acceptance Committees And Guarantee Acceptance Committees. G. The Inspector Should Supervise The Construction (Construction Works) At Such Intervals To Ensure The Effectiveness Of Supervision, And Depending On The Needs Of The Contractor And Investor - At Least Once A Week. H. Ordering The Contractor To Perform Additional Tests Of Materials Or Works That Raise Doubts As To Their Quality. I. Request That The Contractor Uncover, At His Own Expense, Any Questionable Elements Of The Work In Order To Check The Quality Of Their Execution, If The Execution Of These Works Was Not Reported For Verification Before Covering Them.J. Suspend The Works If It Is Necessary For Their Proper Execution And If The Contractor Does Not Fulfill His Obligations With Due Diligence And Technical Knowledge. 11. The Ordering Party Requires That The Implementation Of The Order Include The Participation Of Students/Graduates And Young Conservators Of Monuments As Part Of An Internship Or Student Practice.13. The Ordering Party Does Not Require The Employment Of Persons Performing The Activities Of A Supervision Inspector Under An Employment Contract - An Independent Technical Function In The Construction Industry.
