In Order To Prepare An Offer, The Contractor Should Prepare A Bill Of Quantities Based On The Description. Power Supply To The Area Where The Works Will Be Carried Out In Electricity And Water, As Well As Waste And Sewage Collection, The Contractor Will Be Obliged To Perform At His Own Expense And With His Own Efforts. If Using The Ordering Partys Connections, The Contractor Is Obliged To Install Its Own Meters At Each Water And Electricity Consumption Point Before Starting The Works, Document Their Readings And Submit Them To The Ordering Party. The Contractor Is Obliged To Secure Demolition Materials In Such A Way That They Can Be Reused. This Applies In Particular To Curbs, Road Blocks, Tiles And Road Markings. The Contractor Is Obliged To Report The Resulting Material, Taking Into Account Its Type And Quantity. Before Disposing Of Any Demolition Material, Please Ask The Ordering Party Whether It Is Needed For Reuse. The Contractor Will Provide Construction Services: A Construction Manager In The Road Industry. All Costs Related To Carrying Out The Works, Making Arrangements For The Duration Of The Works, Occupying The Area For The Duration Of The Works Is Covered By The Contractor.4. The Ordering Party Allows The Execution Of Part Of The Order To Be Entrusted To A Subcontractor. The Ordering Party Does Not Reserve The Obligation For The Contractor To Perform Key Tasks.5. The Ordering Party Requests That The Contractor Indicate In The Offer The Parts Of The Order Whose Performance Will Be Entrusted To Subcontractors, And Provide The Names Of Any Subcontractors, If They Are Already Known.6. The Ordering Party Requires The Contractor Or Subcontractor To Employ Persons Under An Employment Contract To Perform The Activities Indicated Below During The Execution Of The Order:• Preparatory Works, Including Demolition;• Surface Works Related To Laying The Surface;• Finishing Works7. During The Execution Of The Order, The Ordering Party Is Entitled To Carry Out Inspection Activities Towards The Contractor Regarding The Contractors Or Subcontractors Compliance With The Requirement Of Employing Persons Performing The Activities Indicated In Point 6 Under An Employment Contract. The Ordering Party Is Entitled In Particular To: A) Request Declarations And Documents Confirming Compliance With The Above-Mentioned Requirements. Requirements And Their Assessment, B) Request Explanations In Case Of Doubts Regarding Confirmation Of Compliance With The Above-Mentioned Requirements. Requirements, C) Carrying Out On-Site Inspections.8. During The Execution Of The Order, At Each Request Of The Ordering Party And Within The Time Specified In The Request, The Contractor Will Submit To The Ordering Party The Evidence Indicated Below In Order To Confirm The Fulfillment Of The Requirement Of Employment Under The Employment Contract By The Contractor Or Subcontractor Of Persons Performing The Activities Indicated In Point 6 During The Execution Of The Order: • Declaration Of The Contractor Or Subcontractor On The Employment Under An Employment Contract Of Persons Performing The Activities Referred To In The Ordering Partys Request. This Declaration Should Include In Particular: Precise Identification Of The Entity Submitting The Declaration, Date Of Submission Of The Declaration, Indication That The Activities Covered By The Request Are Performed By Persons Employed Under An Employment Contract, Together With An Indication Of The Number Of These Persons, Their Names And Surnames, The Type Of Employment Contract And The Salary. Declaration Of The Contractor Or Subcontractor (Together With A Document Regulating The Scope Of Obligations, If Prepared). A Copy Of The Contract(S) Should Be Anonymized In A Way That Ensures The Protection Of Employees Personal Data, In Accordance With The Provisions Of The Act Of May 10, 2018 On The Protection Of Personal Data (Journal Of Laws Of 2018, Item 1000) (I.E. In Particular Without Addresses, Employees Pesel Numbers). The Employees Name And Surname Are Not Anonymized. Failure By The Contractor To Submit, Within The Deadline Set By The Ordering Party, The Evidence Requested By The Ordering Party To Confirm That The Contractor Or Subcontractor Has Met The Employment Requirement Under The Employment Contract Will Be Treated As The Contractors Or Subcontractors Failure To Meet The Requirement To Employ Persons Performing The Activities Specified In Point 6 Under The Employment Contract.
