1) The Subject Of The Order Is To Provide Medical And Veterinary Care And Medical Services For Animals At The Shelter For Homeless Animals At Ul. Gliwicka 20 In Kędzierzyn-Koźle In Accordance With The Act Of August 27, 1997 On Animal Protection (Journal Of Laws Of 2023, Item 1580). Important Information:• The Shelter Admits Approximately 140 Dogs And 140 Cats Annually. • The Average Number Of Animals Staying In The Shelter Per Month Is: Approximately 75 Dogs And 70 Cats. • The Shelter Has A Room Where Animal Health Checks, Deworming And Flea Removal As Well As Vaccinations And Microchipping Can Be Carried Out. • However, In The Shelter There Is No Room Intended For Performing Complex Medical And Surgical Procedures Requiring Sterile Conditions, Therefore Such Procedures And Hospitalization Will Take Place In The Contractors Medical Facility. 2) The Contractors Duties Will Include Constant Care For Homeless Animals Staying In The Shelter And Performing Treatments To Prevent Animal Reproduction. 3) Performance Of The Subject Of The Order Includes: 1) Carrying Out Services According To The Needs Of Animal Treatment And At The Urgent Request Of The Ordering Party; 2) Protective Vaccination Of Animals; 3) Conservative Treatment; 4) Surgical Treatment; 5) Development Of Nutritional Schedules; 6) Keeping Records Of Sick Animals; 7) Keeping Records Of Used Medicines And Medical Materials; 8) Taking Care Of The Health Condition Of The Animals; 9) Taking Care Of The Sanitary Condition Of The Shelter; 10) Observation Of Dogs Suspected Of Rabies; 11) Performing Activities Provided For By Law In Relation To Animals Staying In The Shelter Suffering From An Infectious Disease; 12) Issuing Passports Necessary For Adoptions Abroad; 13) Compliance With Occupational Health And Safety And Fire Protection Regulations Applicable In The Shelter; 14) Maintaining Documentation Of Statutory Provisions In The Field Of Veterinary Medicine; 15) Cooperation With The Management And Employees Of The Shelter; 16) Visit The Shelter Three Times A Week For 2 Hours; 4) Contractors Availability: In The Scope Of Providing Veterinary Care For Animals At The Shelter And Providing Services By Visiting, Analyzing And Performing Activities At The Shelter Three Times A Week For 2 Hours. On Other Days (Saturdays, Sundays, Public Holidays) And Outside The Shelters Opening Hours On Working Days, The Contractor Will Be On Veterinary Duty - By Telephone, During Which Emergency Cases Will Be Handled That Require Immediate Assistance Due To A Life-Threatening Or Life-Threatening Condition. Protective Vaccination Against Viral Diseases Of Cats -9019. Euthanasia Of An Animal Weighing Up To 10 Kg -1020. Euthanasia Of An Animal Weighing From 10 Kg To 20 Kg -321. Euthanasia Of An Animal Weighing More Than 20 Kg -222. Putting Down Blind Litters - 123. Lump Sum Remuneration - 12 Months24 Medicines And Medical Materials (According To Consumption) -12 Months25. Urgent Veterinary Intervention On Call - 64) Tests That Will Be Necessary To Perform In Order To Ensure Proper Medical Care For Animals Staying In The Shelter, Listed In Point 5. From 1 To 22, The Contractor Is Obliged To Provide A Lump Sum Price, Which Should Also Include The Cost Of Medicines And Medical Materials Necessary For The Implementation Of The Contract. Purchased By The Contractor After Consultation With The Ordering Party And Settled Monthly On An Invoice, Divided Into Medicines And Medical Materials. 6) The Ordering Party Informs That The Estimated Cost Of Purchasing Medicines And Medical Materials (Taking Into Account Expenses From Previous Years) Is Approximately Pln 78,700.00 Net. The Amount Should Be Included In The Price Form (Fc).7) If It Is Necessary To Perform Treatments Not Included In The Offer Form, The Basis For Settlements Between The Ordering Party And The Contractor Will Be The Prices Included In The Applicable Price List Of Services Provided By The Contractor. After Selecting Its Offer As The Most Advantageous, The Contractor Is Obliged To Provide The Applicable Price List Of Services No Later Than On The Date Of Conclusion Of The Contract. Act On Animal Medical Facilities (Journal Of Laws Of 2019, Item 24).9) The Contractor Is Obliged To Have At Its Disposal An Animal Medical Facility Equipped With The Necessary Equipment And Devices To Provide Veterinary Services, I.E. An Ultrasound Scanner And A Camera, Throughout The Duration Of The Contract. X-Ray And A Place For Keeping Animals During The Convalescence Period, Located In An Area No Further Than 30 Km From The Shelter.
