1. The Service That Is The Subject Of The Order Will Be Provided On The Basis Of: - The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended), - The Act Of August 19, 1994 On The Protection Mental Health (Journal Of Laws Of 2022, Item 2123, As Amended), - Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2005, No. 189, Item 1598, As Amended).2. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders Provided In The City Of Lipno, At The Recipients Place Of Residence. 3. Specialized Care Services For People With Mental Disorders Are Adapted To The Specific Needs Resulting From The Type Of Disease Or Disability. 4. Specialized Care Services Will Be Provided Based On An Administrative Decision, In Accordance With The Scope Established For Each Person Under Care.5. Specialized Care Services Will Be Provided On Weekdays From Monday To Friday From 8.00 To 19.00. A Therapeutic Hour Is 60 Minutes.6. On A Monthly Basis, A Group Of Approximately 48 Patients Is Expected To Receive Specialized Care Services. 7. The Ordering Party Does Not Allow The Possibility Of Submitting Partial Offers Referred To In Art. 7 Point 15 Of The Public Procurement Law. Justification: The Ordering Party Does Not Divide The Order Into Parts In Order To Avoid Communication Disruptions In The Process Of Providing The Service, Thus Increasing The Efficiency And Rationality Of Service Provision By The Ordering Party And The Potential Contractor. Moreover, The Implementation Of The Task By One Contractor Will Enable Achieving Unified Standardization Of Services And Clearly Defined Contractors Responsibility For The Services.8. The Following Types Of Specialized Care Services For People With Mental Disorders Have Been Distinguished: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Training Self-Service And Social Skills As Well As Support, Also In The Form Of Assistance In Everyday Life Activities, In Particular Such As: Self-Service, Especially Performing Household And Cleaning Activities, Including The Ability To Maintain And Run A Home, Care For Hygiene And Appearance, Maintaining Contacts With Household Members, Peers At The Place Of Study And Work And With The Local Community, Organizing And Spending Free Time Together, Using The Services Of Various Institutions, B) Interventions And Help In Family Life, Including: Help In Coping With Crisis Situations - Specialist Counseling, Crisis Interventions , Psychological Support, Therapeutic Conversations, Facilitating Access To Education And Culture, Counseling, Coordinating The Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialized Services Is A Member, Shaping Positive Relationships Of The Supported Person With Close People, Cooperation With The Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: Obtaining Social Benefits, Pensions And Disability Benefits, Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: In Searching For Information About Work , Assistance In Finding Employment Or An Alternative Job, In Particular Participation In Occupational Therapy Workshops, Vocational Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, In Completing Documents Needed For Employment, In Preparing For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including: Learning How To Plan A Budget, Assisting In Incurring Expenses, Assistance In Obtaining Tax Relief, Increasing The Ability To Manage Ones Own Budget And Becoming Independent Financial.2) Care - Supporting The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Supervising The Intake Medications And Observing Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As In Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities3) Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (Journal Of Laws Laws Of 2022, Item 2561 As Amended Amended): A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 4) Housing Assistance, Incl. Including: A) Obtaining An Apartment, Negotiating And Paying Fees, B) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relationships Of The Person Receiving Help With Neighbors And The Host Of The House. 5) Providing Children And Young People With Disorders With Mental Health Access To Rehabilitation And Revalidation-Educational Classes, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of 19 August 1994 On Mental Health Protection (Journal Of Laws Of 2022, Item 2123, As Amended).9. The Ordering Party Expects To Provide 7,668 Hours Of Specialized Care Services During The Contract Implementation Period. The Ordering Party Indicates That Due To The Fact That It Is Unable To Predict The Final Number Of Hours Of Specialist Care Services, It Reserves The Right To Reduce The Number Of Specialist Care Services Commissioned To The Contractor, Provided That The Minimum Number Of Commissioned Services Is 3,834 Hours.10. Before Signing The Contract, The Selected Contractor Will Present A List Of People Providing Services, Specifying Their Education, And Will Document At Least Three Months Of Professional Experience In Working With People With Mental Disorders Acquired In Accordance With The Provisions Of § 3 Of The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Services Caring.
