1. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders, Performed At Their Place Of Residence In The Dąbrowa Tarnowska Commune From January 1, 2024 To December 31, 2024.2. Estimated Number Of Hours And Persons Covered By Specialist Care Services For People With Mental Disorders: 1) During The Order Implementation Period, Specialist Care Services For People With Mental Disorders Will Be Provided For A Total Of Approximately 10 People, 2) Expected Number Of Hours Of Specialist Care Services For People With Mental Disorders During The Period Of Order Execution Is Approximately 2,257, 3) The Number Of Hours Of Service Provision Specified Above Is Their Maximum Number, And The Ordering Party Will Pay For The Services Actually Provided, 4) The Ordering Party Anticipates That The Minimum Number Of Hours Provided During The Term Of The Contract Will Be No Less Than 50% Of The Assumed Number Of Hours, 5) The Ordering Party Reserves The Right To Increase Or Reduce The Total Number Of Hours Of Services Performed - Within The Amount That The Ordering Party Intends To Allocate To Finance The Order, Therefore The Given Quantity Statement Is Not Binding On The Ordering Party. However, It Is Binding On The Contractor In Terms Of The Amount Of The Unit Price. The Ordering Party Allows For The Possibility Of Increasing The Total Number Of Hours, Provided That Their Number And Value Of Remuneration Do Not Exceed 50% Of The Number And Value Of Remuneration Of The Basic Contract. Increasing The Number Of Hours And The Value Of The Order Requires The Conclusion Of An Annex To This Contract.3. The Number Of Hours And People Covered By The Services May Change And Cannot Be Determined Precisely Due To The Specific Nature Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition.4. The Care Service Is Provided At The Place Of Stay Of The Person In Need Of Support.5. Duration Of Services: 1 Hour Of Service Equals 1 Clock Hour, I.E. 60 Minutes. The Concept Of The Number Of Hours Of Services Provided Should Be Understood Only As The Actual Time Of Providing Services Without Preparatory Activities, I.E. Without The Time Needed To Arrive Or Arrive.6. Specialized Care Services For People With Mental Disorders Will Be Provided By The Contractor On All Days Of The Week, I.E. On Working Days, Saturdays, Sundays And Holidays From 8:00 A.M. To 6:00 P.M. Or At Other Times Depending On The Clients Needs. The Hours And Method Of Providing Services Will Be Determined And Adapted To The Individual Needs Of People Under Care, Based On A Decision Issued By The Ordering Party. After Individual Arrangements, The Contractor Will Develop A Schedule For The Provision Of Services.7. The Contractor Undertakes To Provide Specialized Care Services For People With Mental Disorders In Accordance With § 2 Of The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws Of 2005, No. 189, Item 1598, As Amended). D.). Providing Specialized Care Services To People With Mental Disorders Includes: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Self-Service Skills Training And Social Skills, As Well As Support, Also In The Form Of Assistance In Everyday Life Activities, In Particular Such As: – Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, – Care For Hygiene And Appearance, – Maintaining Contacts With Household Members , Peers, At The Place Of Study And Work And With The Local Community, – Organizing And Spending Free Time Together, – Using The Services Of Various Institutions, B) Interventions And Help In Family Life, Including: – Help In Coping With Crisis Situations - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access To Education And Culture, - Counseling, Coordination Of Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, - Shaping Positive Relationships Of The Supported Person With Other People Relatives, – Cooperation With The Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: – Obtaining Social, Pension And Disability Benefits, – Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - In Searching For Information About Work, Assistance In Finding Employment Or Alternative Work, In Particular Participation In Occupational Therapy Workshops, Professional Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, - In Completing Documents Needed For Employment, - In Preparation For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, - In Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including: - Learning How To Plan A Budget, Assisting With Incurring Expenses, - Assistance In Obtaining Fee Reductions, - Increasing The Ability To Manage Ones Own Budget And Financial Independence; 2) Care - As Support For The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits , Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medications At The Pharmacy, D) Monitoring The Intake Of Medications And Observing Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Auxiliaries And Medical Materials, Orthopedic Items, And Also In Maintaining Hygiene, F) Assistance In Getting To Health Care Facilities, G) Assistance In Getting To Rehabilitation Facilities; 3) Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of 27 August 2004 On Financed Health Care Services From Public Funds (Journal Of Laws U. No. 210, Item 2135, As Amended. Amended): A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 4) Housing Assistance, Incl. Including: A) Obtaining An Apartment, Negotiating And Paying Fees, B) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relationships Of The Person Receiving Help With Neighbors And The Host Of The House; 5) Providing Children And Young People With Disorders Mental Health Access To Rehabilitation And Revalidation-Educational Classes, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection (Journal Of Laws No. 111, Item 535, As Amended).
