4.1. The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders And Disabilities By Persons With Qualifications To Practice The Profession, I.E. Psychologist, Educator, Speech Therapist, Occupational Therapist, Psychiatric Nurse, Specialist In Medical Rehabilitation, Physiotherapist For People With Mental Disorders, Performed In Place Of Residence In The Municipality Of Kowal From The Date Of Signing The Contract, I.E. From January 1 To December 31, 2024.4.2. Expected Number Of Hours And People Covered By Care: Care Services Will Be Provided To At Least 7 People Under Care Per Month. The Number Of Expected Hours Is 1,800 Per Year From The Date Of Signing The Contract, I.E. From January 1, 2024 To December 31, 2024. The Specified Number Of Hours Of Providing Services Constitutes Their Maximum Quantity, And The Ordering Party Will Pay For Those Actually Completed. The Minimum Number Of Hours Is 1,300. The Number Of Hours And People Covered By The Services May Change And Cannot Be Determined Precisely Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition. The Care Service Is Provided At The Persons Place Of Stay. In Need Of Support. Duration Of Services: 1 Hour Of Service Equals 1 Clock Hour, I.E. 60 Minutes. The Number Of Hours Of Services Provided Should Be Understood Only As The Actual Time Of Providing Services Without Preparatory Activities, I.E. Without The Time Needed To Get There Or Travel. Specialist Care Services For People With Mental Disorders Will Be Provided By The Contractors Employees On All Days Of The Week, I.E. On Working Days, Saturdays. From 7:00 A.M. To 8:00 P.M. Or At Another Time Depending On The Clients Needs. The Hours And Method Of Providing Services Will Be Determined And Adapted To The Individual Needs Of People Under Care, Based On A Decision Issued By The Ordering Party. After Individual Arrangements, The Contractor Will Develop A Schedule For The Provision Of Services.4.4 The Contractor Undertakes To Provide Specialized Care Services For People With Mental Disorders In Accordance With §2 Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services (Journal Of Laws No. 189 , Item 1598, As Amended).1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning, Motivating For Activity, Treatment And Rehabilitation, Conducting Skills Training Self-Service And Social Skills, As Well As Support, Also In The Form Of Assistance In Everyday Life Activities, In Particular Such As: - Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, - Care For Hygiene And Appearance, - Maintaining Contacts With Household Members, Peers, At The Place Of Study And Work And With The Local Community, - Organizing And Spending Free Time Together, - Using The Services Of Various Institutions, B) Interventions And Help In Family Life, Including: - Help In Coping With Crisis Situations – Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access To Education And Culture, - Counseling, Coordination Of Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, - Shaping Positive Relationships Of The Supported Person With Close People, - Cooperation With The Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: - Obtaining Social Benefits, Retirement And Disability Benefits, - Filling Out Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - In Searching For Information About Work, Assistance In Finding Employment Or Alternative Work, In Particular Participation In Occupational Therapy Workshops, Professional Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, - In Completing Documents Needed For Employment, - Preparing For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, - Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Money Management, Including: - Learning How To Plan A Budget, Assisting When Incurring Expenses, - Assistance In Obtaining Fee Reductions, - Increasing The Ability To Manage Ones Own Budget And Financial Independence; 2) Care - As Support For The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On Appointments Medical Examinations, Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At The Pharmacy, D) Monitoring The Intake Of Medicines And Monitoring Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Auxiliaries And Medical Materials, Orthopedic Items, As Well As In Maintaining Hygiene, F) Assistance In Getting To Health Care Facilities, G) Assistance In Getting To Rehabilitation Facilities; 3) Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (I.E. Journal Laws Of 2021, Item 1285, As Amended), A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 4) Housing Assistance, Incl. Including: A) Obtaining An Apartment, Negotiating And Paying Fees, B) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relationships Of The Person Receiving Help With Neighbors And The Host Of The House. 5) Providing Children And Young People With Disorders With Mental Health Access To Rehabilitation And Revalidation-Educational Classes, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection (Consolidated Text: Journal Of Laws Of 2020, Item 685, As Amended). 4.5 Additional Requirements: 1) Persons Who Will Participate In The Execution Of The Order Must Not Have A Criminal Record, They Must Be Physically And Intellectually Fit, Available, Cultured, And Must Have The Ability To Maintain Proper Interpersonal Contacts. In Addition, They Will Be Obliged To Comply With The Following Principles: • Maintain Professional Secrecy Regarding All Information They Obtain While Performing Their Duties, And In Particular Not To Disclose To Third Parties Personal Data Of Beneficiaries, Their Personal, Family, Health And Economic Situation, • Not To Enter The Beneficiarys Home Unauthorized Persons And Their Own Pets, • During Their Stay In The Apartment, Beneficiaries Cannot Smoke, Use Drugs Or Consume Alcohol, • They Cannot Burden The Beneficiaries With Their Problems, They Must Use Polite Phrases In Contact With The Beneficiary And Respect The Will Of The Beneficiary.2) Duration Of The Service Services, Means The Time Of Performing The Ordered Scope Of Services. The Ordering Party Stipulates That The Time Of Providing Services Does Not Include The Time Of Travel Or Arrival To The Place Of Provision Of Services - 1 Hour Means 60 Minutes;3) The Contractor Is Liable For The Processing Of Personal Data Resulting From Legal Provisions, In Particular The Personal Data Protection Act - Necessary Requirements To Protect The Data In Question, To Which It Will Gain Access In Connection With The Implementation Of The Entrusted Tasks, Against Unauthorized Access, Unjustified Modification Or Destruction, Illegal Disclosure Or Acquisition, To The Extent Appropriate To The Obligations Related To The Processing Of The Data In Question. He Is Also Obliged To Repair Damage Due To Violation Of The Rules For The Processing Of Information And Personal Data, If Any;4) The Contractor Will Bear The Costs Related To The Performance Of The Subject Of The Order, I.E. He Will Bear The Costs Of Telephone Calls, The Cost Of Preventive Examinations And Personal Accident Insurance;/If Applicable5 ) The Contractor Cannot Charge Any Additional Fees From The Persons To Whom He Will Provide Services;6) The Contractor Cannot Entrust The Performance Of Services To A Third Party Without The Written Consent Of The Ordering Party;7) The Contractor Undertakes To Enable Direct Control Of The Service Implementation At The Request Of An Authorized Institution And The Ordering Party.8 ) The Contractor Must Guarantee Constant, 24-Hour Direct Contact (Only Telephone Contact Is Not Allowed) With The Person Acting As A Coordinator Authorized To Make Decisions Or Directly With The Contractor. The Contractor Will Ensure Direct Personal Contact Within No More Than Two Hours From A Telephone Or E-Mail Request.9) Specialized Care Services For The Charges Will Be Provided By The Contractor, In Accordance With The Issued Decisions Of The Head Of Mops In Kowal Specifying The Rules For Granting Them, Scope, Form, Period And Place Of Provision. The Administrative Decision Granting Care Services Will Contain The Following Data: A) Name And Surname And Residential Address Of The Client, B) Hourly Number Of Services, C) Scope Of Services Provided, D) Duration Of Services, E) Payment Incurred By The Client For One Hour Of Service.10) The Contractor Undertakes To Perform The Activities With Due Diligence And Ensure Their Correct Implementation Only By Persons Acting As Guardians Who Have Experience
