Health And Social Work Services - Specialized Care Services For People With Mental Disorders In Their Place Of Living. 1. The Subject Of The Order Is The Provision Of Specialized Care Services In Place Of Residence For People With Mental Disorders, Pursuant To: A) Art. 8 And 9 Of The Act Of 19 August 1994 On Mental Health Protection B) Art. 18.1 Point 3 Of The Act Of 12 March 2004 On Social Assistance And C) Regulation Of The Minister Social Policy Of September 22, 2005 On Specialist Care Services (Journal Of Laws No. 189, Item 1598, As Amended) 2. The Estimated Average Number Of Service Recipients Is About 81 People. 2.1. The Estimated Number Of Service Hours Over The Contract Period Is Approx. 1,500 Hours/Month. 2.2. Services Will Be Provided 7 Days A Week Hours From 8:00 A.M. To 8:00 P.M., Depending On The Needs Of The Clients; 2.3. Time The Service Is 60 Minutes Of Actual Performance For The Client And Does Not Include Time Necessary To Travel Or Reach The Place Of Residence Of The Client; 2.4. The Ordering Party Determines The Scope Of Services Provided To The Client And Their Quantity, As Well As Provides The Contractor With Information About Changes To The Above Data. 2.5. Number Hours And People Covered By Services, Are Subject To Change And Cannot Be Determined Precisely Due To The Specifications Of The Order, Which Is Characterized By Variability Of Customer Needs Attention To Their Changing Health Condition. 2.6. The Ordering Party Stipulates That It Will Not Be Bear No Responsibility If Only Part Of The Above-Mentioned Hours Of The Subject Of The Order Will Be Implemented Depending On The Amount Of Funds Available In The Budget Year And Depending On The Needs Arising From Changes In The Circumstances Giving Rise To The Grant Benefits. 3. Required Service Completion Date - 6 Months From The Date Of Signing The Contract. 4. Scope Of Specialist Services For People With Mental Disorders: 1) Teaching And Developing Skills Necessary For Independent Living. In Particular: A) Developing The Ability To Meet Basic Life Needs And Skills Social Functioning, Motivating For Activity, Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Support, Also In The Form Of Assisting In Everyday Life Activities, In Particular These Such As: Self-Service, Especially Performing Economic And Cleaning Activities, Incl Including: Housekeeping And Housekeeping Skills, Attention To Hygiene And Appearance, Maintenance Contacts With Family Members, Peers, Places Of Study And Work, And The Community Local, Organizing And Spending Free Time Together, Using Various Services Institution. B) Interventions And Help In Family Life, Including: Help In Coping In Crisis Situations - Specialist Counseling, Crisis Interventions, Support Psychological, Therapeutic Conversations, Facilitating Access To Education And Culture, Counseling, Coordination Of Activities Of Other Services For The Family Of Which The Recipient Is A Member Assistance In The Form Of Specialized Services, Shaping Positive Relationships Of The Supported Person With Close People, Cooperation With Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: In Obtaining Social, Pension And Disability Benefits, In Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: Searching For Information About Work, Assistance In Finding Employment Or Alternative Employment, In Particular Participation In Occupational Therapy Workshops, Professional Activity Centers, Community Self-Help Homes, Social Integration Centers And Clubs, Clubs Work, In Completing Documents Needed For Employment, In Preparation For Conversations With The Employer, Supporting And Assisting In Contacts With The Employer, In Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing The Household Money, Including: Learning How To Plan A Budget, Assisting With Expenses, Assistance In Obtaining Tax Relief, Increasing Self-Management Skills Budget And Financial Independence; 2) Care - As Supporting The Process Treatment, Including: A) Assistance In Access To Health Services, B) Coordination And Supervision Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At The Pharmacy, D) Monitoring The Intake Of Medicines And Observing Possible Effects Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities, H) Access To A Sensory Integration Room In The City Of Nysa, I) Developing Communication. 3) Physical Rehabilitation And Improvement Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27 2004 On Health Care Services Financed From Public Funds (Journal Of Laws No. 210, Item 2135, As Amended): In Accordance With The Recommendations Of A Doctor Or Specialist Scope Of Physical Rehabilitation Or Physiotherapy, Cooperation With Specialists In The Field Psychological, Pedagogical And Educational-Therapeutic Support Aimed At For Multilateral Activation Of A Person Using Specialized Services; 4) Help Housing, Including: A) Obtaining An Apartment, Negotiating And Paying Fees, B) In Organizing Minor Renovations, Adaptations, Repairs, Removal Of Architectural Barriers, Shaping Appropriate Relationships Between The Person Receiving Help, Neighbors And The Host Home; 5) Ensuring Access To Classes For Children And Adolescents With Mental Disorders Rehabilitation And Revalidation And Educational Activities, In Exceptional Cases, If Are Not Able To Gain Access To The Classes Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection.
