The Subject Of The Order Is The Provision Of Services Consisting In Providing Specialist Care Services At Home For People With Mental Disorders (Including Autism) Who Are Beneficiaries Of Mops In Kwidzyn For 2024 - Hereinafter Referred To As Suo.1. Specialized Care Services Will Be Provided At The Place Of Residence Of People With Mental Disorders Supported By Mops In Kwidzyn, Hereinafter Referred To As Beneficiaries.2. The Average Monthly Estimated Number Of Beneficiaries Will Be 35 People. Suo May Be Provided On Working Days And Public Holidays In Accordance With Issued Administrative Decisions.3. The Estimated Annual Number Of Hours Will Be 5,000. Due To The Specific Nature Of The Subject Of The Order, The Actual Number Of People To Whom Suo Will Be Provided And The Actual Number Of Hours Of Service Provision Per Month Will Depend On The Actual Number Of People Who Require Assistance In The Form Of Suo (The Number Of People And Hours May Be Reduced Or Increase).The Ordering Party Reserves The Right Not To Exhaust The Entire Scope Of The Subject Of The Order. 4. When Providing Services, The Following Provisions Shall Apply In Particular: 1) Art. 50 Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended),2) Regulation Of The Minister Of Social Policy On Specialized Care Services Of September 22, 2005 (Journal Of Laws Laws Of 2005, No. 189, Item 1598, As Amended),3) Regulation Of The Minister Of Health Of February 28, 2017 On The Type And Scope Of Preventive, Diagnostic, Therapeutic And Rehabilitation Services Provided By A Nurse Or Midwife Independently Without A Doctors Order (Journal Of Laws Of 2017, Item 497)4.1. In Accordance With The Regulation Of The Minister Of Social Policy On Specialized Care Services, The Following Types Of Specialized Services Are Established, Adapted To The Specific Needs Of People Requiring Assistance In The Form Of Specialized Services Resulting From The Type Of Their Disease Or Disability, Provided By People With Specialized Professional Preparation: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating People To Be Active, Undergoing Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Providing Support, Also In The Form Of Assisting In Everyday Life Activities, In Particular, Such As: – Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, – Care For Hygiene And Appearance, – Maintaining Contacts With Household Members, Peers, At The Place Of Study And Work And With The Local Community, – Shared Organizing And Spending Free Time, - Using The Services Of Various Institutions, B) Interventions And Assistance In Family Life, Including: - Help In Coping With Crisis Situations - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access For Education And Culture, – Consulting, Coordination Of Activities Of Other Services For The Benefit Of The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, – Shaping Positive Relationships Of The Supported Person With Close People, – Cooperation With The Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person ,C) Assistance In Dealing With Official Matters, Including: - Obtaining Social Benefits, Pension And Disability Benefits, - Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - Searching For Information About Work, Assistance In Finding Employment Or An Alternative Job, In Particular Participation In Occupational Therapy Workshops, Vocational Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, - In Completing The Documents Needed For Employment, - In Preparing For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer, - Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including: - Learning How To Plan A Budget, Assisting In Incurring Expenses, - Assistance In Obtaining Tax Relief, - Increasing Management Skills Own Budget And Financial Independence;2) Care - As Support For The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Supervising The Intake Of Medications And Monitoring Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities; 3) Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (Journal Of Laws U. No. 210, Item 2135, As Amended): A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 4) Assistance Housing, Including: A) Obtaining An Apartment, Negotiating And Paying Fees, B) Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relationships Of The Person Receiving Help With Neighbors And The Owner Of The House; 5) Providing Children And Young People With Mental Disorders Have Access To Rehabilitation And Revalidation-Educational Classes, In Exceptional Cases, If They Are Unable To Obtain Access To The Classes Referred To In Art. 7 Of The Act Of 19 August 1994 On Mental Health Protection (Journal Of Laws No. 111, Item 535, As Amended).5. To Perform Activities In The Field Of Suo, The Contractor Will Assign Persons With Qualifications To Perform The Following Professions: Social Worker, Psychologist, Educator, Speech Therapist, Occupational Therapist, Nurse, Assistant To A Disabled Person, Community Caregiver, Specialist In Medical Rehabilitation, Physiotherapist Or Other Profession Providing Knowledge And Skills Enabling The Provision Of Specific Specialist Services. Persons Providing Specialist Services For People With Mental Disorders Must Have At Least 3 Months Of Professional Experience In Working With People With Mental Disorders, Gained * In One Of The Following Units: 1) A Psychiatric Hospital; 2) An Organizational Unit Of Social Assistance For People With Mental Disorders; 3 ) A Therapy Facility Or Educational Facility Attended By Children With Developmental Disorders Or Mental Disabilities; 4) A Therapeutic, Educational And Upbringing Center; 5) An Occupational Therapy Workshop; 6) A Unit Other Than Those Mentioned In Points 1-5, Providing Specialized Care Services For People With Mental Disorders. Note*: By The Term Professional Experience, The Ordering Party Understands The Skills To Perform Specialized Care Services Acquired And Consolidated During Their Actual Performance In Various Legal Forms: Employment Contract, Contract For Specific Work, Contract Of Mandate, Volunteering And Personal Experience As A Caregiver Actual.6. Persons Providing Suo Referred To In § 2 Point 1 Lit. A Of The Regulation Of The Minister Of Social Policy Of September 22, 2005 On Specialized Care Services, They Must Have Training And Experience In The Field Of: 1) The Ability To Develop Motivation For Behavior Accepted By The Environment; 2) Shaping Habits Of Purposeful Activity; 3) Conducting Social Behavior Training. 7. Suo Will Be Available Every Day From 7:00 A.M. To 8:00 P.M.8. The Contractor Ensures: Supervision And Organization Of Specialized Care Services For People With Mental Disorders, Setting The Work Schedule For Service Providers.9. As Part Of The Implementation Of The Subject Of The Order, The Contractor Will Be Obliged To Secure Materials Necessary To Implement The Individual Scope Of The Suo.
