The Subject Of The Order Is The Provision Of Care Services To Clients Of The Social Welfare Center In Suchy Las, Which Fall Within The Scope Of Social Assistance Benefits Specified In Art. 50 Of The Act Of March 12, 2004 On Social Assistance (Consolidated Text: Journal Of Laws Of 2023, Item 901) Estimated Number Of Hours: Care Services (Article 50(3) Of The Act On Social Assistance): 7000I. The Scope Of Activities Constituting Care Services Depends On The Health Condition Of The Entitled Person And His Or Her Family Situation And Includes In Particular: 1. Satisfying Basic Life Needs Assistance In Making Purchases - Food, Medicines, Cleaning Products, Newspapers, Etc. (Only For Service Recipients) Near The Place Of Residence And Delivering Them To The Patient  Preparing Meals (Only For Service Recipients) In Accordance With The Principles Of Dietetics And The Appropriate Disease Entity (E.G. Diabetes - Anti-Diabetic Diet), And If Necessary, Pureeing Or Mixing Food (Recommendations From The Doctor Should Be Prepared As To What The Patient Can And Should Not Eat)  Preparing Products Or Meals For The Part Of The Day When The Patient Will Be Left Alone (Afternoon Snack, Dinner) And Storing Them Properly,  In The Case Of Limited Mobility Of The Ward, Feeding And Watering,  Assistance In Paying Fees And Charges,  Keeping Records Of Expenses And Settling Accounts With The Ward For The Collected Funds2. Hygienic Care A) Activities Related To Maintaining The Personal Hygiene Of The Patient: Maintaining Personal Hygiene (Bathing, Lubricating The Body, Changing Clothes And Putting On Clean Clothes) For People Who Move Independently Or Using Rehabilitation Equipment. Toileting And Caring For A Person Lying In Bed. The Patient (Bath, Oiling The Body, Changing Clothes And Dressing In Clean Clothes) Oral Care Care Of Hands And Feet (Cutting Nails) Only To A Basic Extent (If The Patient Has Neglected Nails, E.G. Fungal Or Sparrowhawk, The Intervention Of A Beautician Is Necessary) Hair Care In Terms Of Washing And Combing (Without Cutting) Changing Diapers, Toilet After Getting Dirty Changing Personal Underwear And Bed Linen Preventing Pressure Sores And Sores (Bedsore Care And Other Medical Procedures Should Be Performed By Community Nurses) B) Activities Related To Maintaining Environmental Hygiene Airing Rooms  Wiping Dust In The Service Recipients Room,  Washing The Floor And Window In The Service Recipients Room,  Washing Dishes After The Service Recipients Meal,  Lighting The Stove And Bringing 1 Bucket Of Coal A Day In The Case Of Single People.3. The Contractor Undertakes To Perform Other Activities Not Covered By The Above Scope And Resulting From The Need To Ensure The Proper Functioning Of The Patient In The Environment. Care Services Will Be Provided In The Suchy Las Commune To Clients Authorized To Use This Form Of Services, Granted On The Basis Of Administrative Decisions Delivered To The Contractor. The Administrative Decision Contains The Personal Data And Address Of The Beneficiary, The Type Of Assistance Granted In The Form Of Services, The Number Of Hours Of Services Granted Along With The Number Of Days And The Period Of Service Provision. The Ordering Party Assumes That 1 Hour Of Each Type Of Services Equals 1 Clock Hour, I.E. 60 Minutes. In Exceptional Cases, It Is Possible To Place An Order For The Provision Of A Service Orally (By Telephone) - With Immediate Confirmation Of This Fact In The Form Of An Administrative Decision. The Contractor Is Obliged To Immediately Inform The Ordering Party About The Suspension Or Discontinuation Of The Customers Service Due To Death, Hospitalization Or Other Reason. The Ordering Party Reserves The Right To Request, In Justified Cases, A Change Of Caregiver For A Given Client.
