The Subject Of The Order Includes The Following Services: 3.1. The Provision Of Specialized Care Services For Disabled People Covered By The Assistance Of Gops Morzeszczyn Will Be Provided In The Following Amounts: - Estimated Number Of Hours Throughout The Entire Contract Implementation Period - 2,400. - The Number Of Hours Given Above Is An Estimated Number Based On Data From 2022, Calculated On The Basis Of Specialized Care Services Provided To The Ordering Partys Clients And Is Intended To Enable Comparison Of Offers Submitted By Contractors. - Therapeutic Hour Is 60 Minutes. - The Individual Scope Of Services, The Number Of Hours, Time Of Day And Period Of Provision Of Specialist Care Services Will Result From The Arrangements Made Between The Social Worker, The Client And The Contractor. The Ordering Party Reserves The Right To Order Additional Activities Not Listed Above, Depending On The Specific Needs Of The Client. - The Number Of Hours And People Covered By The Services May Change And Cannot Be Determined Precisely Due To The Specificity Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition. The Ordering Party Reserves That It Will Not Bear Any Responsibility If Only Part The Above-Mentioned Hours Of The Subject Of The Order Will Be Implemented Depending On The Amount Of Funds Available In The Budget Year And Depending On The Needs Resulting From Changes In Circumstances Resulting In The Granting Of The Benefit. Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating Activity, Treatment And Rehabilitation, Conducting Self-Service And Social Skills Training And Supporting, Also In The Form Of Assisting In Everyday Life Life Activities, In Particular Such As: - Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, - Care For Hygiene And Appearance, - Maintaining Contacts With Household Members, Peers, At The Place Of Study And Work And With The Local Community , - Organizing And Spending Free Time Together, - Using The Services Of Various Institutions, B) Interventions And Assistance In Family Life, Including: - Help In Coping With Crisis Situations - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access To Education And Culture, - Consulting, Coordinating The Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialist Services Is A Member, - Shaping Positive Relationships Of The Supported Person With Close People, - Cooperation With The Family - Shaping Appropriate Attitudes Towards The Person Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including: - Obtaining Social Benefits, Pension And Disability Benefits, - Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular: - In Searching For Information About Work , Assistance In Finding Employment Or An Alternative Job, In Particular Participation In Occupational Therapy Workshops, Vocational Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs, - In Completing The Documents Needed For Employment, - In Preparing For An Interview With The Employer , Supporting And Assisting In Contacts With The Employer, - In Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including: - Learning How To Plan A Budget, Assisting In Incurring Expenses, - Assistance In Obtaining Tax Relief, - Increasing The Ability To Manage Ones Own Budget And Becoming Financially Independent; 3.2.2. Care - As Supporting The Treatment Process, Including: A) Assistance In Access To Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Supervising The Intake Of Medicines And Observing Any Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In Using Medical Aids And Materials, Orthopedic Items, As Well As In Maintaining Hygiene, F) Assistance In Getting To Health Care Facilities, G) Assistance In Getting To Rehabilitation Facilities ; 3.2.3. Physical Rehabilitation And Improvement Of Disturbed Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004. On Health Care Services Financed From Public Funds A) In Accordance With The Recommendations Of A Doctor Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical, Educational And Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 3.2.4. Housing Assistance, Including: A) In Obtaining An Apartment, Negotiating And Paying Fees, B) In Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Appropriate Relationships Of The Person Receiving Help With Neighbors And The Owner Of The House; 3.2.5. Providing Children And Adolescents With Mental Disorders With Access To Rehabilitation, Revalidation And Educational Activities, In Exceptional Cases If They Do Not Have Them. The Possibility Of Gaining Access To The Classes Referred To In Art. 7 Of The Act Of 19 August 1994. On Mental Health Protection (I.E. 2022, Journal Of Laws, Item 2123) 3.3. Maintaining A Card For Providing Specialist Care Services To Record Hours Worked In Accordance With The Contract.3.4. Qualifications Of A Person Providing Specialist Care Services, Which Must Be Met In Total: 3.4.1. Qualifications To Perform One Of The Following Professions: Social Worker, Psychologist, Educator, Speech Therapist, Occupational Therapist, Nurse, Assistant To A Disabled Person, Community Caregiver, Medical Rehabilitation Specialist, Physiotherapist. 3.4.2. Having At Least Half A Year Of Documented Experience In One Of The Following Facilities: - A Psychiatric Hospital, - An Organizational Unit Of Social Assistance For People With Mental Disorders, - A Therapy Facility Or An Educational Facility Attended By Children With Developmental Disorders Or Mental Disabilities, - A Therapeutic And Educational Center - Educational Institution, - Rehabilitation Facility, - Unit Other Than Those Mentioned Above, Providing Specialist Care Services For Mental Disorders. Moreover, The Demonstrated Skills Should Clearly Indicate The Possibilities Of Performing The Services Specified In The Subject Of The Contract Provided To Individual Persons - Depending On The Needs Of A Given Person And Medical Recommendations. You Must Submit Documents (Such As A Diploma, Certificate, Etc.) Confirming Your Qualifications To Conduct The Above-Mentioned Activities. Specialized Therapies By Persons Intended To Conduct Them.
