The Subject Of The Order Is The Provision Of Specialized Care Services For People With Mental Disorders Provided At The Place Of Residence Of Mgops Residents In The Wieleń Commune From January 2, 2024. Until December 31, 2024 1.1 Expected Number Of Hours And People Cared For. 1) Specialized Care Services For People With Mental Disorders: A) Approximately 3,500 Hours Per Year, B) The Expected Number Of People Covered By The Services: Approximately 6 People Per Month. Due To The Specificity Of The Subject Of The Order, The Ordering Party Provides For The Possibility Of Increasing Or Decreasing The Number Of Hours And People Covered By Care, Depending On Actual Needs. Duration Of Services: 1 Hour Of Service Equals 1 Clock Hour, I.E. 60 Minutes. The Concept Of The Number Of Hours Of Services Provided Should Be Understood Only As The Actual Time Of Providing Services Without Preparatory Activities, I.E. Without The Time Needed To Arrive Or Arrive. Specialized Care Services For People With Mental Disorders Will Be Provided By The Contractors Employees On All Days Of The Week, I.E. On Working Days, Saturdays, Sundays And Holidays From 7:00 A.M. To 8:00 P.M. Or At Other Times Depending On The Needs Of The Client. The Hours And Method Of Providing Services Will Be Determined And Adapted To The Individual Needs Of People Under Care, Based On A Decision Issued By The Ordering Party. After Individual Arrangements, The Contractor Will Develop A Service Schedule. 1.2 The Scope Of Specialized Care Services For People With Mental Disorders Includes: 1) Teaching And Developing Skills Necessary For Independent Living, In Particular: A) Developing The Ability To Meet Basic Life Needs And Social Functioning Skills, Motivating For Activity, Treatment And Rehabilitation, Conducting Training Self-Service And Social Skills As Well As Support, Also In The Form Of Assistance In Everyday Life Activities, In Particular Such As: - Self-Service, Especially Performing Economic And Cleaning Activities, Including The Ability To Maintain And Run A Home, - Care For Hygiene And Appearance, - Maintaining Contacts With Family Members, Peers, At The Place Of Study And Work And With The Local Community,  Organizing And Spending Free Time Together,  Using The Services Of Various Institutions, B) Interventions And Help In Family Life, Including:  Help In Coping With Situations Crisis - Specialist Counseling, Crisis Interventions, Psychological Support, Therapeutic Conversations, - Facilitating Access To Education And Culture, - Counseling, Coordination Of Activities Of Other Services For The Family Of Which The Person Receiving Help In The Form Of Specialized Services Is A Member, - Shaping Positive Relationships Of The Supported Person With Close People,  Cooperation With Family - Shaping Appropriate Attitudes Towards A Sick Or Disabled Person, C) Assistance In Dealing With Official Matters, Including:  Obtaining Social, Pension And Disability Benefits,  Completing Official Documents, D) Support And Assistance In Obtaining Employment, In Particular:  Searching For Information About Work, Assistance In Finding Employment Or Alternative Work, In Particular Participation In Occupational Therapy Workshops, Professional Activity Centers, Community Self-Help Centers, Social Integration Centers And Clubs, Job Clubs,  In Completing Documents Needed For Employment,  In Preparing For An Interview With The Employer, Supporting And Assisting In Contacts With The Employer,  In Solving Mental Problems Resulting From Work Or Lack Of It, E) Assistance In Managing Money, Including:  Learning How To Plan A Budget, Assistance In Incurring Expenses,  Assistance In Obtaining Tax Relief,  Increasing The Ability To Manage Ones Own Budget And Financial Independence; 2) Care - As Supporting The Treatment Process, Including: A) Assistance In Accessing Health Services, B) Arranging And Keeping An Eye On The Dates Of Medical Visits And Diagnostic Tests, C) Assistance In Purchasing Or Ordering Medicines At A Pharmacy, D) Supervising The Intake Of Medicines And Observing Possible Side Effects Of Their Use, E) In Particularly Justified Cases, Changing Dressings, Assistance In The Use Of Medical Aids And Materials, Orthopedic Items, As Well As In Maintaining Hygiene, F) Assistance In Reaching Health Care Facilities, G) Assistance In Reaching Rehabilitation Facilities; 3) Physical Rehabilitation And Improvement Of Impaired Body Functions To The Extent Not Covered By The Provisions Of The Act Of August 27, 2004 On Health Care Services Financed From Public Funds (Consolidated Text: Journal Of Laws Of 2021, Item 1285, As Amended), A) In Accordance With The Recommendations Doctors Or A Specialist In The Field Of Physical Rehabilitation Or Physiotherapy, B) Cooperation With Specialists In The Field Of Psychological, Pedagogical And Educational-Therapeutic Support Aimed At Multilateral Activation Of The Person Using Specialized Services; 4) Housing Assistance, Including: A) In Obtaining An Apartment, Negotiating And Paying Fees, B) In Organizing Minor Renovations, Adaptations, Repairs, Eliminating Architectural Barriers, C) Shaping Proper Relations Of The Person Receiving Assistance With Neighbors And The Owner Of The House; 5) Ensuring That Children And Adolescents With Mental Disorders Have Access To Rehabilitation, Revalidation And Educational Activities, In Exceptional Cases, If They Are Unable To Obtain Access To The Activities Referred To In Art. 7 Of The Act Of August 19, 1994 On Mental Health Protection (Consolidated Text: Journal Of Laws Of 2020, Item 685, As Amended). 2. Performance Of The Subject Of The Order. 1) The Contractor Must Have Its Registered Office Or Place Of Permanent Contact In The Wieleń Commune Or At Least In The Czarnków-Trzcianecki Poviat, So That There Is A Constant Possibility Of Dealing With All Substantive, Administrative And Other Matters Related To The Provision Of Services. 2) The Contractor Must Guarantee Constant, 24-Hour Direct Contact (Only Telephone Contact Is Not Allowed) With The Person Acting As Coordinator Authorized To Make Decisions Or Directly With The Contractor. The Contractor Will Provide Direct Personal Contact Within No More Than Two Hours Of The Telephone Call. 3) Lack Of Contact With The Contractors Coordinator For A Period Longer Than Two Hours Is Not Allowed. 4) Lack Of Contact With The Coordinator Twice For A Period Longer Than Two Hours May Be The Basis For Withdrawal From The Contract Due To The Sole Fault Of The Contractor. 5) It Is Not Allowed To Maintain Contacts Via A Post Office Box Or Only In Writing Sent By Post Or Only Via E-Mail. In The Absence Of Direct Contact With The Person Referred To In Point. 2) 6) Specialized Care Services For People With Mental Disorders Will Be Provided By The Contractor, In Accordance With The Decisions Issued By The Director Of Mgops In Wieleń Specifying The Rules For Granting Them, The Scope, Form, Period And Place Of The Service. The Administrative Decision Granting Care Services Will Contain The Following Data: A) Name And Surname And Residential Address Of The Client, B) Hourly Number Of Services, C) Scope Of Services Provided, D) Duration Of Services, E) Fee Incurred By The Client For One Hour Of Service. 7) In Justified Cases, The Ordering Party May Require Caregivers To Provide Services At A Time And Scope Individually Tailored To The Needs Of The Clients. 8) The Contractor Undertakes To Perform The Activities With Due Diligence And Ensure Their Correct Implementation Only By Persons Acting As Caregivers Who Have Experience In Caring For The Sick At Home And Qualifications In Accordance With Applicable Law. 9) The Contractor May Not Entrust The Performance Of Services To Other Persons Without The Express Written Consent Of The Ordering Party. 10) The Client Must Be Taken Into Care Within The Time Limit Specified In The Decision. 11) If The Contractor Is Unable To Perform The Ordered Service On A Given Day, He Will Be Obliged To Immediately Notify The Ordering Party By Telephone And Then Confirm It In Writing. 12) The Contractor Undertakes To Inform The Ordering Party About Any Change In The Life And Health Situation Of Persons Under Care That Affects The Provision Of The Service. 13) Persons Acting As A Guardian Cannot Have A Criminal Record, They Must Be Healthy, Physically And Intellectually Fit, Have The Skills To Maintain Proper Interpersonal Contacts And Undertake To Keep Secret All Information Obtained In Connection With The Provision Of The Service. 14) The Contractor Will Be Obliged To Keep Up-To-Date Time Sheets For Each Of Its Employees. Worksheets Of Persons Providing Services Must Be Delivered To The Ordering Party After The End Of Each Month, No Later Than The 5Th Day Of The Following Month. 15) The Contractor Is Obliged To Collect, Store And Process Personal Data Obtained In Connection With The Implementation Of The Contract, In Accordance With The Provisions Of Regulation (Eu) 2016/679 Of The European Parliament And Of The Council Of 27 April 2016 On The Protection Of Natural Persons With Regard To The Processing Personal Data And On The Free Movement Of Such Data And Repealing Directive 95/46/Ec (General Data Protection Regulation) (Oj Eu L 119 Of 04/05/2016, P. 1), Hereinafter Referred To As Gdpr.
