The Subject Of The Order Is The Provision Of Care Services By The Contractor, Including Specialized Care Services, Pursuant To: Art. 18 Section 1 Point 3 Of The Act Of 12 March 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended), Art. 8 And 9 Of The Act Of August 19, 1994 On Mental Health Protection (Journal Of Laws Of 2022, Item 2123, As Amended) And In Accordance With The Scope Specified In The Regulation Of The Minister Of Social Policy Of September 22, 2005. Regarding Specialist Care Services (Journal Of Laws No. 189, Item 1598, As Amended), For Persons Indicated By The Ordering Party.2. Estimated Average Monthly Number Of Service Recipients: 25 People. Estimated Number Of Hours Of Services During The Contract Period: 5,400 Hours In Total.3. The Number Of Hours And People Covered By The Services May Change And Cannot Be Determined Precisely Due To The Specifications Of The Order, Which Is Characterized By Variability Of Customer Needs Due To Their Changing Health Condition. The Specified Number Of Hours Of Service Provision Constitutes Their Maximum Number, And The Ordering Party Will Pay For The Services Actually Provided.4. A Detailed Description Of The Subject Of The Order Is Included In The Description Of The Subject Of The Order Constituting Annex No. 1 To This Tor.
