1) Care Services Will Consist In Assistance In Meeting Everyday Life Needs, Hygienic Care, Care Recommended By A Doctor And, If Possible, Ensuring Contacts With The Environment For People Who, Due To Age, Illness Or Other Reasons, Require The Help Of Other People.1. The Subject Of The Order Is The Provision Of Care Services Specified In Art. 50 Of The Act Of March 12, 2004 On Social Assistance (Journal Of Laws Of 2023, Item 901, As Amended) For People Who, Due To Age, Illness Or Other Reasons, Require Partial Care And Assistance In Meeting Their Daily Life Needs From Other People And They Are Deprived Of This Help Or This Help Is Insufficient. 2. Services Will Be Provided For Approximately 25 Environments, The Number Of Which May Change +/- 25%. The Amount Of Care Services Is Estimated At 5,500 Hours Per Year And Will Be Determined By The Ordering Party Depending On The Needs Of The Community.3. The Ordering Party Anticipates The Possibility Of Failure To Meet The Hour Limit. The Ordering Party Shall Not Be Liable For Any Reduction In The Number Of Hours Of Services Due To Reasons Beyond The Ordering Partys Control.4. Care Services Will Be Provided At The Residents Place Of Residence On Weekdays, And In Special Cases On Saturdays, Sundays And Holidays.5. People Requiring Assistance Will Be Residents Of The Długosiodło Commune.6. Care Services Will Include: 1) Assistance In Meeting Everyday Life Needs, In Particular: - Assistance In Running The Household, - Assistance In Keeping The Rooms And Equipment Used By The Person Using The Assistance Clean By Performing Routine Cleaning, - Shopping For Groceries And Other Products Necessary In The Household, - Preparing Meals Taking Into Account The Diet Recommended By The Doctor, - Bringing Lunches In Accordance With The Issued Administrative Decision, - Assistance With Eating Meals, - Bringing Fuel And Lighting The Stove, - Dealing With Current Matters Depending On The Needs.2) Care Hygiene, In Particular: - Assistance In Maintaining Personal Hygiene (Washing, Bathing, Combing), - Washing And Changing Personal Underwear And Bed Linen, - Making The Bed. 3) Care Recommended By The Doctor, In Particular: - Prevention Of Pressure Sores And Sores, - Putting On And Changing The Absorbent Product In A Bedridden Person, - Putting The Patient In Bed And Helping With Changing Position, - Applying Care According To The Doctors Recommendations, - Feeding Or Assistance With Eating Meals, - Using Compresses, - Administering Medications According To Doctors Recommendations. 4) Ensuring Contacts With The Environment, In Particular: - Taking Care Of Psychomotor Condition, - Organizing Walks, - Organizing Contacts With Neighbors, - Ordering Doctors Visits, - Filling Prescriptions, - Dealing With Official Matters. The Scope Of Services Will Be Tailored To The Individual Needs Of The Client. 7. The Contractor Will Provide Services To A Specific Client In Accordance With The Procedures Specified Below: A) Information About The Granting Of Services, The Start Date And The Amount Will Be Provided By The Ordering Party By Phone Or E-Mail. This Information Will Be Confirmed With The Care Services Order Card In Writing Without Undue Delay, B) The Contractor Will Be Obliged To Collect The Care Services Order Card From The Ordering Party Before The Date Of Provision Of Services, C) In A Situation Where The Completion Of The Services Will Take Place Earlier Than Indicated In The Care Services Order Card, The Ordering Party Will Inform The Contractor About It By Phone Or E-Mail, And Then Confirm It With The Care Services Order Card. D) The Care Services Order Card Will Contain Information Necessary To Provide The Services, Such As: Name And Surname And Residential Address Of The Person Authorized To Use The Services, Hourly Number Of Services Per Week, Scope Of Services Provided, Duration Of The Services, Borne By The Person Entitled To Use The Services. Services Fee For One Hour Of Service. 8. The Hourly Dimension Specified In The Care Services Order Card Means The Time Of Actual Implementation Of The Ordered Scope Of Services. The Time Needed To Provide Services Does Not Include Travel To Or From The Clients Home. 9. The Frequency Of Services Provided And Their Scope Must Be Consistent With The Content Of The Care Services Order Card. In A Situation Where The Person Providing Services To A Specific Client Cannot Perform Them Within Specified Dates, E.G. Due To Illness, The Contractor Is Obliged To Provide A Replacement Each Time. 10. The Contractor Undertakes To Undertake Activities Related To The Provision Of Services Within The Time Specified In The Care Services Order Card, Except For Emergency Cases, In Which The Contractor Undertakes To Commence Activities Related To The Provision Of Services No Later Than Within 24 Hours From The Moment Of Notification Of The Need For Them. Implementation, And In Situations Requiring It, Immediately. 11. The Contractor Will Be Obliged To Collect Fees From People Using The Services For The Services Provided, In Accordance With The Payment Indicated In The Care Services Order Card. 12. Care Services Should Be Performed With Particular Care, Conscientiousness, Honesty And In Accordance With The Principles Of Respect For Human Rights, Dignity, Feelings And Faith. 13. The Provider Of Care Services Shall Inform The Commune Social Welfare Center In Długosiodło About Any Change In The Life And Health Situation Of The Patient That Affects The Provision Of Service Assistance As Soon As Possible.14. In The Event Of Difficulties In Collecting Payments From Patients For Care Services Provided, The Contractor Is Obliged To Inform The Ordering Party About This Fact. 15. The Contractor Undertakes To Submit The Settlement For Each Month Of Service Provision By The 10Th Day Of The Following Month In The Form Of An Invoice/Bill/Accounting Note With An Attachment Containing: A) A List Of Names Of Persons Covered By Assistance In A Given Month; B) The Number Of Hours Of Services Ordered In A Given Month. Month, For Each Ward;C) A Total Statement Of The Number Of Hours Actually Provided By The Ward And The Total Amount Due For Services In A Given Month;D) A Collective Statement Of Hours Of Services Provided In The Billing Month.16. In Particularly Justified Cases, The Ordering Party May Request The Contractor To Settle The Services Performed In A Given Month In Advance.17. If The Contractor Submits The Documents Referred To In Point 15) Prepared Incorrectly, The Ordering Party Will Have The Right, Until They Are Clarified, To Refrain From Paying The Remuneration Due To The Contractor. 18. The Contractor Undertakes To Submit Care Services Implementation Cards With The Signatures Of The Person Under Care To The Ordering Party For Inspection Each Month. The Current Worksheet Will Be Kept In The Residents Home To Ensure Control. After Each Visit Of The Guardian, The Work Card Will Be Certified By The Ward. 19. The Contractor Is Obliged To Appoint A Coordinator With Whom The Ordering Party Can Contact In Person, By Telephone And E-Mail.2) The Ordering Party Requires That The Offer Include All Elements Necessary For The Proper Performance Of The Subject Of The Order. 4) Settlements And Payments Are Specified In The Swz And The Provisions Of The Agreement. Failure To Include The Costs Specified In The Swz And The Provisions Of The Agreement By The Contractor In His Offer, As Well As Underestimation, Omission And Lack Of Sufficient Recognition Of The Scope Of The Subject Of The Order Cannot In Any Case Constitute The Basis For The Ordering Party To Incur Any Additional Costs At A Later Date.5) Conditions Settlement Of The Execution Of The Subject Of The Order: A) The Contractors Remuneration Is The Price For The Execution Of The Subject Of The Order, Resulting From Its Scope From This Tor. Any Error In Price Calculation Does Not Affect The Correctness Of The Offer Evaluation. B) The Offer Price Is The Amount Of The Total Remuneration Offered By The Contractor In The Offer Form.
