Prof. Bogusław Frańczuk Along With The Service Of Medical Gases. 2. The Order Covers Services Performed At The Małopolska Orthopedic And Rehabilitation Hospital. Prof. Bogusława Frańczuka, Al. Modrzewiowa 22 In Kraków.3. A Detailed Description Of The Subject Of The Order Is Included In The Contract Template (Including Annexes) Which Is Annex No. 4 To The Swz With Annexes. The Provisions Of The Swz Should Be Read Together With Any Changes To The Content Of The Swz, E.G. As A Result Of Responses To The Contractors Inquiries.4. All Contractors Employees (Referred To In Point 5.1.3 Of The Swz) Performing Duties (Acting As A Guard) Must Be Authorized To Handle Medical Gases. 5. The Contractor Will Be Obliged To Have An Insurance Policy Confirming That He Is Insured Against Civil Liability In The Scope Of His Business Activities For The Duration Of The Contract.6. In The Light Of Art. 95 Of The Public Procurement Law, The Ordering Party Requires The Contractor (Or Subcontractor) To Employ, On The Basis Of An Employment Relationship, Persons Performing Activities Related To Acting As A Guard/Porter And Handling Medical Gases, Because The Performance Of These Activities Involves Performing Work In The Manner Specified In Art. 22 § 1 Of The Act Of June 26, 1974 - Labor Code.
