Work Detail

1. The Subject Of The Order Is The Provision Of Services In The Field Of: 1) Rodent Control, 2) Disinfestation, 3) Disinfection And Deodorization, 4) Removal Of Insect Nests, E.G. Wasps And Hornets Or Relocation Of Bee Or Bumblebee Nests To Another Place, 5) Disinfection, Disinfestation And Deodorization After Death, 6) Disinfection And Deodorization After Flooding With Liquid Waste, Along With The Removal Of Remaining Waste And Their Disposal, 7) Removal Of Dead Birds, Rodents And Domestic Animals As Well As Their Waste (Including Feces, Bird Nests, Etc.) Along With Their Disposal And Disinfection, Disinfestation, Deodorization And Further Cleaning Of The Surfaces Subject To Treatment, 8) Removing From The Premises Food Products Abandoned By Users, Together With The Equipment In Which They Were Stored And Spoiled, Posing An Epidemiological Risk, Their Removal And Disposal, And Carrying Out Disinfestation, If Necessary, Disinfection And Deodorization Of The Room, 9) Disinfection Of Rooms, Equipment And Vehicles (Especially Cars) Infected With Viruses That Pose A Threat To Life And Health. 2. As Far As The Agents Used Are Concerned, The Ordering Party Requires That They Be Included In The List Of Biocidal Products Kept By The President Of The Office For Registration Of Medicinal Products, Medical Devices And Biocidal Products And That They Are Safe For Humans And Do Not Require Washing. The Products Should Be Used In Accordance With The Intended Use And Method Of Use Specified By The Manufacturer.3. The Services Commissioned By The Ordering Party Will Be Performed In: 1) Properties Managed By The Ordering Party (Internal And External Areas), 2) Premises Managed By The Ordering Party And Located In Properties Managed By Other Entities (In Particular By Housing Communities), 3) Properties Commissioned The Ordering Party To Provide Security That Is Not Managed By The Ordering Party (Internal And External Areas).4. Subject Of The Contract Specified In Section 1 Will Be Implemented On The Basis Of An Order Signed By The Manager Or An Authorized Employee Of A Given Building Operations Department In Accordance With The Template Constituting Annex No. 1. In Order To Confirm Receipt Of The Order, The Contractor Is Obliged To Return Its Signed Scan By E-Mail To The Ordering Party Within 4 Hours From The Moment It Is Sent By The Ordering Party. 5. In Case Of Failure To Meet The Deadline Specified In Section 3, The Order Is Deemed To Have Been Received And Accepted For Execution Within This Period. The Above Does Not Release The Contractor From The Obligation To Immediately Deliver A Scan Of The Order Signed By Him By E-Mail To The Ordering Party.6. The Ordering Party Provides For The Following Procedures For Ordering Services: 1) Periodic - Includes Pest Control Carried Out On The Dates Specified Annually In The Ordinance Of The Mayor Of Katowice On Carrying Out Rat Removal Campaigns In The City Of Katowice, In The Properties Mentioned In Section 2 Points 1 And 3. The Ordering Party Will Inform The Contractor About The Date Of The Service By E-Mail 7 Days Before Commencing The Service, 2) Ad Hoc - Includes All Types Of Services Listed In Section 1, And The Service Will Be Provided Within 3 Days From The Date Of Receipt Of The Written Order By E-Mail By The Contractor. The Date Of Receipt Of The Order Is The Date Of Its Effective Delivery By E-Mail, 3) Emergency - Covers All Types Of Services Listed In Section 1, And The Service Will Be Provided 24 Hours A Day, Seven Days A Week, Including On Public Holidays, Throughout The Duration Of The Contract, And The Service Will Commence Within 3 Hours After Notifying By Telephone (Confirmed By E-Mail) By The Ordering Partys Representative, In Accordance With With Annex No. 2.7. Emergency Orders Will Be Issued In Urgent Cases, In Particular: A) Posing A Threat To The Health Of Residents, Especially Children, The Elderly And Those With A Weakened Immune System (E.G. Discovery Of A Corpse In The Premises, The Condition Of Which Indicates Advanced Decomposition, Insect Nests In The Facade Building, Dead Birds And Rodents), B) Posing A Threat To Human Health Or Resulting In Significant Material And Financial Losses, E.G. As A Result Of Destruction Of Building Structure Elements, Equipment (E.G. Rodent Nests In Residential Premises, Chewing Of Electrical Cables, Destruction Of Furniture E.T.C.). 8. The Order Will Specify, Among Others: A) The Type, Place Of Service (Address) And The Area On Which The Service Is To Be Performed Or The Quantity In The Case When The Service Concerns Insect Nests, B) The Date Of The Service (Including The Start Date And Completion Of The Service, The Specificity Of A Given Property And The Scope Of Work Reported For Implementation), C) Remuneration For The Service Calculated On The Basis Of The Area (Or Quantity) And Rates Indicated In The Offer, A Copy Of Which Is Attached As Annex 3. 9. If The Order Will Apply To Outdoor Areas, The Area On Which The Service Is To Be Performed Will Be, E.G., A Garbage Box Or Other Facility To Which The Order Will Apply. If It Is Necessary To Use Rodent Traps Intended For Outdoor Areas, The Following Conversion Rate Will Be Used: One Trap - 10 M . 10. The Order May Cover One Or More Properties. If The Order Covers More Than One Property, The Ordering Party Will Attach An Appropriate Specification To The Order. The Ordering Party Will Issue Orders Separately For State Treasury Properties And Other Properties. 11. In The Event Of An Unjustified Refusal To Perform The Order In An Emergency Mode, The Ordering Party Is Entitled To Order Substitute Performance, Without A Separate Request, At The Expense Of The Contractor.12. The Ordering Party Envisages Using The Institution Of Option Right. 13. A Detailed Description Of The Subject Of The Order Is Specified In The Draft Provisions Of The Contract Together With The Annexes (Appendices No. 4 To The Swz). 14. The Contractor Undertakes To Perform The Subject Of The Order In Accordance With Applicable Law With The Utmost Care, Observing Occupational Health And Safety Rules (Employees/People Actually Carrying Out The Order Should Be Trained In This Respect) And Provide The Ordering Party With Access Via Telephone/E-Mail And Remain At His Disposal During The Ordering Partys Working Hours.15. All Obligations Of The Contractor And The Ordering Party As Well As The Conditions For Implementing The Subject Of The Order And The Method Of Settlement Are Included In The Draft Provisions Of The Contract Constituting Annex No. 4 To Swz.16. The Means Of Evidence In Question. The Ordering Party Will Not Request Their Submission/Supplementation.17. Employment Requirement Under An Employment Contract Employment Requirement Under An Employment Contract (Social Aspects): The Ordering Partys Requirements Regarding Employment Under An Employment Contract Are Included In 3 Of The Draft Provisions Of The Contract. (Appendix No. 4 To Swz).19. The Requirement To Meet The Requirements Specified In The Provisions Of The Act Of January 11, 2018 On Electromobility And Alternative Fuels (Journal Of Laws Of 2022, Item 1083, As Amended) Is Specified In 5 Of The Draft Provisions Of The Contract (Appendix No. 4 To The Swz).
