The Subject Of The Order Is The Delivery And Implementation Of Valuable Vouchers Or Gift Cards For Basic Food, Chemical And Industrial Products For Customers Of The Municipal Social Welfare Center In Zgierz, Ul. Długa 56 1. Material Scope Of Valuable Vouchers: 1.1. Printing And Delivering To The Ordering Party Valuable Vouchers In Denominations Of Pln 10, Pln 20 And Pln 50, According To The Order Placed 14 Days In Advance Specifying The Number Of Vouchers And Their Denominations.1.2 .The Ordering Party Anticipates The Delivery Of Valuable Vouchers In The Planned, Approximate Number: Pln 10 Each 667 Pcs Per Month Pln 20 Each 400 Pcs Per Month Pln 50 Each 40 Pcs Per Month The Actual Number Of Vouchers (In Individual Denominations) Will Be Determined By The Ordering Party During The Period Performance Of The Contract In Accordance With Each Order Placed. 1.3. The Above-Mentioned Coupons Should Have Appropriate Protection Against Counterfeiting. 1.4. The Required Minimum Validity Period (Possibility Of Redemption By The Mops Customer) Of Vouchers Is 3 Months. The Vouchers Remain Valid For The Indicated Period Also In The Event Of Termination Of The Contract. 1.5. The Cost Of Printing The Vouchers Is Covered By The Contractor. Coupons Damaged For Any Reason May Be Replaced With New Ones At No Additional Cost To The Ordering Party. 2. Material Scope Including Gift Cards: 2.1. Delivery Of Gift Cards To The Ordering Party According To The Order Placed 7 Days In Advance Specifying The Number Of Gift Cards. 2.2. The Ordering Party Expects To Deliver Gift Cards In The Planned Approximate Number: 200 Pcs. The Actual Number Of Gift Cards Will Be Determined By The Ordering Party During The Period Of Contract Implementation In Accordance With Each Order Placed. 2.3. The Above-Mentioned Gift Cards Should Have Appropriate Protection Against Counterfeiting. 2.4. The Required Minimum Validity Period (Possibility Of Redemption By The Mops Customer) Of Gift Cards Is 12 Months. Gift Cards Remain Valid For The Indicated Period Even In The Event Of Termination Of The Contract.3. If Defects Or Deficiencies Are Found In Vouchers Or Gift Cards, The Ordering Party Will Submit A Complaint To The Contractor, Which The Contractor Has 7 Days From The Date Of Receipt To Consider. 4. Method Of Redeeming Vouchers Or Gift Cards: 4.1. The Redemption Of Vouchers Or Gift Cards Awarded To Mops Charges Should Take Place Through The Contractors Own Chain Of Stores Located In The City Of Zgierz, At Least In 2 Commercial Establishments, Including One Located In The City Center . 4.2. The Contractor Will Designate And Indicate To The Ordering Party A Network Of Commercial Outlets In The City Of Zgierz, Which Will Be Obliged To Sell Products On The Basis Of Vouchers Or Gift Cards. 4.3. For The Purchased Vouchers Or Gift Cards, The Contractor Is Obliged To Provide People With Gift Cards Or Vouchers With Valuable Food, Chemical And Industrial Products, With The Exception Of Alcohol And Tobacco Products, In Its Commercial Establishments. 5. The Maximum Value Of All Vouchers Or Gift Cards During The Order Fulfillment Period Will Not Exceed Pln 200,000.00. 6. The Issuance Of Vouchers Or Gift Cards Will Take Place At The Ordering Partys Headquarters On Dates Agreed By The Ordering Party And The Contractor To An Authorized Employee Of The Ordering Party. 7. Note: Persons Or Entities That Directly Or Indirectly Support The Aggression Of The Russian Federation Against Ukraine After February 24, 2022 Cannot Participate In The Public Procurement Procedure. Offers For The Award Of Public Procurement Cannot Be Submitted By Persons And Entities That Have Been Entered Into The List Of Foreigners Whose Stay On The Territory Of The Republic Of Poland Is Undesirable, Referred To In Art. 434 Of The Act Of 12 December 2013 On Foreigners And Persons Or Entities Against Which Sanction Measures Are Applied, Entered In The Register Available On The Bip Website Of The Ministry Of Interior And Administration.
